: Acts as an extension of the psychiatric providers and team. Works in collaboration with the behavioral health team, medical team, and community stakeholders, is responsible for assisting with tasks that are program specific but center around initiating/coordinating placement in appropriate level of care and/or aftercare planning for patients in order to ensure a smooth transition back into the community once symptoms are stabilized. Familiar with community resources and able to make referrals to community organizations/ programs based on identified treatment needs. Maintains ongoing professional relationships with community organizations for ease of referrals, etc. Assists with obtaining insurance authorizations for treatment as needed, obtaining collateral information related to patient care/assessment, and help with completing psychiatric scales/measurement-based tools.

The Klein Family Harford Crisis Center (KFHCC) is a comprehensive, trauma-informed behavioral health hub that uses innovative evidence-based modalities to serve our community. The KFHCC was created with Centers for Disease Control (CDC) recommendations in mind to ameliorate the behavioral health crisis currently being experienced in Harford County, Maryland. All aspects of crisis response behavioral healthcare are offered directly by the KFHCC. Specifically, an Urgent Care Walk-In Clinic, an Outpatient Therapy/Psychiatry Clinic, and short-term Residential Crisis Beds are offered to those in need of behavioral health/substance use disorder care. In addition, targeted education is provided in the community with the goal of eliminating stigma associated with mental health and substance use disorders.
Qualifications
Education & Training: High School Diploma is required. Associates or Bachelors Degree in Human Services related field preferred.
Work Orientation & Experience: Requirements contingent on specific program/ acute care treatment setting. Training to gain specific capabilities to perform essential job functions may be necessary.
Lived experience (mental health and/or substance use treatment) required. Previous experience working with acute mental illness and those impacted by trauma is required. Minimum of two (2) years of experience serving persons with chronic mental illness is preferred. Must be familiar with community resources and referral processes.
Skills & Abilities: 1) Ability to effectively communicate with patients, family members, medical team members and community stakeholders in a fast-paced setting; 2) Engage with patients in a patient-centered, trauma-informed manner; and 3) Thorough understanding of HIPAA requirements and confidentiality as it relates to care provided.

The department of Health Behavior & Society is seeking an **_Outreach Worker_** to work as part of a large community-level intervention aimed to reduce HIV risk and increase community cohesion among female sex workers in Baltimore, MD. The intervention is largely centered around providing services on a mobile unit and at a full-service drop-in center. Will be asked to conduct community outreach, in groups of two or more, to provide harm reduction services, as part of the SPARC Center, which has been operating in Pig town since the summer of 2017. Outreach workers will provide safer injection equipment and education, overdose response trainings, as well as conduct HIV/STI testing and harm reduction education. This will include the distribution of drug use equipment, condoms, resource lists, hygiene items, clothes, snacks and seasonal items. Prior to any community engagement, Outreach Workers will be trained to administer and distribute Naloxone and medication that can reverse the effects of an overdose of opioids (i.e., heroin, oxycontin, fentanyl), as well as trained in syringe service provision.)
*Outreach activities take place in Pigtown, Mount Clare, Carrolton Ridge, Curtis Bay and Brooklyn.
**Schedule: 4-18 hours per week. Tuesdays 11-4; Thursdays 11-4 and other outreach shifts as desired/available.
**Specific Duties & Responsibilities**
+ Promote the SPARC Center services and events (clinic hours, classes, support groups etc.).
+ Distribute Naloxone, train community members on its use and benefits.
+ Distribute safer drug use supplies and safer sex supplies, while providing harm reduction education.

Job Description
: Acts as an extension of the psychiatric providers and team.
