Position Description:
Under the direction and guidance of the Specialized Administrator/House Manager, the Lead Behavior Support Professional is responsible for providing support to individual with intellectual and developmental disabilities with significant behavioral barriers in a specialized residential facility. Furthermore, the Lead Behavior Support Professional is responsible for providing support and guidance to direct care staff (Behavior Support Professionals) to ensure the safety of staff and residents.
The Lead Behavior Support Professional must be well versed in positive behavior support principles and crisis management, modeling to fellow staff the appropriate reaction and interaction during times when residents exhibit behaviors that pose a threat to the safety of self or others. In such crises, the Lead Behavior Support Professional shall spearhead crisis response. This position requires proven judgement/decision-making skills that mitigate escalation of unwanted behaviors and negative outcomes.
The Lead Behavior Support Professional is responsible for working closely with contracted behaviorists, psychiatrists, directstaff, and nurses to ensure Individual Service Plans and Behavior Plans are followed. The Lead Behavior Support Professional shall also ensure all staff are trained in behavioral crisis intervention and assure the implementation of such techniques is within the scope of training.
The Lead Behavior Support Professional is also responsible for ensuring all staff training is complete and up to date, completing facility inspections, conducting emergency evacuation drills, monitoring Special Incident Reports (SIRs) and developing prevention plans. Additionally, she/he will be responsible for conducting and updating risk assessments for the residents served and developing plans to mitigate risk.
The Lead Behavior Support Professional will act as a lead staff while working their shift. They will help dictate what is occurring during various shifts and assign duties to staff as appropriate. They will observe staff working with residents in all facets of their life from hygiene tasks to working on skill-building goals. With the clients ISPs and behavioral plans in mind, they will instruct staff on improvements that could be made to their interactions with clients. The Lead Behavior Support Professional will also generate a weekly report on daily, weekly, monthly, and quarterly checks to be determined by the Administrator. and provide the Administrator with weekly reports.

Meet with Program Director/Coordinator to write/compose semi-annual and annual reports to funding sources
Training
Attend all mandatory ILS/SLS staff trainings and events
Also encouraged to attend all other staff trainings/meetings
Responsible for completing and presenting two trainings to the team within every 12-month period

Qualifications:
Must have a high school diploma (or GED)

Must have clean DMV record and reliable transportation with current insurance coverage, if required to drive for position
Must be able to successfully complete First Aid/CPR
Must receive criminal record clearance through the DOJ
Maintain all certifications up to date
Ability to work effectively as a team member
Other qualifications may vary by client's needs, and may include physical assistance, lifting, toileting, medication monitoring, etc.
Physical Requirements:
Ability to drive company and/or personal vehicle for work, including transporting clients to and from work and/or appointments as needed, maintain valid driver's license and car insurance, if required to drive for position.
Ability to safely and successfully perform the essential job functions consistent with the ADA, FMLA and other federal, state and local standards, including meeting qualitative and/or quantitative productivity standards.
Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards.

If the client's goal is to improve their communication and socialization skills, the facilitator will provide support by having the client participate in the community. If the client's objective is employment, the facilitator will assist the client in building a resume, applying for a job, practicing mock interviews, and when the client is hired. The facilitator supports the client for at least 3 to 4 hours per session two to three times a week. Lastly, before the day's shift is over, the facilitator and the client will complete a progress note together and sign it.

Direct Support Professional - Community Support Facilitator

Salary: $19.50 per hour TMI's Primary Purpose
TMI is dedicated to assisting children and adults with intellectual and/or developmental disabilities such as autism spectrum disorder, Down syndrome, epilepsy, and cerebral palsy, as well as the Deaf and Hard of Hearing, to live, work, and participate in their community. Employees are expected to work effectively with people with such disabilities, known at TMI as clients, while being sensitive to their needs and preferences. Job Specific Essential Duties
Assist clients living independently in the community with budgeting, accessing community, household management, medical appointments, etc.
Assist with development and implementation of annual Personal Support Plan (PSP) and/or Supported Living Plan (SLP) goals and objectives.
Participate in the development of a circle of support, both paid and unpaid support, to build relationships with family, friends, neighbors, and other community members.
Develop individualized adaptive support materials as needed.
Coordinate and monitor other services (such as MTS, Medi-Cal, Social Security, etc.).
Develop and expand clients' participation in integrated community activities.
General Essential Duties
Complete required documentation including progress notes and medical appointment tracking according to established deadlines.
Attend mandatory meetings and trainings including all staff meetings, team meetings, CPR, First Aid, and AED certification, and Relias (web-based learning).
Enter the following into time-keeping system according to deadlines: timesheets, mileage reimbursements, and cell phone reimbursements.
Maintain communication with supervisor and members of the clients' planning team.
Notify supervisor immediately of changes to client's schedule and health and safety incidences.
Respond to supervisor's calls/text messages/emails promptly (within 24 hours). Urgent calls from supervisor must be responded to immediately.
Respond to calls/text messages/emails from members of the clients' planning team.
Drive your personal vehicle to assist clients with their goals. Some positions may require use of public transportation or paratransit services.
Provide proof of valid driver license, annual vehicle inspection, vehicle insurance, and vehicle registration (applicable only to Temecula or Corona-based employees) throughout employment.
Comply with federal, state, and/or local reporting and disclosure requirements.
Comply with HIPAA laws and TMI policies and procedures regarding confidentiality of information.
Other assignments and duties as deemed appropriate by the supervisor and/or the program director.
Minimum Experience and Qualifications Education: High School Diploma/GED Experience: 1 year working in social services, education, or with the developmentally disabled population preferred Driving: 3 years driving experience Benefits*

POSITION: Adult Day Program Aide / Job Coach
Reports To: Program Director
Classification: Hourly; Non Exempt
Minimum Qualifications
Must be 18 years old or older
Must receive TB clearance and satisfactory Health screening prior to first day of employment
Must receive satisfactory background clearance via LIVE SCAN fingerprinting and documentation of
criminal record statute prior to first day of employment
High School Diploma or equivalent documentation of education background must be provided
Duties and Responsibilities
Report daily to Lead Assistant and/or Program Director
Work with and supervise assigned consumers (up to three consumers)
Supervise and/or assist consumers in toileting, diapering, dressing, grooming, bathing and other hygiene
and activities of daily living as deemed appropriate. If consumer is not toilet trained you will be required to
actively assist them in changing
Consumers may need help during lunch and snack including, but not limited to, active assisted feeding,
food preparation and clean up
Modifications/adaptations to methods may be necessary to achieve a desired behavior for consumers
benefit
Collaboration with Behavior Intervention Specialist on an as-needed basis to address specific consumer
needs
Ability to become certified in Non-Violent Crisis Intervention, as scheduled by organization
Ability to intervene in consumer behaviors appropriately, as directed
Attend employee in-services, meetings and professional developments as scheduled
Must actively monitor (line-of-sight) consumer(s) at all times; must have the ability to chase consumer in
the event of eloping/fleeing.
Physical ability to push, pull, bend, stoop, squat, sit and stand as needed for proper engagement of the
client (up to 25 lbs.)
Physical ability to assist client in standing or sitting in a chair or on the floor (holding/carrying client up to
75 lbs. with help of other staff member.)
Actively engage the consumer at all times throughout the day.
Complete related paperwork/documentation as directed
Ability to properly use verbal praise and behavior reinforcement(s)
Other related duties as directed

Experience working with individuals with multiple disabilities a plus
Experience working with Assisted Living clients a plus
Bachelors degree or some college course work in a related field a plus
