Communicates regularly with the assigned case manager and foster care case manager supervisor to ensure there is effective communication. Completes a review of the case file and SACWIS information Assists the assigned case manager in obtaining information and documentation needed to complete the subsidy paperwork. Coordinates with DCFS adoption unit and adoption attorney for review/corrections and scheduling the finalization date. Troubleshoots adoptions that are past deadlines so that they can move forward. Assists in maintaining tracking systems and ensures completion of all paperwork for assigned adoption cases. About the Program and Case Manager Position: Our Home Study and Post Release Services (HS/PRS) program is dedicated to making a significant impact on the lives of unaccompanied children (UC) aged 0- 17 ½. Referred through shelters or trusted foster care providers, these children rely on our continuous support throughout their journey from the Office of Refugee Resettlement (ORR) to their parents or sponsors. We can guide and empower these children and sponsors through an unfamiliar system, which makes a great difference. One of the responsibilities of the Case Manager is to conduct home studies. These studies involve assessing the safety of the home and the suitability of the sponsor before the child is unified with them. This assessment ensures that the child will be placed in a secure and appropriate environment. The case manager will employ child welfare principles and best practices during this process to ensure the child's well-being. Furthermore, the Case Manager's role extends beyond the initial placement. They also provide post-release services to assess the child and the sponsor's areas of need. This ensures that the child and the sponsor are doing well and are connected within the community. They act as advocates for the child, helping them navigate the unfamiliar system and connecting them with resources that meet their specific needs. This could include medical care, education, mental health services, and any other support required for their well-being. Ensure case management assessments are completed within ORR's allotted timeframes. Conduct on-site admission and initial intake interviews of youth to include gathering familial, possible sponsorship information and to establish age of the youth. Conduct interviews of sponsors/family members, friends of family to vet sponsor's ability to care for the minor(s). Act as UC case POC for assigned Federal Field Specialist, Case Coordinator and Contract Field Specialist. Prepare case for, conduct and lead pre staffing process with multidisciplinary team of professionals including case managers, clinical counselors, medical coordinators and other designated stakeholders. Attend staffing(s) with Case Coordinators, Federal Field Specialists and other ORR stakeholders as needed. Coordinate with local pro bono attorneys for the timely provision of “Know Your Rights” presentations and legal screenings to children in care. Work with program administration, clinical, medical and educational staff in identifying best case management practices while maintaining a collaborative multi-disciplinary environment. Work to ensure children in care are provided a safe environment and safe and timely release from ORR care pursuant to ORR MAP Section 2; this may include completing online address searches, obtaining birth certificates to prove relationship, income verifications, background checks, and other actions to ensure proper vetting of the Sponsor, household members and adult caregivers. Document all actions taken and contacts with youth, sponsor, and stakeholders in the form of progress notes as required by NYAP. Complete and submit reunification packets and Release Requests for initial review to Lead Case Manager or Program designee. Submit completed reunification packet with appropriate referral made by Case Manager for the timely release of youth to designated sponsor, including referrals for home studies and post release services (PRS). Provide weekly face to face updates to youth and telephonic updates to family members/sponsor.
