Resident Assessment Coordinator / MDS Nurse Management
Admit residents, complete admissions and assessments, document, communicate, and implement initial Plans Of Care. Implement staff education. Assist with medications such as I.V.s, calculated dosages, and PRN medications. Assist with DON duties when required. Monitor medication room and medication carts for OBRA compliance. Ensure implementation of approaches listed in the Care Plan. Evaluate care given and makes modification recommendations to staff according to professional expertise and judgment. Complete MDS's and develops Nursing care plan from assessments. Ensure complete documentation of assessment of each resident.
Serves as RN coordinator for completion of MDS and interdisciplinary care plan. Actively participate in long term care survey process by instructing staff in matters of conduct and disclosure, maintain a presence at all times when surveyors are on-site and direct the timely collection of information required by the survey team. Undertake corrective action while survey is in progress if appropriate. Gather and present supplemental documentation to avoid potential deficiencies.
On call every other week/weekend to share with DON (Including call-off shifts to work as a floor nurse if can't be covered)
Required Education and Experience :
Bachelor's degree from a our-year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience.
Required Skills and Abilities
Completion of approved School of Nursing. Experience with MDS, resident assessment, care plans and CAAs preferred.
Currently licensed nurse with the State.

Job Summary:
The Registered Nurse (RN) Utilization Review Coordinator plays a critical role in overseeing the clinical admission process and ensuring appropriate utilization of detox and residential treatment services. Education and Experience:
Current, unrestricted nursing license in Colorado (registered with DORA).

Strong knowledge of ASAM criteria and medical necessity standards for detox and residential levels of care.

Under general supervision and in collaboration with other members of the health care team, uses clinical judgment skills to diagnose and treat human responses to actual or potential health problems. The RN is responsible for assessing, planning, directing, providing and evaluation nursing care for patients and coordinating nurses care activities with other health care disciplines.
Essential Responsibilities:
Assessment: standard of performance: Performs accurate multi-system assessments, including; biophysical, psychosocial, environmental, self-care, education needs and discharge planning needs. Identifies deviations from normal on routine lab diagnostic tests. Reassesses patient throughout hospitalization. Performs complete discharge assessment.
Planning: standard of performance: Develops an individual nursing plan of care. Nursing plan of care is sensitive to the developmental needs of the patient. Coordinates patient care activities and discharge planning with other hospital and clinic departments and community agencies. Develops patient teaching plan and documents on care plan or teaching plan. Appropriately delegates patient care activities based on patients condition and scope of practice of other health care providers. Established priories for patient care. Classifies acuity level of assigned patients.
Implementation: standard of performance: Initiates appropriate measures based on protocol for the management of the medical and/or surgical patient. Bases nursing care on protocol in place for management of each patient. Correctly carries out the physicians plan of care. Accurately documents in the EMR. Documents care according to policy and procedure. Utilizes a multidisciplinary approach when appropriate. Performs all necessary procedures correctly. Utilizes available teaching plans in addressing patient teaching needs. Accurately calculates and administers medications. Initiates appropriate measures in emergency situations.
Evaluation: standard of performance: Evaluates assessment methods, plan of care, and patients response to treatment. Participates in unit quality assurance activities.
Basic Qualifications: Experience
Minimum one (1) year of labor and delivery or post partum experience.
Education
Graduate of an accredited nursing program.
License, Certification, Registration
This job requires credentials from multiple states. Credentials from the primary work state are required at hire. Additional Credentials from the secondary work state(s) are required post hire.
Registered Nurse License (Washington) within 3 months of hire OR Compact License: Registered Nurse within 3 months of hire
Registered Nurse License (Oregon) within 3 months of hire
Neonatal Resuscitation Program required at hire
Basic Life Support required at hire
Additional Requirements:
Computer keyboarding skills.
Demonstrated knowledge of high risk medication administration (Magnesium/Oxytocin/Insulin).
Demonstrated knowledge to support maternal-newborn teaching.
Preferred Qualifications:
Previous health care experience.
Bachelors degree in nursing or related field.
Minimum three (3) years of labor and delivery experience.
Minimum three (3) years of postpartum experience.
Minimum one (1) year of medical/surgical nursing experience.

Position Overview:
We are seeking a compassionate and reliable Licensed Practical Nurse (LPN) to provide part-time support for an individual with intellectual and developmental disability. This role includes assistance with daily activities, light physical therapy, etc.
Responsibilities:
Support with meal preparation
Assist with basic physical therapy exercises
Personal care assistance
Support with daily living activities
Foster a positive, respectful, and supportive environment
Maintain punctuality and consistent attendance
Qualifications:
Active LPN license in New Mexico
Prior experience caring for an individual with disability preferred
Must be able to pass a background check and maintain a valid form of identification
Complete initial and ongoing training requirement within established time frame
Schedule:
Monday - Thursday, 9:00 AM-4:30 PM (Approximately 30 hours/week)
Pay:
$25.74/hour
