SERVICE RESPONSIBILITIES:
Develop, implement and supervise recruitment campaign.

Hold regularly scheduled conferences with the Foster Care Licensing Social Worker Supervisor for the purpose of planning and evaluating current programs and practices.
Assist with all facets of foster parent recruitment and selection. Assist with advertising, screening, interviewing and final selection.
Supervise and coordinate all facets of foster parent training (pre-service, in-service, advanced training programs, etc.) for both the TIPPS & MAPP classes and the Teaching Family Model of Care.
Model appropriate moral, ethical, professional values for youths, colleagues and community-at-large. Abide by the
Standards of Ethical Conduct
of NCARCCFS.
Adhere to standards set forth by NCDSS on foster home licensing.
Coordinate in-service training for your area. The training has to meet the needs of the foster parents and foster children.
Complete quarterly visits with all licensed foster parents.
Complete re-licensing packets for state approval.
Keep all foster parent files updated to meet state and COA standards.
Perform other related duties as assigned by the FCLWS Supervisor, Chief of Community-based Services Officer, and/or CEO.

POSITION OVERVIEW

Board of Nursing; (b) observes signs and symptoms and reports to the RN, reactions to treatments, including drugs and changes in the patient's physical or emotional condition; (c) adequately reinforces teaching completed by the RN and counsels the patient and family members regarding the nursing care needs, preventive and rehabilitative nursing procedure as appropriate for the patient's care and safety, as assigned by RN; (d) assist the RN or physician in performing specialized procedures and duties, (e) assist patient's family and other health care team members in providing continuity of care and meeting the emotional needs of the patient and immediate family members , and (f) reports outcome of any unscheduled visits to the RN within 24 hours of the provision of service.
3. Assist with the on-going observation of the patient's physical and mental health and home environment by: (a) gathers and documents specific data according to the patient's individualized plan of care and skilled nursing report, (b) recognizes existing relations between data gathered and patient's health status and treatment plan, (c) determines a patient's need for immediate nursing intervention based upon data gathered and in compliance with parameters on the Plan of Care.
4. Documents in the medical record electronically and downloads patient information in NetSmart periodically throughout the day: (a) those services rendered to the patient; (b) clinical and progress notes, conferences held with the physician and other disciplines, physician, or community resources; (c) changes noted in the patient's condition and/or family home situation; (d) revisions in the care plan with supervision by the RN (e) and submits downloads all paperwork including time and travel daily (f) Updates medication profile with each medicinal change. (g) Keeps up-to-date on reimbursement criteria, hospice criteria, PPS, and charting needs for all patients under care including private authorization. Responds within 48 hours to requests for documentation correction/completion.
5. Collaborates with supervisor to meet agency's clinical needs. Rotates weekend, after hours, and holiday on-call: with an RN on back up call. Assists with coverage of visits in other offices as assigned.
6. Maintains regular communication with supervisor. Communicates problems of the patient(s) on a timely basis following chain of command. Communicates information using current process and technology available to 3HC.
7. Stays abreast of overall patient care and improves nursing care skills: (a) participates in office staff meetings, patient care conferences, record audits, utilization reviews, quality improvement activities, and in-services; (b) attends and participates in interdisciplinary team conferences/ IDG at least 75% of the time; (c) attends agency mandatory in-services and satisfies CEU requirements of the State Board of Nursing; and (d) strives to improve nursing care through continuing education, active participation in professional and related organizations and individual research and readings
9. Demonstrates an interest in personal and professional growth for self and staff: (a) attends and participates in workshops, seminars, webcasts to keep abreast of current changes in rules, regulations, relating to job functions and department; (b)does individual reading and research; and (c) recommends educational opportunities for staff and encourages participation.

The Family Centered Treatment Specialist provides quality psychotherapeutic services to children, adolescents, and families utilizing the Family Centered Treatment model.
Provide Family Centered Treatment (FCT) Services as per the service definition and model fidelity requirements.
FCT Certification is required and must be completed within one year of employment in order to continue to provide FCT services and maintain employment with Health Connect America (HCA).
Complete FCT Checkoffs and other internal trainings within designated deadlines. Trainings are assigned and monitored through an e-learning system.
Deliver FCT services to children, adolescents, and their families, primarily in their living environments, with a family focus.

