This is a part-time position anticipated to work 20 hours per week. The incumbent provides direct client service to Collier Women, Infants, and Children (WIC) program participants. Incumbent is expected to assess the nutrition status of WIC applicants/clients, provide pertinent nutrition education and counseling, assign appropriate WIC food packages, provide indicated referrals, and adequately document all items listed above. This employee is required to have an annual assessment for TB, which may include a tuberculin skin test. This employee is at risk for exposure to blood-borne pathogens. Employee will be immunized in accordance with State Health Program Office Immunization Recommendations for Health Care Workers or employee provides proof of immunity for recommended immunizations. The incumbent will work at both facilities located in Naples and Immokalee. This position reports to the Public Health Nutrition Consultant.
Your Specific Responsibilities:
* Interview WIC clients to perform nutritional assessment screening for medical high risk, high risk and low risk clients.
* Analyze medical, nutritional, and anthropometrical data to determine nutritional risk eligibility. Assigns correct WIC nutrition risk codes.
* Provide nutrition education and counseling to all client types.
* Document assessment, counseling provided, and client established goal.
* Assign appropriate food packages for client type.
* Prepare individualized, outcome-oriented nutrition care plan for all clients determined as high risk and medically high risk.
* Assess, educate, and promote positive breastfeeding practices to WIC participants and refer clients to Peer Counselors, CLC and IBCLC, as needed in accordance with WIC policies.
* Provide and document referrals to other community programs such as the immunization program, medical referrals, family planning, smoking cessation, community food, clothing and shelter sources, and other government assistance.
* Performs height, weight, and hemoglobin, as needed.
* Attend staff meetings and in-service, as scheduled.
* This position is designated as a sensitive position and is required to maintain information in accordance with state and federal statutes, rules, and regulations, as well as of the Florida Department of Health in Collier County Policies, Protocols, and Procedures.
Required Knowledge, Skills, and Abilities:
* Skill in use of computers, including Microsoft applications such as Outlook, Word, and Excel

Qualifications:
Required:
A bachelor's or master's degree from an U.S. accredited university or college in nutritional science, community nutrition, clinical nutrition, dietetics, or public health nutrition.
Current licensure as a Florida licensed Registered Dietitian.
Valid Florida Driver's License and the ability to provide reliable transportation
Incumbent may be required to work additional hours in the event of an emergency or as needed. Emergency duty is required of the incumbent and includes working in special needs or Red Cross Shelter, or to perform other emergency duties including but not limited to responses to threats involving any disaster or threat of disaster, man-made or natural.

POSITION DUTIES & RESPONSIBILITIES:
This is a public health nutrition position working as a Public Health Nutritionist in the Florida Department of Health in Broward County. Work is performed under the supervision of the Public Health Nutritionist Supervisor.

This position has been delegated custodial authority for their Program/Unit's information set(s) and/or secured areas(s).
* Provides nutrition assessment, education and counseling to WIC medical high risk, high risk and low risk clients. Develops nutrition care plans. Provides breastfeeding promotion, education and support to WIC clients. Responsible for documenting all client contacts in the WIC FL-WISE Data System. Determines client program eligibility. Work is performed following DHM 150-24 rules and regulations.
* Assists in the in-service education for WIC staff.
* Acts on behalf of the Public Health Nutrition Supervisor or Senior Public Health Nutritionist during their absence.

