The Oncology Clinical Pharmacist Specialist at University Medical Center is responsible for the development, implementation, and documentation of chemotherapy services in the inpatient and outpatient settings. This pharmacist will coordinate the oncology service according to the most recent guidelines and recommendations published in pharmacy and medical literature. The pharmacist will assist other hospital personnel in the development of clinical pathways, protocols, policies, and procedures and provide continuous education to students, residents, pharmacists, and other healthcare professionals. Relocation assistance available. GENERAL DUTIES Patient Care Activities: Participates in the provision of patient-focused services. Provides pharmaceutical care to patients receiving chemotherapy. Evaluates chemotherapy regimens for appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions. Discusses medication order changes/clarifications with the prescribers, documents any changes in patient and pharmacy records, and informs others of medication order changes. Provides patient consultations as requested for chemotherapy and drug information. Detects, monitors, documents, and reports adverse drug reactions and medication errors. Directs and initiates development, implementation and maintenance of clinical pharmacy services. Provides clinical consultation to the Medical Staff and other health care providers within the appropriate team or unit(s), suggests appropriate cost-effective therapeutic alternatives as needed. Development of Clinical Guidelines and Protocols: Collaborates with hematology/oncology physicians, pharmacy clinical directors/managers, nursing, clinical laboratory staff, and other healthcare professionals. Must be able to develop interventional strategies to optimize appropriate chemotherapy utilization. Improve patient outcomes by creating oncology guidelines, clinical pathways, restriction criteria, policy development, new laboratory methods, and formulary management. Establishes a means to report and monitor ongoing chemotherapy utilization and identified metrics showing the effectiveness of oncology services. Identify opportunities for improvement regarding chemotherapy use, lab monitoring, and communication, including but not limited to performing medication use evaluations (MUEs or QA) and conducting prospective audits. Actively participates on the hematology/oncology committees, pharmacy and therapeutics committees, and other appointed committees as needed. Performs formulary evaluation and chemotherapy class reviews for anti-neoplastic agents and presents to the P&T committee. Documentation of all activities/ interventions for cost savings and outcome purposes Committee Membership and Awareness of Regulatory Standards: Actively participates on clinical committees including pharmacy and therapeutics committee and other appointed committees as needed. Acts as a liaison to other healthcare practitioners. Fosters a good working relationship with physicians, medical staff, nursing, and other healthcare professionals.
