The assignment is for approximately 8 weeks depending on when you start. No two days and no two patients are alike. You champion multiple patients as they begin their physical rehabilitation journey. Whether it's a neurological issue or a physical trauma, you encourage, comfort, and ready your patients for therapy, interacting with physicians, case managers, nurses, and families as part of a team care approach. Your Everyday
* Take the lead: prioritizing, coordinating, and delegating all physical rehabilitation care touchpoints for your patients.
* Interact with other members of the rehabilitation team to coordinate goals and improve quality of care.
* Ensure patient physical therapy care and activities consistent with the medical plan of care.
* Maintain and advocate a safe environment for everyone.
* Monitor and engage with your patients, families, and appropriate care team members for teaching and education opportunities, patient condition changes, or patient questions and concerns.

* Provide guidance to new and experienced personnel and oversight for ancillary personnel
* Reassess and revise goals and treatment plan as indicated by patient's status, program requirements, and CMS rules and regulations.
* Evaluate patient progress toward expected outcomes and document as indicated regarding subsequent assessments, evaluation of treatment and interventions including responses.
* Maintain appropriate data and contribute to department Quality Management activities providing recommendations for systematic and programmatic improvements based upon the results of the data.
* Demonstrate skill and accuracy in performing age-appropriate assessments of patients and document results of standardized assessments and act as a resource to Program Managers and Directors for achievement of standards for accreditation by CARF and/or other accrediting organizations.

* B.S., Masters or Doctorate in Physical Therapy
* Current license to practice Physical Therapy in the state of Louisiana
* American Heart Association Basic Life Support/CPR certification (can be certified once onboarded)
* Non-Violent Crises Intervention certification (can be certified once onboarded)
Preferred:
* 1-year experience in setting

Seeking PRN weekend Coverage for Mid-City Location- Inpatient SNF/REHAB and Acute as needed.
JOB PURPOSE OR MISSION: Provide Physical Therapy services to all patients at a level consistent with Baton Rouge Rehabilitation Hospital (BRRH) mission, vision, and values. JOB FUNCTIONS ESSENTIAL JOB FUNCTIONS include, but are not limited to: 1. Completes patient assessments and treatments.
PERFORMANCE STANDARDS:
• Assesses patient, plans, and implements treatment, periodically re-evaluates patient's progress toward goals adjusting treatment plan appropriately.
• Utilizes Standardized testing to document a patient's current levels and progress.
• Formulates plan based on evaluation results, patient input, discharge goals, interdisciplinary team goals, all available technology and resources, age related norms and evidence based best practices.
• Provides patient and family with hands-on training including all transfers, mobility, functional independence needs and exercises specific to that patient's needs. A hard copy of the home
exercise program is provided to patient/family.
• Provides safe, effective individualized treatment focused on functional tasks to improve endurance, strength and range using therapeutic exercise as an adjunct only.
• Adapts patient's environment in and outside of therapy to facilitate patient's progress.
• Provide age specific direct and indirect patient care for school age (6-12), Adolescent (12-18) adult (18-65) and elderly ( 65+)
• Utilizes technology effectively to maximize patient gains.
• Assess and recommends equipment in time frame to obtain for training and/or discharge.
• Assess, recommend, plan and provide home evaluations, family training, and community reentry as appropriate to maximize patient outcomes.

5. Maintains compliance.

• Follows the Code of Conduct, Conflict of Interest, Medicare, Joint Commission Guidelines. Physical Therapy Practice Act and policies and procedures.
• HIPAA: Maintains knowledge of and adherence to all applicable HIPAA regulations appropriate to Job Position including but not limited to: Medical records w/o limitation both paper and
electronic, patient demographics, lab and radiology results, patient information related to surgery or appointment schedules, information related to patient location, religious beliefs and/or public health records, medical records related to quality/data, patient financial information and/or 3rd party billing, patient-related complaints, and research information.

GENERAL DUTIES
Patient evaluation and interpretation of findings: Prior to assessment, gathers necessary information from appropriate people and records, and demonstrates effective utilization of staff resources. Accurately documents charges according to services provided. Consistently reports evaluation results in a manager that reflects interpretation of the patient's overall functional abilities. Documents treatment plans including measurable short-term goals for patient progress.
Education and training: Provide education to patients and family members. Gives patients and family a resource for any questions that may arise. Assist with student supervision, teaching, community education and staff education. Participates in assigned PI committee activities and shares information with OT staff.
Communication and coordination of goals and treatment plan: Participates in team conferences and other meetings as required or advised by leadership. Communicates with the patient and family members of the patient. Collaborates with team members on various Occupational Therapy initiatives.