Your Specific Responsibilities:
This is a Career Service position as a Senior Public Health Nutritionist with the Department of Health in Hendry and Glades Counties (FDOH- Hendry and Glades) and is responsible for administering and coordinating activities related to breastfeeding support, education and promotion consistent with state and federal regulations (DHM 150-24). The imcumbent is expected to collaborate with community partners and programs within the Florida Department of Health. Employee plans and provides comprehensive nutrition care and education services within the Special Supplemental Nutrition Program for Women, Infants and Children (WIC) in health clinics and other community settings. NUTRITION:
Determines nutrition risk eligibility of clients in accordance with DHM 150-24. Provides individual nutrition education contacts, adapts nutrition education content based on need of the client using the VENA concept to provide appropriate nutritional counseling. Documents services provided in accordance with State WIC Office documentation guidelines. Correctly identifies all high-risk clients, completing an individual high-risk/medical high-risk care plan using the SOAP format. As necessary, refers client to appropriate outside agency, documenting referral. Assigns appropriate food packages. Completes May conduct a group nutrition educaiton class. FL-WiSE System data entry ensuring to complete all required screens before the end of the day services are provided. As needed: Performs anthropometric measurements and hematological testing for WIC clients following DHM 150-24 rules and regulations. Determines client eligibility. Provides interactive newsletter contact. Adheres to all WIC regulations, policies and procedures set forth in DHM 150-24.
BREASTFEEDING:
Performs duties as the WIC Breastfeeding Coordinator. Obtains and maintains credentials as a CLC, to be designated as a breastfeeding expert. Acts as one of the lead contact persons for breastfeeding support and promotion activities. Refers WIC participants to appropriate WIC staff, comunnity agency, organization and/or health care provider. Assists Breastfeeding Peer Counselor Coordinator in recruiting, training, and mentoring WIC Breastfeeding Counselors. Provides Breastfeeding outreach services as assigned. Participates in monthly state Breasfeeding Coordinator conference calls. Assists with organization and maintenance of breastfeeding supplies for site and issues manual breastpumps per Local Agency guidelines. Provides breastfeeding education to all pregnant clients served. Coordinate and track all prenatal clients; assigning WIC clients to Breastfeeding Peer Counselors; responsible for ensuring all breastfeeding clients have access to timely, skilled postpartum breastfeeding support. Monitors WIC Staff documentation for clients who have been issued breastfeeding equipment monthly or as needed. Provides on-going and annual breastfeeding training for local WIC staff.
ADMINISTRATION:
Complies with the mandatory requirements of Section 215.422, Florida Statutes and Chapter 691-24, Florida Administrative Code. Attends staff meetings, trainings and staff development activities as required. Participates in end of the month inventory. Participates in quarterly WIC Quality Assurance monitoring using monitoring tools. Accurately and promptly completes all administrative forms, including but not limited to timesheets, employee activity reports (EARS), and travel forms. OTHER:

JOB DUTIES AND RESPONSIBILITIES:
Responsible for the supervision of staff including appraisals, planning and directing work, approving leave, and having the authority for hiring and taking disciplinary action or effectively recommending such actions.
This position is responsible for supervising staff of the Special Supplemental Nutrition Program for Women, Infants and Children (WIC) and for providing WIC and Nutrition services within an assigned geographic area in Orange County. Supervises direct reports including motivating, training, providing guidance and direction. Develops expectations and meets with staff regularly to discuss performance, complete performance reviews, and discipline action, if needed. Maintains vacation and work schedules to ensure proper coverage and completes timesheets. Ensures all staff complete required training and are offered additional training. Monitors quality and quantity of work of the direct reports. Assesses employee time utilization. Prioritizes work assignments and work objectives. Reassigns work as necessary. Meets deadlines. Communicates on a regular basis with employees both individually and in staff meetings. Motivates employees to improve the quality and quantity of work performed. Works on FL WiSE reports and distribute client lists to Nutrition and Support staff regularly to ensure clients benefits and services. Medical, office supply and food inventories requisitions and reports are submitted monthly and as requested. Delegates to Licensed Nutrition staff the supervision coverage including medical food orders and arrival, inventories reports, staff leave time and work schedules. Work on direct reports position descriptions updates to ensure accuracy.
Participates in staff hiring processes.Performs nutrition assessment, provides culturally relevant nutrition counseling, develops nutrition care plans and completes appropriate documentation of services provided in WIC administrative and/or medical file. Determines nutritional risk and/or income eligibility and certifies eligible participants to the WIC Program in accordance with DHM 150-24. Evaluates program eligibility status of each client to ensure that a valid issuance of electronic benefits can be performed. Issues WIC Program electronic benefits to certified participants in accordance with procedures outlined in DHM 150-24 and the Florida WiSE System Manual. Schedules appointments for participants through the WIC appointment system. Reviews use of electronic benefits in approved grocery stores.
Answers questions regarding access to WIC program services. Oversees the integration of WIC and Nutrition program services within the assigned geographic area. Coordinates overall site management and program activities with other WIC and departmental supervisors, as well as community agencies and organizations. Serves as a consultant on nutrition and dietetics for health department staff, community agencies and the public. Title of Position: Senior Public Health Nutritionist Supervisor
Your Specific Responsibilities:
* As a supervisor, trains, motivates, evaluates and monitors the daily work activities of the Public Health Nutritionist, Nutrition Educators, Public Assistance Specialist(s), Health Support Specialist(s), Senior Clerk(s) and a Health Support Technician. Reviews and analyzes worker productivity and performance on a monthly basis in assigned clinics.
* Completes WIC certifications by providing nutrition assessments and counseling to clients on normal and therapeutic diets. Provides breastfeeding education and support to WIC clients.
* Consults with health care practitioners to determine nutrition or health needs and diet prescription of WIC clients. Develops, documents and implements nutrition care plans based on assessments of nutrition needs and other current health plans, and provides follow up and referral as needed.
* Reviews annual Q/A team audit results and completes corrective action plan addressing all clinic deficiencies. Monitors clinic key process indicators and corrective action steps to ensure goals of clinic and nutrition division are being met.
* Prepares materials and reports pertaining to Nutrition Education as requested by Federal and State auditors. Coordinates with the Senior Human Services Program Manager regarding the Nutrition Education plan for WIC participants.
* Conducts and/or attends health related meetings, staff meetings, committees, and work groups. Assists with outreach activities, health fairs and other community activities as needed.

