POSITION OVERVIEW
We are seeking compassionate and organized Care Coordinators to join our team in providing comprehensive case management and support services to individuals experiencing or at risk of homelessness. The Care Coordinator plays a key role in helping clients access and navigate resources related to housing, physical and mental health, employment, and other social services. This position supports various interim and permanent supportive housing programs across the agency.
KEY RESPONSIBILITIES
Essential duties include, but are not limited to:
Conduct timely assessments for new clients to identify housing, financial, medical, and psychosocial needs.
Collaborate with clients and partner agencies to create and implement individualized service plans.
Provide referrals and support to connect clients with appropriate internal and external services.
Maintain clear, accurate, and timely case notes and client records in accordance with agency standards.
Support site operations by coordinating intake flow, resident needs, and on-site services and events.
Utilize motivational interviewing and strength-based approaches to build trust and encourage client progress.
Administer and track standardized assessment tools such as the VISPDAT, coordinating appropriate referrals.
Support a safe and welcoming environment for all residents, guests, and staff.
Identify and address mental health and/or substance use concerns by facilitating referrals to appropriate services.
Collaborate with program partners and participate in case conferencing and coordinated entry system (CES) meetings.
Assess client readiness for employment and provide referrals to workforce development programs as appropriate.
Assist with arranging transportation and other logistics to support client access to community resources.
Complete required documentation and reporting in a timely and professional manner.
Perform other duties as assigned to support program goals and client needs.
QUALIFICATIONS & REQUIREMENTS
Familiarity with housing resources, public benefits, and service systems within Los Angeles County.
Strong knowledge of case management practices, including trauma-informed care, harm reduction, and housing-first principles.
Experience working with individuals experiencing homelessness and/or co-occurring mental health and substance use challenges.
Proficiency in Microsoft Office and client data systems (e.g., HMIS).
Excellent verbal and written communication skills.
Detail-oriented, organized, and able to manage multiple priorities.
Effective problem-solving and crisis intervention skills.
Ability to work independently as well as collaboratively with a multidisciplinary team.
Bilingual (English/Spanish) preferred.
Must possess a valid California driver's license and have reliable access to a registered vehicle.

Extend health promotion and self-management training.
Arrange transportation.

This program provides a collaborative and highly individualized process for developing specific resources and services for the referred child and their families. The goal of the program is to utilize a strength-based approach to maximize the capacity of each family to meet the child's needs, maintain the child in a family based-setting, and prevent or reduce the need for Short-Term Residential Therapeutic Program (STRTP) placement.
JOB DUTIES & RESPONSIBILITIES:
Effectively manage and coordinate all aspects of the Child and Family Team (CFT).
Perform initial and ongoing strengths-based assessments of children and families. Identify and prioritize family's needs across all domains.
Schedule and facilitate all Child and Family Team (CFT) meetings; involve the family in the planning and decision-making process.
Role model positive communication and conflict resolution skills.
Identify opportunities and resources to promote family strengths, self-sufficiency, and independence.
Coordinate the development of both Plan of Care and Safety Plan and ensure services are relevant to established goals and needs as expressed by the Child and Family Team (CFT).

Engage and conduct in-person outreach with eligible MCP Members
Accompany MCP Member to office visits, as needed and according to MCP guidelines
Extend health promotion and self-management training
Arrange transportation
Connect MCP Member to other social services and supports needed
Educate MCP Members about MCP Member benefits, including crisis services, transportation services, etc.
Distribute health promotion materials
Offer services where the MCP Member lives, seeks care, or finds most easily accessible and within MCP guidelines
Advocate on behalf of MCP Members with health care professionals
Use motivational interviewing, trauma-informed care, and harm-reduction practices
Work with hospital staff on discharge plan
Monitor treatment adherence (including medication)
Contact MCP Member to schedule in-person visit with the contract provider
Schedule: Monday - Friday 8:00 am - 4:30 pm

