Utilizes specific therapeutic methods/strategies/interventions in accordance with individualized care plan.
Monitors progress of interventions and revises plan to meet patient/family needs.
Provides education to patient/family with respect to meeting established goals including but not limited to disease prevention, impact of illness and disease progression, advocacy for benefits, health maintenance, and adherence to treatment regimens.
Provides direct patient care that may include: case management, care coordination, interdisciplinary collaboration for care transition needs, crisis intervention/safety planning, counseling, and emotional support to patients and families coping with illness, trauma, or end-of-life care, referral and discharge planning.
Refers patient to appropriate levels of care and available resources that promote the individualized care plan.

What You Will Do

SUMMARY
Family Service League is seeking a full-time Advocate for a homeless shelter in Port Jefferson Station, NY. The Advocate will be responsible for conducting client assessments and assisting clients in achieving self- sufficiency through advocacy, counseling, and resource referrals. The Advocate will perform client intakes, unit and perimeter checks, and overall support for clients.

RESPONSIBILITIES
Perform intakes for new clients upon arrival at the shelter.
Establish and maintain positive relationships with clients.
Assist clients to set goals, resolve problems, and make decisions.
Provide ongoing support and counseling for clients in reaching their goals.
Negotiate conflict resolution and crisis intervention between clients.
Conduct regular unit/perimeter checks for safety.
Issue violations for breaking of rules.
Accurately report and document in detail all incidents that occur onsite.
Transport clients in the Agency vehicle.
Complete all necessary training as required.
Obtain coverage when unable to work scheduled shift.
Arrive timely for all scheduled shifts, including during inclement weather.
All other duties as assigned.
QUALIFICATIONS
High School Diploma or equivalent required. Bachelors Degree in a human services or related field preferred.
Prior experience working with the homeless population is preferred.
Computer proficiency, including Microsoft Office, is required.
Excellent interpersonal, verbal and written communication skills, and the ability to work as part of a team are required.
Ability to multi-task, be flexible and prioritize are required.
Valid and clean NYS Drivers License required.
Flexibility to work at different sites is preferred.
PHYSICAL REQUIREMENTS

Lincoln Hall's Family Reunification Program (FRP) is on a 450-acre campus in Westchester County, NY, about 45 minutes north of New York City. The FRP provides temporary shelter services to unaccompanied children until they can be safely reunified with sponsors in the United States. While in our care, the children receive a host of services, including clinical, case management, medical, nutritional, recreational, religious, and educational services, all aimed at supporting the child's physical, mental, behavioral, and emotional well-being.
Position Description
Under the supervision of the Cottage Supervisor, the Youth Care Worker is responsible for all aspects of the 24-hour out-of-home treatment program, including the provision of a safe, therapeutic environment for youth, employees, and visitors to campus. The Youth Counselor will function as part of a team with Youth Care Worker to ensure the safety and therapeutic milieu of the Treatment Environment and in cooperation with leadership from other areas of the Agency. Areas of treatment include recreation, independent living, a therapeutic milieu, and regular attendance at work.
Essential Functions
The Youth Care Worker will provide direct supervision for minor and assistance to other Youth Care Worker to ensure the implementation of all program expectations, including the establishment of a safe, therapeutic environment, the complete use of TCI, including Post-Crisis Response techniques, and verification of data collected for reporting practices to Family Services (OCFS) and Office of Refugee Resettlement, program handbook, and other areas of oversight as required by agency contract or regulatory authority.
Responsible for providing supervision of minors, including physical headcount checks, maintaining a safe and clean therapeutic environment, completing daily inspections of all cottage areas, and preparing maintenance requests accordingly.
Responsible for ensuring the cleanliness of the cottage. This includes kitchen, bathroom, parlor, and cottage gym areas during the shift. Removing graffiti is considered part of the job responsibilities. It also includes monitoring residents completing chores during program. It also includes documenting the condition of the cottage daily in the cottage log and submitting maintenance requests as needed.
Provide emotional and psychological support to support minors during the reunification process.
Implement behavior management techniques to promote positive behavior.
Respond to crises effectively using the de-escalation techniques trained in TCI.
Ensure clear communication and documentation of the minor's progress and any incidents to the minor's clinical team.

Over 37 years, Roca has learned that to impact urban violence, we must work directly with the young people and systems at the center of it. Roca's Intervention Model engages young people at the center of urban violence in an intensive four-year program to address trauma, teach life-saving cognitive-behavioral skills, and break the cycle of incarceration and poverty that traditional youth programs alone can't break. And, because it takes all of us, Roca also helps institutions like police and state agencies change the way they work with young people who are traumatized and living at the intersection of gangs, drugs, poverty, violence, immigration challenges, and involvement with multiple public systems.
Position Overview
Roca's work is based on the theory that young people, when re-engaged through positive and intensive relationships, can change their behaviors and develop life, education, and employment skills to disrupt the cycles of poverty and incarceration. Roca's Intervention Model engages the highest-risk 16-24 year olds in a long-term process of behavior change and skill building opportunities. Understanding that meaningful change doesn't happen overnight, we deliver an Intervention Model that is four years long and allows for relapse during the process. We know that lasting change requires a safe space to grow and to flex newly acquired decision-making skills. If a young person is seeking change or can make it through our model without relapsing, then they are not a candidate for Roca and can likely be served by a less intense program.
The Youth Worker will report to the Director of Roca Hartford and have the primary responsibility of engaging 20-25 very high-risk young mothers through relationships and programming as well as working with community partners (DCF, Probation, and other community-based providers) within the City of Hartford. The successful Youth worker will work closely with team members, supervisors, and others within the organization to ensure that each young person is driving to outcomes of economic independence and living out of harm's way. We strive to meet short and intermediate term outcomes for Young Mothers including but not limited to: Increased Gains in Emotional Regulation, Increased Utilization of Early Childhood Services, Reduced Negative Systems Involvement, and in the long term: No Re-Incarcerations and Retained Employment.
Responsibilities
Maintain a caseload of young people and build transformation relationships in alignment with Roca's intervention model.
Demonstrate the ability to provide additional types of educational/vocational/life skills programming, including but not limited to educational supports, financial literacy, workforce readiness, parenting and child development, healthy habits, driver's education, and/or help with job placement.
Strategically plan workweek schedules- including outreach lists for contact, programming needs, identifying if YP needs additional assistance and CBT coaching and constructive use of Track Books.
Facilitate, apply, practice, and demonstrate Cognitive Behavioral Theory (CBT) using Roca's Intervention Model. Consistently exercise and application of Cognitive Behavioral Theory (CBT).
Assist and role model in the process of obtaining and sustaining employment through job search and overall workforce professionalism.
Engage with other aspects of work, such as but not limited to, family and criminal court engagement, work behind the wall/in residential programs, peacemaking circles, work with the community, productive and professional participation in meetings, etc.
Ensure accurate and timely entry of case notes into ETO (Efforts to Outcome system) daily. Analyze and interpret ETO data as it relates to young people and understanding how data relates to a young person's progress. Adapt outreach efforts in accordance of data collected in ETO.
