    Under the supervision of the Program/Sports Director, the Sports Program Specialist is responsible for direct care and supervision for all participants in the sports programs. The Sports Program Specialist may assist in officiating youth sport games for enrolled participants. ESSENTIAL FUNCTIONS: General Responsibilities: * Organizes and executes all activities for enrolled participants. * Monitors the area at all times to ensure safety, and immediately makes changes as necessary. * Provides a positive environment for all participants, and actively encourages participation to ensure growth and development. * Exemplifies appropriate behavior and is a role model for participants; actively listening, shows patience, understanding, and respect for others. * Instructs and guides participants through the rules of the program and ensures adherence * Immediately reports all unusual incidents or accidents to the Program Director, and fills out all applicable and necessary reports. * Ensures the area is neat, orderly and free of all safety impediments. * Develops and fosters interpersonal relationships between participants, and is pro-active creating an environment of cooperation, friendliness, and helpfulness towards staff and customers. * Ensures all necessary supplies are on hand and well maintained at all times. * Set up for sports programs as directed by supervisor * On-site attendance is required of this position * Responsible for upholding all staff safety requirements including cleaning protocols throughout the day, as required by my department. * All other duties as assigned Additional Refereeing Responsibilities: * Responsible for being prompt and professional during all games and program activities * Interact with coaches, other sports officials / umpires, youth, parents and YMCA staff * Enforce program rules and policies by maintaining constant surveillance of program participants * Set up facility or equipment, store equipment, and secure facility upon completion of games/program (on an as needed basis) * Report equipment problems * Ensure safety of members/participants * All other duties as assigned Requirements QUALIFICATIONS: * Prior experience working with children and adults * This person should be familiar with the game rules and possess the knowledge to teach the basic skills, as well as supervise the children * Must have previous playing or officiating experience or have completed referee training clinic * Successfully complete State of Florida Criminal History Background Check requirements as well as meet the Association's policies on background screening * Adhere to drug/alcohol/smoke free workplace policy * Able to maintain confidentiality * Employment is contingent upon completing required Level 2 background screening through the Florida Care Provider Background Screening Clearinghouse. Primary Responsibilities: Advocates for the mission of the organization and promotes the vision and direction of the agency's values Delivers services with safety, respect, and effectiveness Follows all policies & procedures of the organization Works with referral sources to ensure continuity of care and compliance to funding source requirements Upon admission, gathers baseline data concerning client's use of medical services and housing prioritization needs Participates in bimonthly case conferencing and rounds on-site with all involved team members Provides and arranges referrals to appropriate service providers within the agency and externally Balances and maintain a caseload of six (6) or more clients based on acuity, length of time in program, status of housing stability, etc. Advocates for clients as needed Provides hands-on modeling and teaching to assist clients with developing daily living skills (cleaning, cooking, meal planning, shopping, budgeting, etc.). Reviews and evaluates client progress on an ongoing basis and adjusts care plans as appropriate Organizes, documents, and maintains client case files/documentation in a timely fashion in accordance with policies & procedures and professional practice Employees will set up, maintain and use daily the following communication tools to keep abreast of agency updates: agency voicemail and agency email. In the provision of above services, the employee will utilize HCCH HOPE Team Outreach support and approved HCCH/OBFH rideshare resources to coordinate transportation of clients based on priority service needs. Only agency approved vehicles will be used by authorized employees to transport clients in accordance with agency transportation policy. Performs other duties as assigned Requirements: High School Diploma, GED or equivalent experience (Bachelor's degree in human services and/or certification in Case Management with commensurate experience preferred) At least two years' experience in Human Services field providing supportive case management services to at-risk, indigent, and/or homeless population (advocacy, housing placement, etc.) Knowledge, Skills and Abilities: Must be able to work effectively with populations that have experienced vulnerability, trauma, economic poverty, incarceration, substance use, development delays, serious mental illness, traumatic brain injuries, literacy issues, and/or other conditions or situations that have impacted housing stability Ability to work in a fast-paced environment Ability to be flexible and able to multi-task Must be solution-focused and organized in achieving objectives with all service users Must be able to motivate and organize self to complete tasks with service users, often in non-therapeutic environments Ability to maintain confidentiality and follow all federal and state HIPAA laws and regulations Ability to handle stress and high pace of crisis intervention and follow protocols for safety and security Excellent oral and written communication skills, including use of phones, in-person assistance, computers, fax, and other office machinery Computer proficient and experience with EHR (Electronic Health Record) documentation preferred Experience with HMIS (Homeless Management Information System) preferred Additional Requirements: Sitting, walking, lifting to 20 pounds, reaching, carrying, listening, speaking (Physical demands are representative of those that must be met by an employee to successfully perform the essential functions of this position.
