POSITION OVERVIEW
The MDS Coordinator (LPN, RN) is responsible for overseeing the resident assessment and care planning process and ensuring compliance with federal and state regulations related to resident assessments, quality of care and Medicare/Medicaid reimbursement.
Key Responsibilities
* Conduct and complete the Minimum Data Set (MDS) assessment to evaluate residents' physical, psychological and functional status, including the implementation of Care Area Assessments (CAA)s and triggers.
* Evaluate each resident's condition and pertinent medical data to determine any need for special assessment activities or a need to amend the admission assessment.
* Prepare and electronically transmit timely reports to the national Medicare and Medicaid databases.
* Develop a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident and the goals to be accomplished for each problem/need identified.
* Provide information to residents/families on Medicare/Medicaid and other financial assistance programs available to the residents.
* Ensure that MDS notes are informative and descriptive of the services provided and of the residents' response to the service.
* Assist with completing the care plan portion of the residents' discharge plan. Evaluate and implement recommendations from established committees as they pertain to the assessment and/or care plan functions of the health campus.
Qualifications
* Must have and maintain a current, valid state LPN or RN license
* Three (3) to five (5) years' experience working in the MDS or assessment role in a senior residential care, healthcare, senior living industry or long-term care environment, preferred

KEY RESPONSIBILITIES:
1. Oversee Residency Program
a. Assists with activities related to echocardiography laboratory accreditation for Piedmont Healthcare hospitals to ensure compliance to IAC standards.
b. Assists with monitoring and reporting on quality metrics identified by Piedmont Healthcare and associated with IAC standards.
c. Compile physician and sonographer competency / registry and CME attendance records.
d. Routinely review cardiac sonographer exams as part of a formal quality assurance program.
2. Training and Education
a. Assess the educational needs and competencies of cardiac sonographer staff and interns and develop, schedule, coordinate, and conduct educational programming for new and existing staff to improve quality of care and reduce variation in workflow.
b. Maintain an up to date knowledge of echocardiography trends and best practice and conduct, participate in, facilitate, and utilize research to foster evidence-based practice.
c. Provide clinical supervision and evaluation of patient care related to echocardiography.
d. Attend and participate in echocardiography leadership activities.
e. Quality/Innovation on echo protocols, policies and procedures f. Maintain relationships with technical colleges and oversee student interns.
3. Clinical Support
a. Perform patient evaluations including verifying patient identity, reviewing the medical record for orders and prior studies, and addressing patient questions and concerns.
b. Perform echocardiograms including 2D / Doppler with and without contrast, dobutamine stress and exercise stress echo exams, and transesophogeal echocardiograms according to protocol.
c. Complete appropriate documentation, post processing, and preliminary findings in the medical record and cardiovascular PACS system.
d. Acquire and maintain training and competency in electronic health record system as required to perform duties.
e. Maintain personal competency file if applicable.
KNOWLEDGE, SKILLS, ABILITIES
Skill and competency in performing detailed cardiac ultrasound studies. Skill and ability to communicate effectively both verbally and in writing. Proficient in Microsoft windows-based computer software. Experience with electronic health records and cardiovascular PACS systems. Ability to work as a member of a team. Demonstrated clinical expertise and interest and ability in providing education. Self-starter with outstanding organizational, analytical, and project management skills.
Qualifications:
MINIMUM EDUCATION REQUIRED: Graduate of registry approved ultrasound college according to CAAHEP.
MINIMUM EXPERIENCE REQUIRED: Seven (7) years clinical experience as a registered cardiac sonographer.

POSITION SUMMARY:
We are growing with a new program - OhioRise! We need Moderate and Intensive CareCoordinators to work in Lorain County. We are looking for professionals that understand High-Fidelity Wraparound practice while providing carecoordination services to identified youth that will provide specific, measurable, and individualized services to each person served.
RESPONSIBILITIES INCLUDE:
Provide Wraparound CareCoordination services as part of the CME Project, using the High Fidelity Wraparound model to clients and families identified for the projects. Deliver service in a variety of settings in the home and community. Service plan should include a comprehensive 24 hour Crisis Plan.
Maintain required caseload of 1:20 at any given time. Initial Plan is required within 30 days, and subsequent plans submitted every 30 days.

Work collaboratively with identified partners on behalf of the Child and Family team to include both formal and informal supports.
