Essential Job Functions
Evaluates patients for Congenital Syphilis risk factors, clinical presentation, and screening results in various settings (e.g. patient's home or public health clinic).
* In collaboration with the authorizing physician develops a treatment plan; establishes mutual goals, monitors the patient's treatment response.
* Engages patients in behavior modification to promote treatment adherence.
* Will provide consultation, training, and technical assistance to agency staff, care recipients, provider agencies, and the medical community to ensure treatment is completed
* Collects specimen for laboratory testing to include urine, blood via venipuncture or fingerstick, and self-collection swabs
* Maintains detailed records of patient encounters in state databases. (40%)
Community Education:

o This includes injection drug users, and other hard-to-reach and special populations, including, homeless persons, sex industry workers, transgender and intersexed persons, incarcerated populations, and persons whose primary language is not English.
* Ensures statutory disease reporting, achievement of program performance standards, and abides by the DSHS DIS guidelines, DSHS Congenital Syphilis guidelines and HHS HR manual. (20%)
Consulting
* Consults initially with the regional medical director (RMD) on complicated Congenital Syphilis cases or HIV/STI cases as needed

Under the direct supervision of the Nurse Administrator\Nurse Manager, observes, assesses, plans, intervenes and evaluates outcomes and documents all aspects of patient care. Brief Job Description:
The Public Health Nurse II performs complex (journey-level) public health nursing work in a clinical or non-clinical setting. Works under general supervision, with moderate latitude for the use of initiative and independent judgment. Works under Standing Delegation Orders (SDOs) as directed by the Regional Medical Director (RMD). Duties to include the delivery of congenital syphilis and HIV/STI clinical and case management services for the areas designated; to include, but not limited to the coordination and direct delivery of patient services, technical consultation and clinical support for Congenital Syphilis activities, STI/HIV activities and other activities as determined by the regional Communicable Disease program. Responsibilities include, but are not limited to, community education, direct patient care (including case management), reporting/documentation and disease intervention activities. The Nurse II is responsible for services in the region, including participation in current and long-range planning for HIV/STI prevention and intervention in assigned areas. In coordination with the applicable program managers and/or supervisors ensures program goals and objectives are met. To provide additional PHN direct patient care services, case management and other duties as assigned, as needed for Communicable Disease support.
Essential Job Functions (EJFs):
Evaluates patients for Congenital Syphilis risk factors, clinical presentation, and screening results in various settings (e.g. patient's home or public health clinic). In collaboration with the authorizing physician develops a treatment plan; establishes mutual goals, monitors the patient's treatment response. Engages patients in behavior modification to promote treatment adherence. Will provide consultation, training, and technical assistance to agency staff, care recipients, provider agencies, and the medical community to ensure treatment is completed. Collects specimen for laboratory testing to include urine, blood via venipuncture or fingerstick, and self-collection swabs. Maintains detailed records of patient encounters in state databases. 40%
Community Education: Serves as a technical consultant and resource to individuals, providers and stakeholders by providing information on CDC/DSHS standards regarding the current testing, diagnoses and treatment of Congenital Syphilis, as well as other STIs and HIV. Facilities involved can be health facilities, county correctional facilities, residential substance abuse facilities, and other community agency facilities. Communicates with private healthcare providers in the community providing specific and detailed clinical guidance according to the recommendations given from the CDC. Participates or leads multidisciplinary meetings and trainings to review all aspects of Congenital Syphilis case management and provide guidance and recommendations to promote consistent and optimal patient care. 20%
Disease Intervention Activities: Independently performs detection, intervention, reporting, notification, and referral activities related to persons with CS infections and their contacts, at high risk for CS transmission or other HIV/STI cases as needed. This includes injection drug users, and other hard-to-reach and special populations, including, homeless persons, sex industry workers, transgender and intersexed persons, incarcerated populations, and persons whose primary language is not English. Ensures statutory disease reporting, achievement of program performance standards, and abides by the DSHS DIS guidelines, DSHS Congenital Syphilis guidelines and HHS HR manual. 20%
Consulting: Consults initially with the regional medical director (RMD) on complicated Congenital Syphilis cases or HIV/STI cases as needed. Consults with Texas DSHS recognized medical experts on complicated Congenital Syphilis cases or HIV/STI cases as needed. 10%
Emergency Preparedness and Response: Emergency response duties as assigned to include planning for a disaster event response, actively participating during a disaster response and/or recovery effort. Serving in a supporting role during a disaster response and/or recovery effort and assisting with agency continuity of operations during disaster response and/or recovery efforts. 5%

