Job Summary:
Northwestern University Mesulam Center for Cognitive Neurology and Alzheimer's Disease (Mesulam Center) is seeking an exceptional socialworker who demonstrates a pronounced interest in working with persons with neurocognitive disorders and their families in a diversity of roles. The SocialWorker will work as a team member to provide clinical care for persons living with dementia and their families and achieve the goals and objectives of the Outreach, Recruitment and Community Engagement (ORE) Core, the Miller Family Buddy Program, in addition to other psychosocial initiatives.
Specifically, the SocialWorker will provide clinical social work services for persons living with dementia and their caregiving families in the Neurobehavior and Memory Clinic of Northwestern Medical Group; contribute to the recruitment and retention of research participants that meet Center research priorities; manage and facilitate monthly care partner support groups; and participate in the coordination and planning of an annual scientific conference. In addition, they will participate in the preparation and review of annual progress reports and competitive renewal for the NIA and other funding agencies.
The Mesulam Center is a multi-departmental component of the Feinberg School of Medicine. We specialize in the clinical care and scientific study of neurological diseases that interfere with cognition and behavior. Areas of emphasis include age-related memory, language-based and behavioral dementias. We are focused on Alzheimer's disease, Primary Progressive Aphasia and Frontotemporal Dementia. Our three interacting missions are to investigate the causes and clinical characteristics of neurodegenerative diseases, to ensure that our patients and families are the beneficiaries of the latest research advances, and to prepare future scientists and clinicians in our field.
Specific Responsibilities:
* Provide clinical social work services to patients and families of the Northwestern Medicine Neurobehavior and Memory Clinic and center research participants. Complete psychosocial assessments and provide education, counseling and linkage to appropriate community services. Collaborate with multi-disciplinary team of behavioral neurologists, neuropsychologists, psychiatrists, and other clinicians through weekly clinical rounds and patient care discussions.

Handle and record cashier functions accurately following policies for cash handling procedures.

Provides individual and group counseling to students and their families.
Provides parent/guardian training on such issues as child development and teacher/parent/student communication.
Collaborates with school staff in implementing strategies to promote student learning.
Participates as a member of the IEP,MTSS, and other school based teams to develop interventions for promoting students' academic success.
Provides crisis intervention services.
Provides social work case management for students and families.
Provides staff consultation on behavioral-emotional-environmental issues affecting student articipation in the learning process.
Conducts staff development on issues related to social-emotional-environmental factors that impact learning.
Conducts social skills groups and classroom presentations on identified areas of concern for the students.
Maintains required clinical records and submit appropriate documents for statistical reports in adherence with program standards in school social work.
Completes risk assessments on referred students.
Completes functional behavior assessments on identified students.
Completes classroom observations as necessary.
Serves as liaison with community agencies and assist in fostering communication between schools, parents/guardians, and community leaders.
Locates and mobilizes community resources to support the educational program.
Participates in activities that promote professional development.
Supports and implements District policies and procedures, regulations, and administrative directives.
Adheres to the ethical standards and codes of the profession and to the established rules, regulations, and laws governing special education.