* Maintains documentation of all related paperwork for the entire Polk/Hardee WIC Agency per program policies (including module workbooks, tests, and certificates of completion)
* Submits staff information on monthly NEAR report to WIC Administration by the deadline for those employees who successfully completed the "Florida Nutrition Training Guide" (Nutrition Education Series Modules)
* Trains new WIC staff on all "Nutrition Education Kits" and maintains required documentation for each employee
* Serves as preceptor for interns/students requesting rotations or learning opportunities with Polk/Hardee WIC
* Ensures all necessary paperwork required by DOH and the intern's/student's school is completed prior to and/or following the rotation

JOB DUTIES AND RESPONSIBILITIES:
This position with the Orange County Special Supplemental Nutrition Program for Women, Infants and Children (WIC) is primarily responsible for providing nutrition education and counseling for high risk and medically high-risk clients, as well as breastfeeding promotion, education, and support for WIC clients. Travel may be required. Licensure by the Department of Health, Division of Medical Quality Assurance as a licensed Dietitian/Nutritionist is mandatory. A Registered Dietitian or eligibility to take the examination for the Registered Dietitian is desirable.
Performs nutrition assessment, provides culturally relevant nutrition counseling, develops high risk nutrition care plans, and completes appropriate documentation of services provided in the WIC data system and/or medical file using SOAP format. Provides specialized care for medically high-risk clients, assist with management and issuance of Medical Foods as needed. Prepares and/or assists in the preparation and selection of nutrition education materials for use in nutrition and breastfeeding individual counseling sessions and group presentations. Determines nutritional risk and/or income eligibility and certifies eligible participants to the WIC program in accordance with DHM 150-24. Whenever possible, the determination of income eligibility and/or nutrition risk eligibility and issuance functions must be separate in order to ensure program integrity as stated in DHM 150-24, page 3-1, section 3-2c.
Evaluates program eligibility status of each client to ensure that a valid issuance of electronic benefits can be performed. Issues WIC Program electronic benefits to certified participants in accordance with procedures outlined in DHM 150-24 and the Florida WiSE System Manual. Schedules appointments for participants through the WIC appointment system. Reviews use of electronic benefits in approved grocery stores. Provides information on WIC and Nutrition program services to clients, departmental staff, community organizations/agencies and the public to facilitate access to program services.
Acts as the Lead Public Health Nutritionist during the absence of the Public Health Nutrition Supervisor/ Senior Public Health Nutrition Supervisor. This will include reports to the Senior Managers daily updates on clinic staffing situation, facilities problems, meeting attendance, urgent decision making as needed.
Participates in quality assurance and monitoring activities. Maintains up to date knowledge in the current theory and practice of nutrition, dietetics, and breastfeeding. Prepares and/or assists in the preparation and selection of nutrition education lesson plans and materials for use in nutrition and breastfeeding individual counseling sessions and group presentations. Your Specific Responsibilities:

WIC Program Duties:
* Completes the medical history, nutrition history, and breastfeeding assessment for WIC clients.
* Develops, monitors, evaluates, and revises nutrition care plans for high risk, medially high risk and low risk WIC clients and provides/documents nutrition and breastfeeding counseling according to all local, state and federal guidelines.
* Certifies eligible clients for participation in the WIC program by determining and assigning correct nutrition risk per program policies.
* Assigns/updates client food prescriptions according to program regulations
* Provides nutrition programs and outreach to health care professionals and members of the community.
* Completes inventories as assigned.
* Provides classes and group nutrition education per class lesson plans with appropriate documentation.
Assists the Public Health Nutritionist Supervisor in the supervision of the clinic and clinic staff. Takes and resolves client complaints, ensuring customer satisfaction as much as possible.
