**SocialWorker for Mount Carmel East, Night Shift** *This position will have a rotating schedule of Sunday - Tuesday and Monday - Wednesday; 7:00 pm - 7:30 am. There is a weekend rotation of 1 weekend day every two weeks. The SocialWorker intervenes with patients who have complex psychosocial needs, require assistance with eligibility determination for social programs and funding sources and qualify for community assistance from a variety of special funds and agencies. In addition, offer crisis intervention to patients and families with psychosocial needs and coordinates and facilitates the development of a discharge plan of care for high-risk patient populations. This role will receive referrals for individuals from at-risk populations from Multi-disciplinary team members (including physicians, Case Managers, staff nurses and other members of the care team).
**Minimum Requirements:**
· Education: Masters degree in Social Work
· Licensure / Certification: Current LSW license to practice as a socialworker in the State of Ohio
· Experience: Minimum 1 year experience in discharge planning in an acute care setting or an ambulatory setting.
· Effective Communication Skills
· Strong organization skills, self-directed, flexible, able to adapt to rapidly changing regulatory requirements.

Participates on the IDT's initial assessments, care planning and on-going re-assessments of participant care.

Reviews plan of care with participants, guardian, and/or activated health care proxy as assigned.
Complete authorizations for home care and other approved services timely and accurately.
Completes home and/or skilled nursing facility visits to assess participant as indicated.

Provides timely communication to appropriate staff regarding the following: (disenrollment, conversion to long term care, transfer of sites, participant and/or caregiver demographic changes).
Reports allegations of abuse to appropriate state agency; provides support and resources to participant as he/she will accept; completes required documentation.
Ability to pass a fit test. Position requires mask where seal is critical. Incumbent is required to not have facial hair that interferes with a tight seal of the respirator.

Works collaboratively with Director of Social Work and Behavioral Health provider to ensure guardianship is up to date. Educates participant regarding health care proxy (HCP). Assists participant in completion of HCP form.
Works collaboratively with fiscal department to maintain participant insurance benefits and completes required documentation of fiscal information in the medical record.
Reviews plan of care with participants, guardian, and/or activated health care proxy as assigned.
Complete authorizations for home care and other approved services timely and accurately.
Completes home and/or skilled nursing facility visits to assess participant as indicated.
Works collaboratively with Palliative care team; Assists with end of life planning as indicated.
Provides timely communication to appropriate staff regarding the following: (disenrollment, conversion to long term care, transfer of sites, participant and/or caregiver demographic changes).
Reports allegations of abuse to appropriate state agency; provides support and resources to participant as he/she will accept; completes required documentation.
Ability to pass a fit test. Position requires mask where seal is critical. Incumbent is required to not have facial hair that interferes with a tight seal of the respirator.

The SocialWorker is responsible for providing complete psychosocial assessments, crisis intervention, individual, family, group, bereavement and end of life counseling, and timely and appropriate discharge planning based on known or anticipated needs.
Responsibilities:
+ Demonstrates expertise in accessing social systems by providing resource-specific information to patients and families, initiating contact with appropriate resources and facilitating the patient / family's ability to accept referrals.
+ Maintain extensive knowledge of federal, state and local assistance programs and community resources that affect patient needs.
+ Acts as a mentor to social work students and/or serves as a field instructor to MSW students.

WHAT YOU'LL DO
As a hospice socialworker, you will provide emotional and logistical support to patients and their loved ones during an important and vulnerable time. You will ensure VITAS patients reach the end of life with their affairs in order, honoring their wishes for finances, funeral arrangements, and more. Their loved ones will trust you to help them navigate the grieving process.
Whether you're granting an end-of-life wish or simply listening to a patient's stories at their bedside, your actions-large and small-will make a meaningful difference in the lives of patients and their families.
WHERE YOU'LL WORK
Each day is different for our socialworkers. You will spend most of your time caring for hospice patients wherever they call home, which may be a private residence or an assisted living/skilled nursing facility.
About once a week, you'll come to the office to meet with the rest of your interdisciplinary team: the RNs, physicians, chaplains, and fellow nurses who work together to provide 360-degree support to the patients and families we serve.

JOB REQUIREMENTS
Ability to travel per job requirements.
Thorough knowledge of community resources or the desire/ability to obtain this knowledge.
Knowledge of key psychosocial issues prevalent with terminally ill patients and their family/caregivers, including the knowledge of effective psychosocial interventions to address those issues.
Capacity to relate and work well with others as a member of an interdisciplinary team.
Excellent listening and communication skills.
Ability to be an effective patient and family advocate- both within the patient/family unit, within the core IDG, and within community agencies and the like.
Provide emotional and psychological support to patients and families.
Offer counseling and crisis intervention services.
Provide bereavement support and grief counseling.
Educate patients and families on the emotional and psychological response to the illness progression
QUALIFICATIONS
Masters of Social Work required from a school accredited by the Council on Social Work Education, unless grandfathered in per the 2008 Medicare Hospice Condition of Participation
At least one year social work experience in a health care setting.
Experience as a socialworker with the terminally ill and their families, preferred
Experience in providing counseling and support to patients and families.
Empathy and compassion in dealing with seriously ill patients and their families.
Equivalent experience of licensure may be considered where allowable by State Regulations. (i.e. a BSW in FL or WI) License and/or Certification if required by State

Key Responsibilities:
Conduct individual and group psychotherapy sessions for patients in a subacute or skilled nursing facility.
Assess, diagnose, and treat a wide range of emotional and behavioral health conditions, including depression, anxiety, adjustment disorders, and cognitive decline.

Requirements:

Basic proficiency with technology, including electronic health records (EHR).
Strong organizational and documentation skills, with attention to regulatory compliance.
Ability to provide in-person services at assigned facilities, up to 45 minutes.
Ability to walk, stand, and move between patient rooms and offices within the facility throughout the day.
Full Vaccination and Booster Status may be required in some facilities.
Preferred Qualifications:
