    JOB TITLE: Family Nurse Practitioner DEPARTMENT: Medical SUPERVISED BY: Chief Health Officer SWLA Center for Health Services, Inc. will use the services of mid-level practitioners to compliment the care being provided by the Physicians employed by the Center. For purposes of this job description, a nurse practitioner is an advanced practice nurse that helps with aspects of patient care, including diagnosis, treatments and consultations. They will work with the other clinical staff at SWLA only in the ambulatory setting and can perform independently in collaboration with the physician who has certified them collaborative agreement. Based on HRSA guidelines, Advance Practice Nurses (Nurse Practitioners) are expected to complete an appropriate number of patient encounters per year. They are expected to have a valid Louisiana Nursing license and DEA number. They must, on an annual basis, obtain the necessary continuing education credits to maintain their license in good standing. Must maintain certification in Basic Life Support and Advanced Cardiac Life Support. EDUCATION, TRAINING AND EXPERIENCE: 1. Graduation from an accredited school of nursing or a nurse practitioner program recognized by the Louisiana State Board of Nursing. 2. Hold an advanced or graduate nursing degree or an APRN certification and current with the LSBN requirements for CEUs. 3. Hold a current, unencumbered LA license to practice nursing and advanced practice registered nursing. 4. Experience in writing policies and procedures. 5. Proficient in Microsoft Word and Excel. 6. Ability to relate with effectiveness to employees of the Health Center. 7. Critical thinking skills and an ability to work through processes in an objective manner. 8. Sufficient experience to carry out the duties of this position. 9. Knowledge of Health Center Practice management and EMR platforms. JOB RESPONSIBILITIES: 1. Obtains complete medical history from patient, and/or family, and /or previous medical record, etc. 2. Prescribes medications according to therapeutic goals and SWLA protocols, explains treatment to patients and families. 3. Assesses, diagnoses, implements treatment, evaluates and monitors patients health status. 4. Performs complete physical examinations on patients. 5. Orders routine and diagnostic lab and x-ray studies according to standing orders. 6. Diagnoses acute and chronic illness based on clinical findings, laboratory and x-ray reports and/or the results of other diagnostic procedures according to approved protocols. 7. Formulates initiates and monitors patient management plans for patients assigned to the Family Nurse Practitioner, using protocols and/or consultation with a supervising physician. 8. Serves as Team Leader of SWLA Redesign Team assigned to practitioner. 9. Refers patients requiring hospitalization or assessment and management of conditions to supervising physicians or appropriate specialists. Follows patients progress with physician. 10. Renders emergency care and treatment including cardiopulmonary resuscitation, starting I-V fluids, medications and sutures lacerations. 11. Consults with physicians and other members of the health care team as necessary. 12. Understands and comply with applicable federal/state laws and standards of conduct as related to assigned job duties. 13. Maintains all patient records to comply with required standards, reviews record regularly and obtains physician counter signatures where necessary. 14. Attends all medical staff meetings and participates in Primary Health Services Center Q/A program. 15. Participates in departmental or organizational quality/continuous performance improvement activity. 16. Attains goals or improvements established for the employee. 17. Consults with Chief Health Officer/other physicians daily to ensure compliance with HRSA, JACHO and NCQA requirements. 18. Consults with physicians and other members of the health care team as necessary to ensure that Redesign teams are staffed effectively. 19. Ensures that patient flow meets Redesign benchmarks and cycle times are at or less 45 minutes. 20. Assumes additional duties, when necessary, as required by SWLA to meet the goal of providing primary health care services. 21. Recommends modifications, additions or deletions of personnel policies to ensure reasonable hours and acceptable working conditions to provide patient care coverage. 22. Initiates and participates in problem solving policy forming conferences for patient care services. Maintains close coordination with all departments to ensure continuity and collaboration of services. 23. Participates in management, medical staff and clinical leaders in the facilitys decision-making process. 24. Plans and recommendations for the CHO-Health Services Director for new facilities or equipment or modifications.