Job Summary: The Enhanced Care Management (ECM) Lead Care Manager is responsible for coordinating medical and non-medical supportive services to meet the needs of each member. These services include physical, behavioral, dental, developmental, oral health, long-term services and supports (LTSS), Specialty Mental Health Services, Drug Medi-Cal/Drug Medi-Cal Organized Delivery System programs, Community Supports, and other resources addressing social determinants of health (SDOH)-regardless of the care setting. The ECM Lead Care Manager collaborates closely with the CalAIM Community Supports Program to deliver team-based, patient-centered care for clients experiencing or at risk of homelessness.
Essential Duties and Responsibilities:
Client/Service Delivery
Manage a caseload of approximately 17-25 members, meeting regularly to complete Health Needs Assessments and develop individualized Care Plans with measurable short- and long-term goals
Conduct outreach, engage, and enroll eligible members in ECM services, addressing barriers to access and care
Provide services at locations that are accessible and convenient for the member, including their residence or places where they seek care, in alignment with CalOptima Health guidelines
Assist members with care engagement, including medication reconciliation, scheduling and reminding about appointments, coordinating transportation, and attending critical medical visits when needed
Advocate on behalf of members with healthcare providers and assist with hospital discharge planning
Coordinate care with hospitals, behavioral health providers, specialists, dental providers, LTSS entities, and Community Supports partners
Utilize trauma-informed care and motivational interviewing techniques to engage members
Monitor treatment adherence, including medication compliance
Provide health education, self-management support, and care navigation to empower members

Education and Experience
Bachelor's Degree in Social Work (or related field) with experience working in recovery, mental health, and co-occurring disorders are preferred
Candidates without a BA/BS degree but with at least 2 years of relevant experience working with individuals experiencing long-term homelessness, low-income backgrounds, and diverse populations, and who possess a working knowledge of mental health and substance use issues, are encouraged to apply
Preferred skills include outreach and engagement, care planning, care coordination, health promotion, transitional care support, member and family education, and linkage to social services
A minimum of 1 year of experience in care coordination or case management is strongly preferred, particularly with the following populations:
Individuals or families experiencing homelessness
High utilizers of hospital or emergency services
Individuals with serious mental illness (SMI) and/or substance use disorders (SUD)
Adults transitioning from nursing facilities to community-based settings
Adults with intellectual or developmental disabilities

Program Summary:
The Young Adult Court Program (YAC) works in collaboration with University of California-Irvine, Orangewood Foundation, Probation, the District Attorney's Office, Defense Attorney's Office, and The Orange County Superior Court. The YAC program provides engagement, assessment, clinical case management and resource coordination support to Transitional Age Young Men (ages 18-25) as part of a collaborative research study to reduce recidivism.
Position Summary:
The Care Coordinator III (CADC I) plays a vital role in supporting participants through a trauma-informed, person-centered approach. As part of a multidisciplinary team, the Care Coordinator III provides intensive case management services to young men with a history of nonviolent felonies and history of substance abuse. The primary focus of this role is to support participants in their recovery journey, helping them navigate legal, social, and substance-related challenges to achieve long-term stability and success. The Care Coordinator III delivers direct services, including assessments, case planning, workshops and referrals, while fostering a safe and supportive environment that promotes personal growth and accountability. The program offers a structured support system through four key stages: Engagement, Accountability, Stability & Sustainability, and Attainment.

Major Areas of Responsibility:
Conduct comprehensive assessments of participant's substance use history, legal background, and psychosocial needs to develop individualized treatment and case management plans.

