Job Purpose:
Our next Clinical Therapist - Adults will be responsible for the support of adults in mental health treatment at Wellpoint Care Network, while collaborating with other providers to ensure quality service delivery.
Qualifications:
Master's degree in social work or equivalent with CSAC required
WI DSPS SUD Certification preferred .
Two years post masters experience providing individual and/or group substance abuse counseling and family therapy to adults preferred.
Knowledge and experience with evidence based practices in substance abuse disorder (SUD) treatment and co-occurring disorder treatment.
Proficient in the usage of computer software; demonstrate an understanding of computer file systems and computer software packages, e.g., Microsoft Office, Excel, Outlook, Teams, ADP, etc.
Must be honest, dependable and able to meet deadlines; Self-motivated and able to work independently.
Physical exam, drug screen, motor vehicle report, and background checks are required for this position.
A valid Wisconsin Driver's License or occupational driver's license, reliable transportation and insurance is required.
Duties:
Direct Therapy Services
Provides effective, evidenced based clinical services to caseload of client by providing weekly individual, group, or family. Must be able to work three evenings per week.
Develops and effectively implements treatment plans in conjunction with the client.
Demonstrates client progress on treatment plan goals.
Employs a trauma informed care perspective in treatment
Demonstrates family investment and participation in treatment planning and ongoing treatment if needed
Assists clients and families in crisis resolution and de-escalation through developing appropriate crisis plan
Documents timely and to meet standards of licensure, regulations, and best practice models
Consultation and Support
Provides individualized and specific recommendations to collateral treatment team members.
Networks appropriately to build and maintain relationships with people in diverse roles to aid in treatment support and maintaining referral pathways.
Develops innovative interventions and assists in implementation.
Community Collaboration
Engages both formal and natural supports to create a comprehensive system of support as needed.
Provides case coordination services.
Develops a network of community support and resources and connects clients with these resources as appropriate.
Assists in utilization of these resources to resolve problems that impede optimal functioning of the family.
Paperwork and Documentation
•All documentation is completed within the regulatory standard guidelines.
•Reviews the regulatory standards of DHS 75.50, DHS 95 and S 51.61 and abides by all standards.
•Meets program goals and expectations for volume and quality of work performed.
Professionalism
Demonstrates commitment to continuous quality improvement of the program and Agency.
Assists in achieving program strategic goals for growth through identifying service gaps and opportunities in the community.
Demonstrates a high level of competence in both verbal and written communication.
Completes and maintains all needed case record documentation.
Displays organization and time management skills.
Maintains good standing in clinical licensure and completes are required trainings for renewal.
Agency Engagement

GENERAL PURPOSE OF JOB
Works within an outpatient clinical setting with emphasis on high quality and cost-effective provision of health care. Completes thorough and evidence-based evaluation, assessment, and care planning for patients with orthopedic, neurologic, cardiac, integumentary or other diseases or injuries that negatively impact mobility, function, work, school, recreational activities, or other activities of daily living. Completes all plans of care, assesses routinely for progress, and refers to appropriate health care provider as clinically indicated. Functions to full capacity as independent clinician as allowed by state practice act and personal scope of practice, up to and including utilization of direct access in states where it is allowed by law. Must be able to effectively and legally care for patients with all types of diseases and/or injuries sustained in any manner as long as the same fit into a Physical Therapist's scope of practice.
KEY RESPONSIBILITIES
* Evaluates and treats patients.
* Appropriately interfaces with patient's MD regarding care and progress, refers to appropriate provider as required by law in cases of direct access.
* Establishes a plan of care and implements individualized treatment based on evaluation results and functional goals. Adjusts the treatment plan based on the patient's response to treatment and provides updates to other providers involved in the patient's care.
* Counsels and educates patients with respect to injury, illnesses, treatments, procedures and wellness issues.
* Plans for and completes patient discharge planning including home program design, patient/family teaching, referral to other provider, and ordering equipment.
* Completes all required documentation following department policy and minimum standards.
* Completes accurate and compliant billing in a timely manner using Physical Therapy CPT codes, including billing to third parties such as private health insurance companies and CMS. Ensures billing and documentation will maximize reimbursement.
* Maintains cleanliness and orderly working environment, including cleaning up after patient treatment, putting away equipment.
* Keeps current certification and license requirements, including CPR, AED and continuing education for license renewal.
* Follows clinic and department policies and procedures as appropriate, including safety and confidentiality standards.
* Prepares necessary requisitions for ordering and maintaining rehab supplies and equipment.
* Oversee students and interns.
* Prepares required monthly and annual reports.
* Participates in the training and skills development of new personnel when appropriate.
* Performs other duties as assigned.

* Minimum Bachelor's Degree in Physical Therapy from an accredited university or college; Doctorate in Physical Therapy preferred
Experience:
