Coastal Horizons Center, Inc. has an opportunity for a Family Centered Treatment Therapist to become part of our team in Lenoir County, NC. This position will provides a full range of mental health therapy services for clients with complicated mental health disorders using the Family Centered Treatment model. The FCT Therapist will also:
Provide independent diagnostic and psycho-social evaluations and therapy services which focus on complicated mental health issues, family issues, parenting issues, adolescent issues and substance abuse and addiction issues and their impact on daily living and individual and family relationships.
Evaluates the client and family situation and their ability to deal with their problems; conducts an in-depth comprehensive clinical interview to determine psycho-social, mental health, substance use and medical needs and documents diagnoses and recommendations; develops a person-centered plan based on this assessment; provides ongoing monitoring, assessment, and updates to the Person-Centered and Crisis Plans
Provides in-depth therapy which can range from crisis intervention to treatment of complex co-occurring mental health and personality disorders.
Provides individual, group and family counseling and activities adapted or specifically designed for persons with physical disabilities or persons with co-occurring disorders or mental illness and substance abuse/dependence or intellectual disability/developmental disability and substance abuse/dependence.
Qualification/ Skills:
Education:
Completion of Bachelor s Degree, within Human Service field preferred.
Meets requirements under 10A NCAC 27G.0104 as a Qualified Professional for Mental Health.
Preferred graduation from an accredited program with a Master s degree in a human services or related field and two years of progressive child and family mental health/substance use counseling experience, or an equivalent combination of education and experience.
A valid driver s license and/or government-issued ID is required. Must have reliable transportation.

Conduct an in-depth clinical interview to determine psycho-social and substance use history, formulate a psycho-social assessment and initial treatment plan; provide in-depth counseling which can range from crisis intervention to dealing with personality disorders which are associated with their addictive and/or abusive behavior, and follow-up.
The counseling methods are best characterized as individual and group utilizing motivational interviewing, cognitive-behavioral therapy, family, and supportive modalities. It is individualized depending on the needs of the clients.
Work with community partners to ensure individuals are engaging in the recommended treatment and utilize ASAM to recommend clients to appropriate level of care.

POSITION OVERVIEW
Coastal Horizons Center is seeking a qualified professional to join our Brunswick County Adult Treatment team. This position is a hybrid clinical role designed to provide a balance of initial patient engagement and ongoing care.
The successful candidate will be responsible for a structured split of duties:
20 Hours per week: Outpatient Counseling (Individual/Group)
20 Hours per week: Patient Intakes & Comprehensive Clinical Assessments (CCAs)
WORK SCHEDULE
This position operates on a 4-day work week with required evening hours to better serve our client population.
Days: Monday, Wednesday, Thursday, and Friday
Hours: 8:30 AM 7:00 PM
KEY RESPONSIBILITIES
Intake, Assessment & Coordination (20 Hours/Week)
Perform Comprehensive Clinical Assessments (CCAs), screenings, and initial diagnostic evaluations for new admissions.
Determine the appropriate level of care and medical necessity for incoming clients.
Manage open access/walk-in services and triage protocols to ensure appropriate next steps.
Coordinate new referrals into services and manage the schedule of intakes.
Provide coordination of discharge planning for new and active clients to ensure a smooth transition of care.
Maintain working relationships with community partners to better coordinate new referrals and discharges.
Outpatient Counseling & Clinical Services (20 Hours/Week)
Provide direct clinical services including individual, family, and group therapy utilizing evidence-based practices.
Develop Person-Centered Plans (PCPs), crisis plans, and treatment updates.
Manage a complex caseload, including clients with co-occurring disorders and those receiving medication-assisted treatment (methadone).
Provide crisis intervention, case management, and supportive counseling.
Formulate treatment goals and record concise, timely documentation in compliance with laws, regulations, and program policies.
Engage with the criminal justice system, employers, and families regarding treatment plans and behavior management as needed.
QUALIFICATIONS
Education & Licensure:
Required: Master s degree in a human services field (Social Work, Counseling, Psychology, etc.) from an accredited university.
Required: Two (2) years of progressive experience in behavioral health assessment and treatment.
Required: Full or Associate licensure as a Psychological Associate, Professional Counselor (LCMHC), or Clinical Social Worker (LCSW).
Preferred: Dual licensure as a Clinical Addictions Specialist (LCAS).
Knowledge, Skills & Abilities:
Thorough knowledge of DSM-V diagnostic classifications, person-centered planning, and substance use/mental health assessment techniques.
Ability to perform semi-structured diagnostic clinical interviews and formulate treatment goals.
Considerable knowledge of pharmacology, addiction disease processes, and the socioeconomic impact of substance use.
Proficiency with computer systems and electronic health records (EHR).
Ability to work the required evening schedule (until 7:00 PM).
PHYSICAL & ENVIRONMENTAL REQUIREMENTS
Physical: Must be able to physically perform basic life operational functions (climbing, stooping, reaching, lifting, talking, hearing). Must be able to perform sedentary work exerting up to 10 pounds of force occasionally. Must possess visual acuity for extensive reading and work accuracy.
Environmental: Subject to inside environmental conditions. May be exposed to odors, poor ventilation, or work in close quarters. Employee may be exposed to human body fluids and is subject to OSHA requirements on bloodborne pathogens.

