RN Case Manager (Senior) The Center for Development and Disability (CDD), established in 1990, is New Mexico's University Center for Excellence in Developmental Disabilities Education, Research and Service. These centers, authorized by the Developmental Disabilities Assistance and Bill of Rights Act, build the capacities of states and communities to respond to the needs of individuals with developmental disabilities and their families.
The Nurse-Family Partnership (NFP) seeks a compassionate and dedicated Bachelor's prepared RegisteredNurse (RN) to join our team as a Nurse Home Visitor. This pivotal role offers a unique opportunity to deliver comprehensive, evidence-based preventive services directly to women and families eligible for the NFP program. The ideal candidate will have experience serving high-risk populations, including those impacted by poverty, mental and behavioral health challenges, substance use disorders, and social determinants of health, and will be deeply committed to enhancing family well-being through the NFP model.
Key Responsibilities:
* Provide in-home visitation services to low-income mothers during pregnancy through the child's second year, fostering strong, positive, and therapeutic relationships.
* Implement the evidence-based NFP model with fidelity, ensuring the highest standards of clinical nursing practice and adherence to program protocols.
* Support clients in achieving NFP program goals, including improved pregnancy outcomes, child health and development, and family economic self-sufficiency.
* Assess and respond to the complex needs of families experiencing mental health challenges, substance use, domestic violence, housing instability, or other high-risk circumstances, providing trauma-informed and culturally responsive care.
* Engage in outreach efforts at hospitals, clinics, schools, community events, and other venues where pregnant individuals may be present to promote and recruit for the NFP program.
* Engage respectfully with diverse cultures and communities, demonstrating cultural humility, sensitivity, and an understanding of each family's unique strengths and needs.
* Participate in comprehensive training and certification in the NFP model and commit to ongoing professional development, reflective practice, and supervision.
* Maintain accurate and timely documentation, data entry, and adherence to funder and programmatic requirements, including the periodicity schedule for required screenings.
* Travel is required in the Albuquerque Metro area for home visits using a personal vehicle, mileage is reimbursed.

Oncology Nurse (ICU) The UNMCC is currently recruiting for an Oncology RN in our Infusion Suite to provide professional nursing care and coordinate the delivery of oncology treatments (includes chemotherapy administration and blood products), often in intensive care conditions, to assigned patients in a medical oncology facility, ensuring adherence to hospital or department and University policies, and nursing procedures, standards, and practices. Must obtain a chemotherapy/biotherapy certification from ONS within 6 months of hire and maintain certification during employment.
The University of New Mexico Comprehensive Cancer Center is the Official Cancer Center of New Mexico and the only National Cancer Institute-designated Cancer Center in a 500-mile radius. Its 136 board-certified oncology specialty physicians include cancer surgeons in every specialty (abdominal, thoracic, bone and soft tissue, neurosurgery, genitourinary, gynecology, and head and neck cancers), adult and pediatric hematologists/medical oncologists, gynecologic oncologists, and radiation oncologists. They, along with more than 600 other cancer healthcare professionals (nurses, pharmacists, nutritionists, navigators, psychologists and social workers), provide treatment to 65% of New Mexico's cancer patients from all across the state. And they partner with community health systems statewide to provide cancer care closer to home. In 2024 they treated more than 15,000 patients in almost 105,000 ambulatory clinic visits in addition to in-patient hospitalizations at UNM Hospital. A total of 2,075 patients participated in cancer clinical trials to study new cancer treatments that include tests of novel cancer prevention strategies and cancer genome sequencing. The more than 123 cancer research scientists affiliated with the UNM Cancer Center were awarded $38.3 million in federal and private grants and contracts for cancer research projects. Since 2015, the physicians, scientists and staff have provided education and training experiences to more than 750 high school, undergraduate, graduate, and postdoctoral fellowship students in cancer research and cancer health care delivery.