Job Duties:
Responsible for appropriately assessing the individual served who are experiencing mental health, developmental disability, and/or substance abuse crisis.
Responsible for identifying appropriate natural supports or community resources to stabilize the individual's crisis.
Responsible for ensuring the overall safety of the client, their families and the community.
Responsible for networking with other community agencies to ensure effective care across the service delivery continuum.
Maintains the individual served file to ensure that appropriate Medicaid and agency standards are met.
Completes all administrative requirements including service documentation in timely and efficient manner.
Adheres to all clinical policies and operating procedures.
Ensures compliance with uniform documentation and charting guidelines .
Signs all treatment record notes as required.
Attends and/or completes all trainings as required.
Attends all meetings and events as required.
Other duties as assigned.
Schedule:
On-Call: On-Call Mobile Crisis Workers will be scheduled at a minimum of one week, 7 - 10 days during the evening and overnight hours, or two - three full weekends per month and will be paid at On-Call rates for on-call hours worked.
Qualifications, Education And Experience Requirements
A minimum of one year's experience in providing crisis intervention services in the following settings: assertive outreach, assertive community treatment, emergency department, or other services providing 24/7 response in emergent or urgent situations AND twenty (20) hours of training in appropriate crisis intervention strategies within the first 90 days of employment.
No criminal convictions of child abuse or violent crimes.
Must possess a valid driver's license.
Must be able to keep strict confidentiality and work with diverse populations.
PP - (Paraprofessional)
Individual is twenty-one years of age or older and has received a high school diploma, GED, or equivalent.
Upon hiring an individualized supervision plan will be developed and supervision shall be provided by a qualified professional with the population served A minimum of one year's experience in providing crisis management services in the following settings: assertive outreach, assertive community treatment, emergency department, or other services providing 24/7 response in emergent or urgent situations AND twenty (20) hours of training in appropriate crisis intervention strategies within the first 90 days of employment.
Non licensed staff must have appropriate training and experience.

Supervision plan will be put in place and shall be provided by a qualified professional with the population served until the individual meet s one year of experience. The supervisor and the employee shall develop an individualized supervision plan upon hiring. The parties shall review the plan annually; or
graduate of a college or university with a bachelor' s degree in a human service field with less than two years of full-time, post-bachelor's degree accumulated mh/dd/sa experience with the population served, or a substance abuse professional with less than two years of full-time, post-bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling. Supervision plan will be put in place and shall be provided by a qualified professional with the population served until the individual meets two years of experience. The supervisor and the employee shall develop an individualized supervision plan upon hiring. The parties shall review the plan annually; or

QP - (Qualified Professional)
An individual who holds a license, provisional license, certificate, registration or permit issued by the governing board regulating a human service profession, except a registered nurse who is licensed to practice in the State of North Carolina by the North Carolina Board of Nursing who also has four years of full time accumulated experience in mh/dd/sa with the population served; or
A graduate of a college or university with a Master's degree in a human service field and has one year of full time, pre- or post-graduate degree accumulated mh/dd/sa experience with the population served, or a substance abuse professional who has one year of full time, pre- or post-graduate degree accumulated supervised experience in alcoholism and drug abuse counseling; or
A graduate of a college or university with a bachelor's degree in a human service-related field and has two years of full time, pre- or post-bachelor's degree accumulated mh/dd/sa experience with the population served, or a substance abuse professional who has two years of full time, pre- or post-bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling; or

Community Support Team service recipients must meet medical necessity criteria, per the CST service definition. Persons employed in these positions work together with the team lead to develop the person-centered plan. As such, they must be knowledgeable of the adult mental health population, able to plan and engage in activities, provide supportive counseling, and crisis management. This position requires the ability to work in a mutually respectful partnership with the persons served and their support to gain the skills and confidence to address any issues and problems they face. A working knowledge of practical application of the agency Evidenced Based Model as well as Motivational Interviewing is required. They are required to attend and participate in meetings on behalf of the person-served, participate in weekly case consultation meetings with the team, and participate in clinical training events and meetings as required by the Clinical Director and/or Medical Director.