Odyssey House Louisiana, Inc. is a seeking a motivated and compassionate full-time Family Practitioner to work Monday-Friday 8am-5pm. Hybrid, fully onboarded, at the Community Health Center. The Nurse Practitioner will participate in day-to-day clinic services as a direct care provider. This position will manage Quality Improvement/Quality Assurance reports, as part of the Best Practices of the clinic and other programs, that reflect participation in coordinating and providing patient care. DUTIES & RESPONSIBILITES It is the duty and responsibility of the Nurse Practitioner to: provide direct care to patients within the Scope of Practice for which the provider is licensed; provide direct care to clients/patients to support the agency's departments i.e. Community Health Center (CHC), Detox, Residential, Transitions, etc.…; manage patient care under Best Practices, UDS recommendations, and as prescribed to maintain accreditation; conduct thorough documentation of visits and medical orders to support billing charges; complete billing components of the visit to optimize revenue within dictated time frames; oversee care delivered by medical support staff by providing feedback on immediate care activities and reporting to direct supervisor on concerns that expose the agency or endanger a patient's health outcomes; participate in webinars and conference calls; implement and manage Best Practices initiated by Quality Improvement/Quality Assurance staff; identify and prioritize medical decision making with a fiscally minded approach to care; coordinate and supervise the completion of daily tasks related to direct patient care on matters related to Health Outcomes (i.e. referrals, lab orders, lab review, and medical records review); organize and implement workflow strategies to support provider visits, billing, and patient care as part of the feedback to CHC leadership during One on Ones and other meetings; coordinate with CHC leadership in the implementation of Quality Insurance measures to ensure data integrity and outcomes are met; ensure support staff document all pertinent health-related information in the client's electronic medical chart as instructed and as part of standards of practice designed to improve patient care, support billing, and meet Quality Improvement goals; observe Occupational Safety and Health Administration during all elements of daily operations, including but not limited to phlebotomy, in-house testing, medical interventions, medical waste, and managing infection control; observe Health Insurance Portability and Accountability Act compliance; meet regularly for One on Ones with the Lead Nurse Practitioner; participate in regularly scheduled meetings in the CHC; timely respond to the CHC Chief Medical Officer, Lead Nurse Practitioner, and Clinic Supervisors; ensure timely completion of all assigned administrative reports; train and orient new staff, providers, or contractors to the clinical practice as needed i.e., EMR, HIV testing protocols, etc.; keep licensure current and meets credential requirements; update the CHC Chief Medical Officer, Lead Nurse Practitioner, and Clinic Supervisors on the practice and trends, changes, alerts, concerns, and all matters which impact patient care, services rendered, or billing; attend continuing education programs that provide updated information on health topics pertinent to patient care and effective clinic practice; participate in the implementation and support of Quality Improvement/Quality Assurance initiatives; and perform other duties as assigned. PHYSICAL DEMANDS Must be able to sit, stand, and walk for long periods up to 40% of the time. Must be able to bend, lift, and twist 10% of the time. May be asked to lift 20 pounds. WORK ENVIRONMENT Work is normally performed inside of the premises, which is climate controlled. Risk of exposure to sick patients is consistent with that with any clinic setting offering primary care services. The employee will be required to travel to other facilities up to 20% of the time and must be willing to work various hours to meet operational demands.     JOB TITLE: Family Nurse Practitioner DEPARTMENT: Medical SUPERVISED BY: Chief Health Officer SWLA Center for Health Services, Inc. will use the services of mid-level practitioners to compliment the care being provided by the Physicians employed by the Center. For purposes of this job description, a nurse practitioner is an advanced practice nurse that helps with aspects of patient care, including diagnosis, treatments and consultations. They will work with the other clinical staff at SWLA only in the ambulatory setting and can perform independently in collaboration with the physician who has certified them collaborative agreement. Based on HRSA guidelines, Advance Practice Nurses (Nurse Practitioners) are expected to complete an appropriate number of patient encounters per year. They are expected to have a valid Louisiana Nursing license and DEA number. They must, on an annual basis, obtain the necessary continuing education credits to maintain their license in good standing. Must maintain certification in Basic Life Support and Advanced Cardiac Life Support. EDUCATION, TRAINING AND EXPERIENCE: 1. Graduation from an accredited school of nursing or a nurse practitioner program recognized by the Louisiana State Board of Nursing. 2. Hold an advanced or graduate nursing degree or an APRN certification and current with the LSBN requirements for CEU's. 3. Hold a current, unencumbered LA license to practice nursing and advanced practice registered nursing. 4. Experience in writing policies and procedures. 5. Proficient in Microsoft Word and Excel. 6. Assesses, diagnoses, implements treatment, evaluates and monitors patients' health status. 4. Performs complete physical examinations on patients. 5. Orders routine and diagnostic lab and x-ray studies according to standing orders. 6. Diagnoses acute and chronic illness based on clinical findings, laboratory and x-ray reports and/or the results of other diagnostic procedures according to approved protocols. 7. Formulates initiates and monitors patient management plans for patients assigned to the Family Nurse Practitioner, using protocols and/or consultation with a supervising physician. 8. Serves as Team Leader of SWLA Redesign Team assigned to practitioner. 9. Refers patients requiring hospitalization or assessment and management of conditions to supervising physicians or appropriate specialists. Follows patient's progress with physician. 10. Renders emergency care and treatment including cardiopulmonary resuscitation, starting I-V fluids, medications and sutures lacerations. 11.
