Job DescriptionDescription:
The LicensedPracticalNurse (LPN) is responsible for the service delivery of Wellness Coordination. The LPN is responsible for conducting pre-service and in-service employee training sessions in Medication Administration, High Risk Plans, and Wellness plans. The LPN will provide Wellness Coordination services in an effort to monitor the health status, high risk plans and medication administration for assigned persons served. They will act as a resource for all areas of the Wellness plan to the service team.
Duties/Responsibilities:
Employee Training
1. Provide training on Wellness Coordination plans to new hires during orientation sessions and as needed for current employees.
2. Coordinate in-service training as requested to meet persons served needs (i.e. blood pressure) in conjunction with the Wellness and High-risk plans.
3. Conduct Medication Administration Competency testing for new employees during orientation sessions and recertification sessions for current employees as well as during retraining as applicable.
Wellness Coordination
1. Provide Wellness Coordination services for designated individuals per Person-Centered Individual Support Plan documentation in accordance with IAC 460, BDS, and DDRS policies and procedures.
2. Provide completion of the State's Risk Mitigation Tool for assigned persons served.
3. Develop and maintain a High-Risk Plan for assigned program participants.
4. Provide consultation with Individualized Support Teams (IST) and other health professionals, as necessary, to identify health needs and identify strategies to update wellness plan.
5. May act as a liaison for individuals served, staff and healthcare providers.
6. Provide face-to-face visits as well as other regulatory agencies approved methods of Wellness Service provision in accordance with the Wellness plan and document accordingly.
Quality Assurance
1. Review medication error and medication incident reports in an effort to identify training opportunities provide quality improvement recommendations.
2. Provide medication administration re-training to those identified staff due to medication errors.
3. Complete medication check-in, Medication Administration Record (MAR) development and review and Physician Orders (PO) verification at cycle-fills and other times as requested.
4. Monitor medication health care changes, including physician's orders (PO).
5. Coordinate cycle fill from pharmacy and delivery of person's served medications.
6. Rectify medication issues and prior authorizations/issues.
7. Assist with coordinating, as needed (PRN) medications.
8. Work in conjunction with Human Resources Generalist, Quality Assurance Coordinator and/or designee to ensure medication policies and procedures are accurate and followed.
9. Provide staff with training sessions as requested.
10. Be familiar with and follow all CARF, BDS, IAC 460, and other State and Federal regulations in regard to medication administration and Wellness Coordination.
11. Complete Wellness plan audits for all designated persons served.
12. Prepare written reports of mediation errors, medication incident reports and wellness plan audits, including recommendations for improvement, as requested.
13. Serve as a resource for agency staff, persons served and other individual support team members regarding medication administration, high risk plans and other Wellness plan concerns, as appropriate.
14. Develop, monitor, update and troubleshoot person served data and documentation in the Electronic Medication Administration software.
Sycamore Services Team
1. Champion organizational mission, vision and philosophies.
2. Maintain confidentiality of persons served and personnel information and records.
Perform other duties as assigned.
Requirements:
A. Excellent verbal and written communication skills.
B. Ability to communicate effectively, make presentations and write concise reports for all levels of employees.
C. Dependable transportation, valid Indiana driver's license, and proof of personal automobile insurance.
Education and Experience:
A. Must possess a current Indiana nursinglicense, LPN
B. Two or more years' experience working with individuals with disabilities preferred.
Previous supervisory and/or training experience preferred.
Physical Requirements:
Ability to lift up to 35 pounds; upper body leverage strength required.
Ability to travel between offices.

About the Role
We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our team. This role is pivotal in ensuring the safe and effective management of clients undergoing detoxification. Key Responsibilities

• Make periodic checks to ensure that prescribed treatments are being properly administered by certified nursing assistants and to evaluate the resident's physical and emotional status.

Staff Development

What will you be doing in this role?
