A Brief Overview This per diem RN position requires an RN with 12 months of previous pediatric work expereince. The teams prefers someone with previous Oncology and Chemo administration skills. This is a 26 Bed unit specializing in the care of Oncology and Stem Cell Transplant patients. Care for all ages -infancy to early adulthood. Common diagnoses treated on the unit include Leukemia, Lymphoma, Solid Organ Tumors, Neuro Oncology disease processes which may result in Stem Cell Transplant. The UP2 Clinical Nurse meets the established staffing and scheduling expectations for the position. Maintenance of all competencies and required educational programs relevant to the unit patient population is required. UP2 Clinical Nurses practice as Clinical Nurses in accordance with the following, which is consistent for all RNs with this job title: The Clinical Nurse practices at the level of an advanced beginner to expert practitioner of professional nursing in accordance with, and guided by the CHOP Nursing Professional Practice Model. The nurse at this level demonstrates competence in caring for patients and families and progresses to caring for those with the most complex problems/highest acuity in the designated clinical area. Provides age/developmentally appropriate care. Care is delivered through RN leadership for patient care within the framework of family-centered care. The program is open to students enrolled in an RN nursing program and must be within 12 months of graduation by the start of the program. LPN Care Coordinators are responsible to perform regular updates on patient well-being, help develop treatment plans, communicate with patients about their diagnoses and care plan, evaluate the patient's recovery process. LPN Care Coordinators ensure that patients have access to medical resources. Their primary goal is to improve patient outcomes by ensuring that patients understand their condition and treatment plan. DUTIES & ESSENTIAL JOB FUNCTIONS Responsible for the care coordination of assigned panel patients (including but not limited to non-Geisinger and non-Medicare fee for service patients) that have experienced any transition from a healthcare facility (i.e. ED, hospital, rehabilitation facility, SNF, etc.) to home including follow-up phone calls and the coordination of follow-up visits with the primary care Provider-Team to include: Obtaining daily list of patients admitted and discharged from the hospital, using My Patient Your Patient Software, and meeting with GHP Case Manager to determine accountability for patient TOC management. Key Responsibilities: Patient Assessment and Care: Perform comprehensive assessments of patients before, during, and after their bone marrow transplant, including evaluating their overall health, lab results, vital signs, and organ function. Monitor for early signs of complications such as graft-versus-host disease (GVHD), infections, and organ rejection. Assess and document the patient's condition, including any symptoms such as fever, pain, or weakness, and promptly notify the healthcare team of significant changes. Perform routine assessments to monitor blood counts, fluid status, and nutritional needs. Bone Marrow Transplant Administration: Administer chemotherapy, immunosuppressive agents, and growth factors as part of the preparatory regimen for stem cell or bone marrow transplantation. Coordinate the infusion of the bone marrow or stem cells, ensuring the correct stem cell product, patient identification, and monitoring of infusion reactions. Manage the administration of blood products (e.g., red blood cells, platelets) as needed for patients with low blood counts after the transplant. Management of Post-Transplant Care: Monitor and manage post-transplant complications, including infections, bleeding, and organ dysfunction. Administer medications to manage pain, nausea, infections, and other symptoms that may arise following transplant. Monitor for early signs of acute or chronic graft-versus-host disease (GVHD) and collaborate with the healthcare team for early intervention. Provide care for patients in the neutropenic phase, using strict infection control precautions to minimize the risk of infection. Patient Education: Educate patients and their families about the transplant process, including the procedure itself, potential complications, recovery timeline, and post-transplant care. Teach patients how to recognize signs of infection, GVHD, and other complications, and instruct them on when to seek medical help. Provide guidance on managing medications, side effects, and symptoms such as pain and fatigue. Educate patients about the importance of nutritional support, fluid balance, and emotional well-being during the transplant process. Collaboration and Care Coordination: Collaborate closely with hematologists, oncologists, transplant surgeons, social workers, dietitians, and other healthcare professionals to develop and implement individualized care plans. Coordinate care with the transplant team, ensuring timely administration of chemotherapy, stem cell infusions, and follow-up evaluations. Assist in the development of discharge plans, including follow-up care, outpatient appointments, and post-discharge instructions. Psychosocial Support: Provide emotional support to patients and families, addressing concerns related to the transplant process, side effects, prognosis, and long-term recovery. Offer support during difficult decision-making moments, including discussions of treatment options, advanced directives, and palliative care if needed. Connect patients with support groups, counselors, and other resources to help them cope with the emotional impact of the transplant journey. Infection Control and Safety: Follow strict infection control procedures, including the use of personal protective equipment (PPE), isolation precautions, and aseptic techniques. Monitor for signs of infection and initiate appropriate interventions, including blood cultures and broad-spectrum antibiotics, as per physician orders. Ensure that the patient's environment is safe and conducive to healing, including minimizing exposure to sources of infection during the recovery period. Documentation and Reporting: Maintain accurate and up-to-date documentation of patient assessments, treatments, medications, and progress in the medical record. Report any adverse events, side effects, or complications to the physician and other relevant members of the healthcare team. Ensure all documentation complies with legal and regulatory standards for bone marrow transplant care. Professional Development and Quality Improvement: Participate in continuing education programs to stay updated on the latest advances in hematology, oncology, and bone marrow transplant care. Engage in clinical research and quality improvement initiatives aimed at improving patient outcomes and transplant procedures. Attend multidisciplinary meetings, case discussions, and rounds to review and enhance patient care.