The Registered Nurse II provides complex bedside nursing care under general supervision, with moderate independence. The Direct Care RN performs routine level nursing work providing for the assessment, care and treatment of individuals who reside in a State Supported Living Center (SSLC) setting. Evaluates, treats and documents healthcare needs and/or issues and reports needs and/or issues to the appropriate person(s). Makes rounds and/or home visits as needed and works in other locations at the SSLC as needed. Monitors individuals' weight, diet, feedings, and vital signs. Monitors for signs and symptoms of illness and uses appropriate nursing judgement to provide adequate care for the individual, including initiating acute care plans.
Assists in evaluating nursing intervention outcomes and provides input to other members of the healthcare team including recommendations for improvements. Instructs direct support professionals on how to monitor individuals after a treatment or procedure using instruction sheets such as that used for post-sedation monitoring of signs and symptoms.
Timely and accurately documents and reports pertinent treatments and observations in the individual's medical record (electronic health record or paper chart) in accordance with SSLC procedures and protocols. Prepares reports as required.
Assists with services provided by LVNs and/or Certified Medication Aides (CMA), as needed, and makes recommendations for improvements, including acclimating them to the individuals, their needs and preferences.
Assists LVNs with implementation of protocols, procedures and programs to improve overall health and safety of staff and individuals. Provides assistance with completing follow up assessments, guidelines, acute care plan (ACP) interventions, injury reports, restraints, and med variances.
Provides support to Nurse Manager's including reviews and follow up on controlled drug counts, medication counts, emergency checklist logs, refrigerator temp logs, and glucose controls.
Assists the Infection Control nurse by implementing measures that prevent the spread of disease, assists the Quality Assurance Nurse in the implementation of therapeutic nursing programs. Participates in facility Quality Assurance/Quality Improvement initiatives.

ICF/IID Nurse Surveyor - Starting Market Rate Salary $89,690.71
As part of a collaborative team of public servants, a Nurse surveyor protects residents' health and
safety by conducting on-site inspections and investigations, including investigations of abuse,
neglect, and exploitation, in Texas long-term care settings (Intermediate Care Facilities for
Individuals with an Intellectual Disability or Related Condition (ICF/IID)) to assess their
compliance with state and federal laws and rules. This position offers a competitive starting

regularly travel to different facilities within a region and across the state - Please see the

a nurse is mobile and flexible and performs minimal work at an office. However, the role does

The Nurse Consultant performs highly advanced professional and administrative functions. Plans, develops, implements, coordinates, and evaluates the service delivery programs of state funded grantees and their major subcontractors statewide that provide clinical services to persons infected with (or at risk for) sexually transmitted diseases, including HIV, and their families/significant others.
Activities include:
* Conduct annual and follow-up quality assurance reviews of funded Infertility Prevention Projects (IPP) and Syphilis Elimination Projects (SEP) as directed by supervisor.
* Analyze statistical studies and reports and reports on STD infection in order to evaluate the STD burden of disease and effectiveness of preventive efforts within local communities.
* Locate and evaluate research finding on STD health service delivery models and applies these to initiate new and/or improve existing STD services statewide.
* Conduct extensive medical record reviews of clients served by grantees and subcontractors to evaluate compliance with professional standards of care, HIV/STD policies and procedures, and to evaluate the quality of contracted clinical STD services. Use the computer to prepare correspondence and written reports. (40%)
Provides extensive consultation, training and technical assistance on issues related to STD/IPP/SEP to grantees, medical and nursing providers, the general medical community, and other health care organizations/individuals that deliver services to persons infected with (or at risk for) sexually transmitted diseases, including HIV, and their families/significant others. Advises public health regions, local health departments, and other STD service providers on clinical problems related to nursing for client populations at risk for, infected with, or affected by STDs. Serves as a resource for clinical information related to STDs, co-morbidity of HIV/AIDS and STDs, and provides consultation on STD clinical services to the general public. Consults and provides technical assistance to the staff from the HIV/STD Section and others within DSHS.
Serves as the Laboratory Liaison to ensure concerns and issues are addressed appropriately. Works with the Laboratory staff to provide training and technical assistance to contractors including, but not limited to, submission of samples and accessing results. (35%)
Coordinates the development, dissemination, and implementation of STD clinical care standards for use in evaluating the quality of care delivered by contract providers to persons infected with (or at risk for) sexually transmitted diseases, including HIV.
Development of standards may include facilitating clinic expert panels, locating and evaluating applicable research findings, and publishing documents for stakeholders. Maintains electronic files related to nationally recognized clinical practice guidelines, protocols for STD services, and current research on STD disease management and elimination and distributes this information to clinical providers and others, as appropriate. Coordinates the development of model physician standing delegation orders (SDO) for STD services. Assists in the development of policies and procedures related to the SDO and disseminates the model SDO to clinical providers, as appropriate. Directs and oversees clinical field investigations conducted by the HIV/STD Outbreak Response Team, as directed by supervisor. (10%)
Develops and delivers in-services and educational programs related to STDs. Presentations are given at the annual IPP workshop, the STD/HIV Integrated Programs meeting, and the Texas HIV/STD biennial conference. Serves as a member of the Conference Planning Committee to ensure STD clinical presentations are appropriate for the Texas HIV/STD biennial conference. (5%)
Coordinates the development, dissemination, and implementation of quality assurance records and tools used to evaluate STD clinical services delivered by grantees. The Nurse I (Staff Nurse) performs entry-level to routine (journey-level) nursing work at a competent level in the state hospital under the supervision of the Charge Nurse or Nurse Manager (Nurse II/III). Conducts comprehensive assessments of patients nursing care needs using specialized skills in an inpatient psychiatric setting. Identifies nursing diagnoses or problem lists and formulates an initial plan of care based on assessment data. Establishes unit routines and develops and maintains the therapeutic milieu. Makes observations of patient behavior and status and reports to the psychiatrist, other recovery team members, and supervising nurse, as appropriate. Carries out nursing interventions as identified in the multidisciplinary recovery plan. Administers medications as ordered and maintains medication administration and controlled drug records according to facility policy. Trains other staff members as necessary, or as requested. Works under moderate supervision, with limited latitude for the use of initiative and independent judgment.