General Summary: The Clinical Manager maintains the quality of client care in accordance with the Agency's objectives and policies, through planning, coordination, and implementation and evaluation of the home health services provided. Regularly reevaluate the client's nursing needs and evaluate the outcomes of care. Assist clients in the recovery or maintenance of life functions; prevent illness and relapse.
Develop, initiate, and revise the Plan of Care (POC) as necessary to ensure quality and continuity of care. Initiates appropriate preventative and rehabilitative nursing procedures. Plans for discharge of the client from services.
Provide effective communication to clients, their families, team members, and other health care professionals. Regular participation in Case Conferences.
Apply the concepts of infection control and universal precaution in coordinating and performing client care activities to protect both clients and staff (OSHA).
Coordinate services and inform the physician and other personnel of changes in the client's condition and needs.
Monitor assigned cases to ensure compliance with the requirements of third-party payers.
Complete appropriate documentation in a timely manner to assure compliance with company policy.
Supervise, teach, and provide clinical direction to other nursing personnel (ex. LVN/LPN, HHA, CNA, etc).
Perform on-call responsibilities and provide on-call services to clients and their families as assigned.
Demonstrate commitment and professional growth by participating in in-service programs and maintaining/improving competency.
Meet mandatory continuing education requirements of the agency/licensing board.
Supervision:
LVN/LPNs and CNAs.

Requirements
Experience:
One (1) year experience as an RN in a clinical care setting.
Preferred, Home health experience.
Skills:
Nursing skills as defined as generally accepted standards of practice.
Excellent interpersonal skills and ability to communicate effectively.

If required to make visits, Hepatitis profile.
Must read, write and comprehend English.
Education:
Completion of an accredited RegisteredNursing Program.

Licensure/Certification:

Reliable transportation and auto liability insurance.

When assigned by a Special Education Case Manager, design appropriate goals, objectives and timelines for appropriate school related special education health services for eligible students.
Provide quality educational based special education health service based on goals related to deficit areas.
Monitor and document student progress and communicate progress with appropriate students, staff, parents and other members of the school-based team(s).
Develop care plans for and serve as a resource person in the educational planning for medically fragile students, as requested.
Act as a liaison between home, school and community resources and makes referrals to appropriate community agencies.
Provide health counseling to students/staff regarding chronic illness, nutrition programs and positive lifestyles as appropriate.
Perform various types of health screening assessment activities and cooperates with other district screenings, make recommendations, and do follow-up based on the results of these screenings.
Follow time schedules, attends and participates in all assigned meetings, and maintains a positive professional manner with students, parents and staff.
Interact positively and appropriately with students and staff.
Maintain confidentiality standards as defined by local, state, and federal rules and regulations.
Other duties as assigned.

Qualifications: Must have current professional registeredNurse license (RN), be able to acquire NM PED school nurse license, BS in nursing recommended, CPR certification required and 1 or more years in school nursing preferred.
Physical Requirements: Sitting, standing, lifting and carrying (up to 50 pounds), reaching, squatting, kneeling, prolonged typing, physical ability to type on a keyboard terminal, and moving light furniture may be required.
Equipment/Technology Handled: Must know how to properly operate or be willing to learn to operate all multimedia equipment including current technology.
Work Environment: Must be able to work within various degrees of noise, temperature, and air quality. Interruptions of work are routine. Flexibility and patience are required. Must be self-motivated and able to complete job assignments without direct supervision.

Perform responsible and complex duties as it relates to school health policies and procedures
Essential Duties and Responsibilities:
Demonstrate tact and discretion in preparing, disclosing and handling information of a confidential, controversial and sensitive nature.
Participate in planning, implementation and evaluation of the school health program in accordance with regulations of the State of New Mexico.
Advocate for the health needs and rights of students.
Administer health services to students using the nursing process to assess needs, plan interventions and evaluate outcomes.
Identify health problems including social, physical, and psychological factors that interfere with learning, through screening procedures, assessment and evaluation of the student.
Facilitate communication between student, family, staff, medical provider and the community
.
Participate in health education program activities for students, staff and the community.
Maintain current health and immunization records, health information on student visits as required by the State of New Mexico.
Supervise health assistants and/or school health clerks
Support district and school efforts to keep CCSD safe and secure
Support and comply with grant, program and/or funding source requirements
Complete and comply with all required trainings annually
Adhere to NM Code of Ethical Responsibility of the Education Profession (NMAC 6.60.9.8)
Comply with all Board of Education policies, administrative regulations, Public Education Department guidelines, and local, state, and federal laws.
Perform other job responsibilities as assigned

Minimum Qualifications:
Associate's degree, Bachelor's degree, or higher.
Current license to practice as a RegisteredNurse issued by the New Mexico Board of Nursing.
Current Professional School Nurse License issued by the State of New Mexico.
Knowledge, Skills and Abilities:
Knowledge of Dinè language, culture and history preferred
Attendance at the School Nurse Orientation (one time)
