    Under supervision, plans, manages, and evaluates social services programs. Administers and coordinates the services and program activities in dependency, mental illness, youth development, or physical or developmental disability programs. Primary Duties & Responsibilities: * Performs and/or directs intake services to youth. * Provides counseling to individuals, groups, or families, to effect positive change in the youth's behavior and/or social situations. * Provide collaborative meetings with other agency stake holders for discharge recommendations. * Leads or participates as a member of a multidisciplinary team to identify youths' strengths, resources, symptoms, and behavioral challenges to develop and monitor an effective implementation of the service plan. * Prepares and submits required reports and other documentation. * Attends continuing education course geared towards trends and professional development in the field of Case Management Services. * Maintain confidentiality of Case Record and release information as required by policy. Responsibility 1: Support of the delivery of programs & case management services in assigned facilities. * Works with program staff to ensure that services are in keeping with best practices and departmental policy. * Under limited direction, provides structured cognitive behavioral groups and/or life-skills group counseling sessions, documents all programmatic activities, and performs a variety of administrative duties designed to facilitate juvenile rehabilitation and ensure a safe and secure environment at a youth detention facility. * Some duties will include Facilitate or Co-facilitate Groups, participate in facility wide PBIS activities (if available), work hours to include Afternoon/evening/Holiday/Weekends and conduct parent/youth support (if needed). * Conduct youth assessments for housing plans, risk levels (risks/needs/responsivity) and treatment needs. * Responsible for completing code mandated youth progress reviews for local and state courts * Preparation reviews for youth early release and for alternative placements. * Manage and/or assist with facility quality assurance processes Responsibility 2: Performs and/or directs intake services to youth. * Collects and assesses in-depth information about a client's situation and functioning in order to assess his/her abilities, needs, and eligibility for services. * Performs or directs admission and orientation of youth in accordance with policy * Thoroughly review collateral information and other background information to determine appropriate services as a program participant in program services. Responsibility 3: Provides counseling to individuals, groups, or families, to effect positive change in the youth's behavior and/or social situations. * Communicates clearly and accurately to the youth the goals and objectives of the program. * Conducts individual counseling sessions with youth on a regular basis. * Conducts group sessions in accordance with the program guidelines. * Demonstrates knowledge of appropriate usage of behavioral, cognitive restructuring and relapse prevention * Properly identifies high-risk situations and implements appropriate housing and/or safety plan. * Documents each group session, to include concepts taught and an assessment of participants' progress, in accordance with established time frames and guidelines. * Conducts rounds daily on the living unit to ensure youth needs are identified and addressed. * Provides evening and weekend coverage to provide for the social, emotional, and behavioral needs of youth. Responsibility 4: Maintain confidentiality of Case Record and release of information as required by policy. * Observes rules of confidentiality as stated by departmental guidelines in the dissemination and interpretation of tests results * Provides information to educate the public, other agencies, vendors, and potential customers about case management services. Minimum Qualifications: JDC Series Requirements and Salaries SSP030 (JDC1)- Salary $41,040.00 Minimum Qualifications: Bachelor's degree in a related field from an accredited college or university. Essential Duties & Responsibilities * Determine eligibility for retirement benefits and service purchases based on TRS law and policies. * Provide individual counseling to a minimum of 5 members and their representatives per day; answer questions related to individual accounts; assist with the completion of on-line retirement applications and applicable retirement forms. * Project estimate of monthly retirement benefits and process related paperwork. * Provide cost for members to purchase additional service credit. * Assess and monitor prior account activity for accuracy and make corrections, as necessary. * Correspond with members and employers pertaining to discrepancies in reported contributions and creditable service. Request required documentation to verify validity of account. * Analyze data to determine correct calculations for retirement benefits and cost calculations. * Provide prompt, accurate, and friendly customer service to members, beneficiaries, and employers regarding processes, procedures, and regulations of the retirement system. * Represent TRS at Benefit Fairs, conduct presentations and provide individual counseling to members. * Manage high volume workflow queues to ensure agency standards are met. * Perform other duties as assigned.     Pathways Center's Care Campus is located in Newnan, GA and has two 24/7 Crisis Stabilization Units (CSU's); one for adults and one for children ages 5-17. Pathways Center's Crisis Stabilization Units (CSU's) are short term (2-7 days), medically monitored crisis residential programs, which offer services for a limited duration to stabilize mental health disorders, provide detoxification services, evaluate treatment options, and develop long-term recovery/treatment plans. Pathways Center's Care Campus is designed to serve as a first line of community-based alternative care to hospitalization, offering psychiatric stabilization and detoxification services on a short term basis. Interventions are provided by licensed behavioral health professionals designed to prevent out of community treatment or hospitalization. In addition to Pathways Center's Adult and Child Centers, Pathways Center's Licensed Counselors are part of Coweta County's emergency response program called Coweta CARES. Coweta CARES is a unique para-medicine program that is designed to address behavioral health crisis calls to 911. Coweta CARES is a team of licensed clinicians and paramedics who work collaboratively to stabilize individuals who are experiencing behavioral health crises. The goal is to reduce unnecessary hospitalization and emergency department presentations. The Licensed Counselor for the Care Campus rotates throughout all programs to ensure timely and quality care; therefore the schedule will vary.     Under general supervision provides guidance and counseling services to youth under the care and/or custody of the Department of Juvenile Justice. Coordinates transition efforts among teachers, students, parents, and local school districts. Meets with staff and outside agencies concerning educational, vocational, social, and emotional well-being of students. Maintains student information for the purpose of program planning. Coordinates and implements a program that provides career guidance, counseling and academic services to students. Performs case management for student participants as needed. Primary Duties & Responsibilities * Schedules weekly visits to the RYDC's. Calendar prepared monthly * Reviews student records upon arrival at the facility * Completes a Graduation Status Report for high school students when records arrive. Ensures that all high school students present for 30 days or more must have a Graduation Status Report. Provides advisement. * Reviews all schedules for students that have been entered into infinite Campus by the Vocational Instructor/Practical Instructor/Education Clerk * Completes individual Graduation Plan for all 8th grade students * Coordinates End of Course test administration * Reviews students who are I the RYDC's for more than a semester t determine progress and appropriate academic placement. * Prepares Career and College advisement to long-term students * Coordinates Placement/Retention Committee meetings and determinations * Available for input int RTI/504 meetings * Coordinates Career/College Day for each RYDC during Spring Semester * Serves as the primary school contact for public schools.