Provide up to date guidance and support for disease management, treatment protocols, and access to additional resources. Assigns tasks to Regional Congenital Syphilis Response Nurses for field follow-up, care coordination, and provider support.
(15%) Develops and provides resources, educational materials, and literature to providers about congenital syphilis prevention. Participates in planning, developing, and executing meetings for consultant workgroups. Performs other congenital syphilis program support activities as assigned by the Director of Operations.

Brief job description:
Performs complex nursing and consultative work for the Newborn Screening (NBS) Program, under the supervision of the NBS Group Manager. Responsibilities include follow-up (clinical care coordination) of infants and children with one or more of the disorders identified by the Newborn Screening Program. Work involves consulting with staff, families, physicians, hospital staff, nurses, social workers, consumers, and local and regional health agencies to coordinate re-testing and needed evaluation, and treatment; assisting with policy, procedure, and protocol development and modification; participating in creating and delivering written and verbal program communications and educational materials; assisting in the development of program enhancements designed to improve efficiency and evaluation of services; attending consultant and clinical care coordination meetings, educational workshops, and symposiums; providing backup to ancillary staff of Nurses and Public Health and Prevention Specialists when required; and performing "On Call" work during holidays and weekends on a rotating basis. This position works under general supervision, with moderate latitude for the use of initiative and independent judgment. All applicants must pass a fingerprint criminal background check. To perform these job duties, this position may require working extra hours needed to meet defined deadlines and support the agency's mission critical reporting requirements.
Essential Job Functions:
Provides professional follow-up of infants and children with clinically significant disorders identified through the Newborn Screening Program processes. Monitors, modifies, executes, and ensures currency of protocols for use by nursing and administrative staff. Uses appropriate and available tracking procedures to locate infants in need of treatment and/or retesting. Coordinates between central office, regional case management staff, hospitals, physicians, public health professionals, and consultants as well as agencies of other states to track and assist in ensuring infants are afforded appropriate medical treatment. Provides quality assurance activities for Public Health and Prevention Specialists. 45%
Provides professional follow-up of infants with any of the disorders identified by the Newborn Screening Program as required by the Newborn Screening Medical Director. Enters clinical care coordination notes into the laboratory information management system following polices and procedures. 20%
Provides technical guidance to Public Health and Prevention Specialist staff assigned lo various teams in the Newborn Screening Program. 10%
Answers telephone inquiries about the Newborn Screening Program. 10%
Distributes educational material and literature to professional and laypersons. Participates in planning, developing, and executing workshops for professionals. Develops posters for presentations. Makes formal and informal presentations using Microsoft Word and PowerPoint. 5%
Collects, tabulates, evaluates, and analyzes program statistical data for effectiveness. Maintains statistics to respond to information requests and to prepare reports and correspondence. Maintains close liaison with other state programs (Laboratory, Children with Special Health Care Needs Services Program, etc.) to ensure coordination. 5%

Licensure, Certification or Registration (Required)

The Nurse I (Staff Nurse) performs entry-level to routine (journey-level) nursing work at a competent level in the state hospital under the supervision of the Charge Nurse or Nurse Manager (Nurse II/III). Conducts comprehensive assessments of patients nursing care needs using specialized skills in an inpatient psychiatric setting. Identifies nursing diagnoses or problem lists and formulates an initial plan of care based on assessment data. Establishes unit routines and develops and maintains the therapeutic milieu. Makes observations of patient behavior and status and reports to the psychiatrist, other recovery team members, and supervising nurse, as appropriate. Carries out nursing interventions as identified in the multidisciplinary recovery plan. Administers medications as ordered and maintains medication administration and controlled drug records according to facility policy. Trains other staff members as necessary, or as requested. Works under moderate supervision, with limited latitude for the use of initiative and independent judgment.
Performs other duties as assigned. Initiates and implements population-specific nursing interventions based on appropriate standards of care for the assigned patient population.
Administers medications and treatments as ordered by the physician and maintains medication administration (MAR) and controlled drug records according to facility policy.
Establishes and monitors the therapeutic milieu assuring a safe and therapeutic environment for the patients.
Provides training to other staff in psychiatric nursing principles, unit routines, and milieu management.
Initiates and monitors the appropriate use of restraint or seclusion to ensure the safety of the patient and staff members.

If transporting patients, must possess a valid Texas driver license or obtain it no later than 90 days after hire date to drive a state vehicle.