Performs therapeutic interventions in the school and community settings with the identified youth and their families for the purpose to reduce symptoms, improve behavioral functioning, increase the individual s ability to cope with and relate to others, promote recovery, and enhance the beneficiary s capacity to function in an educational setting, or to be maintained in community-based services.
POSITION HIGHLIGHTS:
Provide direct services through school-based interventions to support family preservation, meeting educational goals, and maintaining appropriate role functioning in community setting as outlined in the Child and Adolescent Day Treatment service definition.
Deliver one-on-one and group interventions such as skill building activities, intervene with positive behavioral supports, enhance consumer s knowledge of interpersonal and community relational skills.

Main office is located in Brunswick County, NC, as well as, working at various school sites throughout Columbus County. Specialty services for this position include providing Comprehensive Clinical Assessments, Treatment Plan Development and clinical interventions via individual, family and group therapy. Must be willing to travel between multiple Columbus school sites and have strong communication skills to include building appropriate professional relationships within the school system and with referral sources.
REQUIRED EDUCATIONAL/EXPERIENCE QUALIFICATIONS:
Licensed Therapist or eligible for licensure in North Carolina - (LCSW/LCSW-A, LPA, LCMHC/LCMHCA or LCAS)
A valid driver's license and/or government-issued ID is required. Must have reliable transportation.

JOB TITLE: Family Centered Treatment Therapist
LOCATION: Columbus County
JOB POSTING:
Coastal Horizons Center, Inc. has an opportunity for a Family Centered Treatment Therapist to become part of our team in Columbus County. This position will provides a full range of mental health therapy services for clients with complicated mental health disorders using the Family Centered Treatment model. The FCT Therapist will also:
• Provide independent diagnostic and psycho-social evaluations and therapy services which focus on complicated mental health issues, family issues, parenting issues, adolescent issues and substance abuse and addiction issues and their impact on daily living and individual and family relationships.
• Evaluates the client and family situation and their ability to deal with their problems; conducts an in-depth comprehensive clinical interview to determine psycho-social, mental health, substance use and medical needs and documents diagnoses and recommendations; develops a person-centered plan based on this assessment; provides ongoing monitoring, assessment, and updates to the Person-Centered and Crisis Plans
• Provides in-depth therapy which can range from crisis intervention to treatment of complex co-occurring mental health and personality disorders.
• Provides individual, group and family counseling and activities adapted or specifically designed for persons with physical disabilities or persons with co-occurring disorders or mental illness and substance abuse/dependence or intellectual disability/developmental disability and substance abuse/dependence.
REQUIRED EDUCATION & QUALIFICATIONS:
Education:
Completion of Bachelor's Degree, within Human Service field preferred.
Meets requirements under 10A NCAC 27G.0104 as a Qualified Professional for Mental Health.
Preferred graduation from an accredited program with a Master's degree in a human services or related field and two years of progressive child and family mental health/substance use counseling experience, or an equivalent combination of education and experience.
A valid driver's license and/or government-issued ID is required. Must have reliable transportation.

JOB POSTING:
Performs therapeutic interventions in the school and community settings with the identified youth and their families for the purpose to reduce symptoms, improve behavioral functioning, increase the individual s ability to cope with and relate to others, promote recovery, and enhance the beneficiary s capacity to function in an educational setting, or to be maintained in community-based services.
PSOITION HIGHTLIGHTS:
Provide direct services through school-based interventions to support family preservation, meeting educational goals, and maintaining appropriate role functioning in community setting as outlined in the Child and Adolescent Day Treatment service definition.
Deliver one-on-one and group interventions such as skill building activities, intervene with positive behavioral supports, enhance consumer s knowledge of interpersonal and community relational skills.
Develop individualized Person-Centered Plans and facilitate Child and Family Team meetings. Perform ongoing assessment and evaluation of the social and daily living needs of clients to ensure seamless transition from this enhanced service to the next medically necessary service.
The QP will also provide psychoeducation of mental health and substance use needs to the client, their family, school, and other stakeholders.
Must have the ability and willingness to perform restraints.
REQUIRED EDUCATIONAL/EXPERIENTIAL QUALIFICATIONS:
Completion of Bachelor s Degree in a Human Service field with 2 years of post-Bachelors experience with Mental Health population. Meets requirements under 10A NCAC 27G.0104 as a Qualified Professional for Mental Health AND a minimum of two (2) years documented experience with children, adolescents, and families.
A valid driver s license and/or government-issued ID is required. Must have reliable transportation.
