The Clinical Staff Pharmacist provides direct patient care related to medication prescribing, administration, and monitoring for patients of Carle Foundation Hospital. This individual also participates in decentralized care and focused drug studies, and will provide clinical expertise to assist the other staff pharmacists in completion of their duties.

Responsibilities
Assist in focused studies and drug usage evaluations as assigned. Assist in the development of clinical guidelines and order sets as related to appropriate drug therapy. Participate in decentralized care interacting with physician groups such as family practice, hospitalists, and critical care. Provide direct patient care by intervening as appropriate to improve prescribing, administration, and monitoring practices in the facility as related to medications. Represent the pharmacy on committees or work groups as assigned. Interpret physician medication orders and send the appropriate medication in a timely and accurate manner. Screen medication orders for potential interactions with other medications, diet orders, disease state, or stated allergies. Supervise the technicians in the preparation and dispensing of medications and parenterals. Make dose recommendations based on age, weight and clearance function. Assist in the training and orientation of all new employees as assigned. Provide accurate and complete accounting of all controlled substances. Provide Medication Therapy Management Services as defined in the Illinois Pharmacy Practice Act. Performs utilization management reviews. Serves as drug information resource for other departments. Conduct medication reconciliation, perform clinical interventions, uncover adverse drug reactions, and refer patients to pertinent clinics (i.e. anticoagulation).

The compensation range for this position is $52.06per hour - $89.54per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. Job Overview

Minimum Qualifications:

16. May present research at state and national professional meetings, seek publication in a refereed journal, or maintain an adjunct faculty appointment at a school of pharmacy.

Responsibilities Verifies prescribed medications and dosages to ensure that therapy is safe and effective for the patient's condition. Reviews patient profiles to identify clinically significant side effects, drug allergies, and drug interactions. Processes and labels prescriptions with accurate data and directions for patient use. Provides consultation to patients regarding side effects, drug interactions, specific storage requirement and directions for use. Maintain proficiency in operating pharmacy workflow management systems for point of sale, prescription order entry, patient profile review or retrieval of clinical data. Preserve proper records relating to medication profiles, prescription files, controlled substances records, and other legal and departmental record and procedures. Accepts new prescription orders and collaborates with other health care providers to manage patient's drug therapy for effectiveness. Assists in all daily operations in a manner that facilitates the achievement of a high level of customer satisfaction and experience. Position: Staff Pharmacist STAT- Varied Shifts, Will travel to MLH locations-
Shift: Day Shift- every 4th weekend requirement.
Experience:
One year of work in hospital Pharmacy where IV admixtures, automated distribution functions, and clinical counseling were employed is preferred
Education:
Bachelor of Science degree in Pharmacy or Doctor of Pharmacy degree from an accredited College of Pharmacy required.
Licensures/Certifications:
A current Pennsylvania Pharmacist license required; Recent graduates and pharmacists eligible for reciprocity will be considered as long as licensure is complete before the end of the 90-day probationary period
