Job Description
Work Schedule: M-F 7a-7p with potential for weekend rotation based on department need.
Licensure as a RegisteredNurse is REQUIRED
Under general supervision of the Manager of CaseManagement, the CaseManager provides care coordination and discharge planning for an assigned patient case load. The CaseManager will assess individual patient's needs, coordinate and implement plans of care and monitor and evaluate outcomes across the continuum of care.
II. Principal Responsibilities and Tasks
1. The following statements are intended to describe the general nature and level of work being performed by people assigned to this classification. They are not to be construed as an exhaustive list of all job duties performed by personnel so classified.
• Assesses assigned patients and integrates psycho social assessment within the multidisciplinary team. Works collaboratively with the multi-disciplinary team to determine potential and actual risks to recovery and formulates plan to determine appropriate level of care upon disposition from the acute care setting.
• Accountable for developing and coordinating the implementation of Discharge Plan A and alternative Plan B, including documentation in the medical record
• Multidisciplinary collaboration with physicians, nursing, social work, and multiple disciplines, departments, payers, and agencies to eliminate barriers to efficient delivery of care in the appropriate setting
• Facilitates timely transition plan
• Active leadership role in the ongoing management of inpatient length of stay and the reduction of readmissions across the organization
• Integrates work with ambulatory and clinic transitional casemanagers
• Leadership in care management rounds, interprets, and integrates the patient's story into the overall multidisciplinary plan of care
• Develops and implements corrective action plans for resolution of complex problematic issues and elevates to leads as necessary
• Ensures authorizations have been completed in a timely manner
• Collaborates with Utilization Management and synthesizes information in order to reduce avoidable days
• Collaborates with Physician Advisor for complex issues
• Guides physicians in appropriate clinical documentation
• Actively participates in clinical performance improvement activities as assigned
• Builds a network of positive working relationships that advocate for the patient
• Remains current on clinical practice and protocols impacting clinical reimbursement
• Participates in department improvement initiatives
• Serves to orient and educate CaseManager 1s to departmental practices
• Provides oversight and education to casemanagement interns placed in the practice area
Qualifications
III. Education and Experience
1. Licensure as a RegisteredNurse or other equivalent health care license in the state of Maryland Required.
2. A minimum of 12 months of experience as a casemanager is required.
IV. Knowledge, Skills and Abilities
1. Knowledge of managed care, discharge planning and casemanagement is required.
2. Highly effective verbal and written skills are required.
3. Strong communication skills, self confidence and experience dealing physicians required.
4. Excellent analytical and team building skills, as well as the ability to prioritize and work independently is required.
5. Must possess the ability to work collaboratively with other disciplines.
6. Ability to work with Hospital/CaseManagement related software programs required.
7. Ability to demonstrate knowledge and skills necessary to provide care appropriate to the patient population(s) served. Ability to demonstrate knowledge of the principles of growth and development over the life span and ability to assess data reflective of the patient's requirements relative to his or her population-specific and age specific needs.
V. Patient Safety
Ensures patient safety in the performance of job functions and through participation in hospital, department or unit patient safety initiatives.
1. Takes action to correct observed risks to patient safety.
2. Reports adverse events and near misses to appropriate management authority.
3. Identifies possible risks in processes, procedures, devices and communicates the same to those in charge.

General Summary

Overall, the CaseManager will promote direct communication with the patient, and appropriate service personnel, in order to optimize outcomes.

Demonstrate critical thinking skills when utilizing the nursing process, based on research, evidence-based outcomes and Standards of Practice to meet patient's health care needs.
Gathers and analyzes specific criteria and guidelines to track inpatient admissions in and out-of-network, ED, readmission and high-cost utilization of members associated with UMQCN/UMMS providers.
Create population-based management strategies and processes (based on a solid understanding of care management, including disease management and preventive care) that help patients manage their healthcare needs and foster care quality, cost-effectiveness, and patient engagement.
Identify patients who may benefit from telephonic outreach or coordination of care; initiate the care-management processes in a quality focused, cost-effective manner across the continuum of care.
Assists the Primary Care Physician to ensure the client's medical needs are met in the most efficient, cost- effective manner.
Reach out to patients assigned by his or her supervisor to assess their most urgent needs, appraise the situation, and listen to the patients' concerns
Establish collaborative partnerships with patients to assist them in examining patterns of health care needs, decisions, lifestyle choices, and utilization of resources that affect their health.
Advocate, educate and coach patients, the family and/or caregiver about treatment options, community resources, and psycho-social concerns in order to set goals and help the client develop self-care skills and independence appropriate to their age and developmental level. Implement CaseManagement interventions with the goal to optimize the patient's health status
Recognizes/understands responsibility of this key role and the responsibility this position demands in direct support of high quality patient care delivery regardless of assignment. Qualifications
Education and Experience
Licensure as a RegisteredNurse in the state of Maryland, or eligible to practice due to Compact state agreements outlined through the MD Board of Nursing, is required; BSN preferred.
3 to 5 years of care coordination experience and/or experience working in an outpatient ambulatory setting
Experience with educating patients and patient goal setting (essential)
CaseManagement Certification (preferred)
Experience in a manage care information environment (preferred)
Preferred experience would include knowledge of quality improvement processes (LEAN or PDSA); practice re-design work such as patient centered medical home and Joint Commission and National Committee for Quality Assurance (NCQA) accreditations.
Knowledge, Skills and Abilities

Education and Experience
Licensure as a RegisteredNurse in the state of Maryland, or eligible to practice due to Compact state agreements outlined through the MD Board of Nursing, is required; BSN preferred.
3 to 5 years of care coordination experience and/or experience working in an outpatient ambulatory setting
Experience with educating patients and patient goal setting (essential)
CaseManagement Certification (preferred)
Experience in a manage care information environment (preferred)
Preferred experience would include knowledge of quality improvement processes (LEAN or PDSA); practice re-design work such as patient centered medical home and Joint Commission and National Committee for Quality Assurance (NCQA) accreditations.
