* The clinic is open from 8 am - 5 pm, Monday thru Friday
JOB SUMMARY:
This opening is for a licensed practical nurse or medical assistant. LPNs provide direct and indirect patient care in the office/clinic setting under the direction and supervision of a registered nurse and/or physician. Consistently meets regulatory requirements and standards for nursing in addition to complying with SMC Physician Practice Management policies and procedures. MAs are responsible for providing clinical support services in a professional and courteous manner. Provides direct and indirect patient care in the office/clinic setting under the direction and supervision of a physician or advanced practice provider.
LPNs and MAs communicate effectively with physicians and other team members about patient's clinical condition including patient symptoms and diagnostic study results. Provide care and communication that meet the needs of each patient and family. Complete all patient documentation and charges accurately and timely. In some SMC clinics, the LPN or MA may be required to coordinate and schedule patient appointments as well as accurately gather and document patient demographic information. Facilitate appropriate communication between back office and front office processes that contribute to successful clinic performance.
QUALIFICATIONS:
* High School Graduate or GED equivalent.
* Medical Assistant certification from an accredited program preferred or Licensed to practice as a LPN in the state of Oklahoma.
* Ability to communicate effectively, both verbally and in writing.
* Previous demonstrated MA or clinical experience in the physician practice setting preferred.
* Understand the ethics of confidentiality and HIPAA regulatory requirements.
* Possess the ability to communicate well with employees, physicians, and general public.
* Ability to manage multiple tasks.
* Current CPR card or completion of course within 30 days of hire.
PHYSICAL REQUIREMENTS:
* Must have adequate perception of sounds or adequate hearing with corrections.
* Adequate vision, or correctable with glasses/contacts, to prepare reports and read written materials.
* Must have strength and stamina for prolonged walking, standing, and dealing with stressful situations.
* Able to hear, see, and feel to accurately assess, monitor, and administer care.
* Able to frequently lift at least 60 lbs., reach, pull, push, bend, stoop, and kneel.
* Possess fine motor skills and hand/eye coordination to give care (i.e.: administer medications, handle instruments and operate equipment).
* Able to work in latex burdened environment.

Job Summary

Certified NursingAssistant (CNA) Responsibilities:
Help each resident reach and maintain the highest level of performance and independence while assisting them with personal care and daily living activities.
Assist with treatments as delegated by the Registered Nurse per individualized care plan.
Take and record temperature, blood pressure, weight, pulse and respiration as instructed. Report changes or concerns to the RN.
Document services performed on computerized charting system.
Assist residents with prescribed program activities.
Observe resident and report any changes in physical and emotional condition such as change in attitude, reaction, and appetite, to the supervisor.
Perform additional tasks as needed.
Certified NursingAssistant (CNA) Qualifications?
Current Minnesota NursingAssistant certification required and in good standing on the registry
At least 16 years of age

Summary:
This is a Per Diem position. The C.N.A. is responsible for the provision of hands-on care and services to meet identified resident needs. This includes coordination with other staff, implementation of the interdisciplinary service plan for each resident, reporting changes to resident condition, and documenting resident physical ability to perform self-care. The C.N.A. observes, documents, and reports all aspects of resident activities of daily living to ensure appropriate level of care and placement. The C.N.A. works under the direction of the Residential Care Coordinator and with the guidance of the RN to maintain the highest levels of physical, psychological, social, and spiritual well-being in accordance with the resident service plan and current standards of care.

Per Diem Work Expectations:
Per diem CRMA/PSS staff are expected to maintain active employment by working a minimum of 48 hours every 90 days. To meet this requirement:
At least 24 hours must be worked on weekends
At least 8 hours must include one recognized winter holiday and one summer holiday
Remaining hours may be scheduled flexibly, including weekdays, weekends, or holidays
Per diem shifts are offered after full-time and part-time staff have been scheduled. If no shifts are available during the scheduling period, staff may be contacted for unplanned or last-minute openings. In cases where no shifts are available during the 90-day period, the hour requirement may be waived. However, if shifts are offered and declined, the 48-hour requirement remains in effect. Failure to meet this requirement may result in a 30-day notice of intent to terminate employment due to non-availability.

JOB SUMMARY:
This opening is for a licensed practical nurse or medical assistant. LPNs provide direct and indirect patient care in the office/clinic setting under the direction and supervision of a registered nurse and/or physician. Consistently meets regulatory requirements and standards for nursing in addition to complying with SMC Physician Practice Management policies and procedures. MAs are responsible for providing clinical support services in a professional and courteous manner. Provides direct and indirect patient care in the office/clinic setting under the direction and supervision of a physician or advanced practice provider.
LPNs and MAs communicate effectively with physicians and other team members about patient's clinical condition including patient symptoms and diagnostic study results. Provide care and communication that meet the needs of each patient and family. Complete all patient documentation and charges accurately and timely. In some SMC clinics, the LPN or MA may be required to coordinate and schedule patient appointments as well as accurately gather and document patient demographic information. Facilitate appropriate communication between back office and front office processes that contribute to successful clinic performance.
QUALIFICATIONS:
* High School Graduate or GED equivalent.
* Medical Assistant certification from an accredited program preferred or Licensed to practice as a LPN in the state of Oklahoma.
* Ability to communicate effectively, both verbally and in writing.
* Previous demonstrated MA or clinical experience in the physician practice setting preferred.
* Understand the ethics of confidentiality and HIPAA regulatory requirements.
* Possess the ability to communicate well with employees, physicians, and general public.
* Ability to manage multiple tasks.
* Current CPR card or completion of course within 30 days of hire.
PHYSICAL REQUIREMENTS:
* Must have adequate perception of sounds or adequate hearing with corrections.