Collaborate with probation officers, legal representatives, and community partners to ensure compliance with legal obligations and treatment plans.
Coordinate services, such as mental health care, housing assistance, employment programs, educational opportunities, and other resources, to support participants' holistic recovery and reintegration into society.
Facilitate group workshops on substance use education, life skills, and coping strategies.
Maintain accurate and timely documentation of all participant interactions, case plans, progress reports, and treatment outcomes in compliance with organizational and regulatory standards.
Conduct regular case reviews and adjust care plans based on participants' progress, emerging needs, or setbacks.
Engage family members, mentors, and community supports to promote positive participant outcomes.
Participate in team meetings and case conferences to discuss strategies, challenges, and successes in supporting program participants.
Uphold the highest ethical standards in all interactions, ensuring participant confidentiality and respectful treatment.
Identify opportunities and challenges, providing coaching and mentoring to help participants overcome barriers and succeed in the program.

Experience Requirements:
Bachelor's degree in Criminal Justice, Psychology, Social Work, Counseling, or a related field, OR Certified Alcohol and Drug Counselor I (CADAC I) is required.
At least two (2) years of experience in the substance use disorder treatment and recovery field is required.
A minimum of one (1) year of experience working with the Transitional Age Youth (TAY) population is preferred
Certification Requirements:
CADAC I certification is preferred.

TEMPORARY EMPLOYEES Temporary employees are those employed to work seasonally, on special projects for short periods of time, or on a “fill-in” basis. These positions are not intended to be a part of continuing operations. A temporary employee will not change from temporary status to regular full-time or part-time status because the employee's assignment is extended. An employee's status only will change if the employee is advised of such a change, in writing, by USHS's HRD. Unless otherwise required by applicable law, temporary employees are not eligible for Organization benefits, and temporary employees remain employed at will at all times.
POSITION OVERVIEW
We are seeking compassionate and organized Care Coordinators to join our team in providing comprehensive case management and support services to individuals experiencing or at risk of homelessness. The Care Coordinator plays a key role in helping clients access and navigate resources related to housing, physical and mental health, employment, and other social services. This position supports various interim and permanent supportive housing programs across the agency.
KEY RESPONSIBILITIES
Essential duties include, but are not limited to:
Conduct timely assessments for new clients to identify housing, financial, medical, and psychosocial needs.
Collaborate with clients and partner agencies to create and implement individualized service plans.
Provide referrals and support to connect clients with appropriate internal and external services.
Maintain clear, accurate, and timely case notes and client records in accordance with agency standards.
Support site operations by coordinating intake flow, resident needs, and on-site services and events.
Utilize motivational interviewing and strength-based approaches to build trust and encourage client progress.
Administer and track standardized assessment tools such as the VISPDAT, coordinating appropriate referrals.
Support a safe and welcoming environment for all residents, guests, and staff.
Identify and address mental health and/or substance use concerns by facilitating referrals to appropriate services.
Collaborate with program partners and participate in case conferencing and coordinated entry system (CES) meetings.
Assess client readiness for employment and provide referrals to workforce development programs as appropriate.
Assist with arranging transportation and other logistics to support client access to community resources.
Complete required documentation and reporting in a timely and professional manner.
Perform other duties as assigned to support program goals and client needs.
QUALIFICATIONS & REQUIREMENTS
Familiarity with housing resources, public benefits, and service systems within Los Angeles County.
Strong knowledge of case management practices, including trauma-informed care, harm reduction, and housing-first principles.
Experience working with individuals experiencing homelessness and/or co-occurring mental health and substance use challenges.
Proficiency in Microsoft Office and client data systems (e.g., HMIS).
Excellent verbal and written communication skills.
Detail-oriented, organized, and able to manage multiple priorities.
Effective problem-solving and crisis intervention skills.
Ability to work independently as well as collaboratively with a multidisciplinary team.
Bilingual (English/Spanish) preferred.
Must possess a valid California driver's license and have reliable access to a registered vehicle.
WORK ENVIRONMENT & PHYSICAL REQUIREMENTS
Work may take place indoors and outdoors in varied conditions.
May require standing, walking, bending, lifting up to 25 lbs., and extended computer use.
Must be able to travel to off-site locations and community meetings.
Potential exposure to individuals in crisis and to sensitive or confidential situations.
