Responsibilities
MAJOR RESPONSIBILITIES:
Serve as the clinical manager for an assigned caseload of Members.
Contribute as an active member of a multi/inter-disciplinary team to assess, plan, organize, review and evaluate the care needs of Members requiring health care services, to include outpatient and home based therapy services.
Conduct on-site reviews and assessments of Member's community long term care needs to determine medical necessity and clinical eligibility for Community Case Management and Continuous Skilled Nursing Services Determine and authorize services for Members in accordance with program guidelines and regulations, meeting all established deadlines.
Prepare and implement Member service records.
Coordinate, facilitate and monitor the delivery of services to members.
Review and document all relevant information into data system applications in accordance with program guidelines and regulations.
Conduct routine and ad-hoc evaluations and re-evaluations of Members' services.
Contact and engage providers, state agency offices, and Members to obtain information and records needed to conduct a comprehensive clinical review of the case and final determination.
Evaluate and document the appropriateness of Members' services and make modifications, as required.
Maintain individual records documenting all Member encounters and contacts; write clinical summaries.
Foster and promote continuity of care and cooperative partnerships by liaising with health care providers, acute care hospitals and other programs/organizations involved in the provision of services.
Prepare and respond to inquiries related to appeals on clinical determinations.
Maintain positive working relationships with peers, leadership, Members, relevant informal supports, provider organizations, and state agencies.
Maintain the confidentiality of all business documents and correspondence per UMass Chan Medical School/ForHealth Consulting procedures and HIPAA regulations.
Participate in performance improvement initiatives and demonstrates the use of quality improvement in daily operations.
Participates in training and onboarding of new clinical staff
Comply with established departmental policies, procedures and objectives.
Perform other duties as required.
Qualifications
REQUIRED QUALIFICATIONS:
RN licensed to practice in Massachusetts
5-7 years of work experience with at least 3 recent years providing direct service or case management to the pediatric, young adult, or adult medically complex population
Ability to travel statewide to Member homes and service providers places of business
Experience with Office Application and database
Valid Driver's license required
PREFERRED QUALIFICATIONS:
Experience with home care
Knowledge of applicable state regulations

Position Overview:
The Nurse plays a central role in ensuring that our students are healthy and the school is in full compliance with the state. In addition to treating students and maintaining health records, the Nurse functions as the primary contact person for all health-related issues. In this role, the Nurse must be able to communicate effectively with students, teachers, and families.
Responsibilities:
Provide direct and appropriate nursing care to students; assessment, treatment, planning, intervention, evaluation, management, and referral as indicated.
Administer first aid and emergency care to students and staff as needed.
Manage the storage and administration of medications according to district policies.
Conduct mandatory health screenings and track immunization compliance.
Develop care plans for students with chronic health conditions, including asthma, allergies, and diabetes.
Educate students on topics such as hygiene, nutrition, and disease prevention.
Serve as a health resource for parents, providing updates and guidance on their children's health issues.
Coordinate with local health agencies for infectious disease management and health promotion.
Maintain accurate student health records and ensure confidentiality under federal and state law.
Administer daily and PRN (as needed) medications and nursing care procedures prescribed by the student's health care provider and in accordance with the Massachusetts Department of Public Health.
Implement and record required screening programs; notify parents when further medical evaluation is indicated.
Collaboration with the interdisciplinary team in the development of 504 or IEPs as indicated

Qualifications- Required:
Current MA Registered Nurse License.
Minimum of 2 full years of employment as a Registered Nurse, preferably in pediatrics, emergency, community health, public health, or other relevant clinical nursing setting
Current CPR/AED certification from a recognized provider.
Required to be SEI Endorsed within one year of employment

Work Environment & Physical Requirements:

While performing the duties of this job, the teacher is frequently required to kneel, stoop, bend, and crouch. The teacher is periodically required to stand while talking and listening. The teacher is occasionally required to sit, walk, lift, or carry.

RESPONSIBILITIES
Collect, retain and review the health records of campers and staff as required by the MA Department of Health summer camp regulations and American Camp Association (ACA) standards prior to each camp session to ensure physicals are dated within 18 months and all necessary immunizations are recorded;
Train appropriate camp staff in the administration of medication as needed, including, but not limited to, off-site trips;
Train all staff in camp-specific health policies and procedures;
Communicate all health needs to the appropriate staff, including dietary restrictions, medication administration times, and conditions which require careful attention, such as asthma, allergies, and diabetes;
Provide care to campers who go to the health center following an injury or showing signs of illness;
Communicate with families when a child appears ill at camp or sustains an injury that may require additional attention;
Manage documentation for all health care visits, meeting standards set by the MA Department of Public Health and ACA, including all administration of medications and care provided to campers;
Communicate with the Aquatics Director to ensure that all care given at the pool is documented with the health center;
Create first aid kits to be distributed to each program area and all buildings and vehicles, regularly reviewing the contents refilling as necessary;
Arrange for emergency transportation as needed;
Manage the organization and inventory of the health center, creating and using systems that track supply usage, needs, and camper/staff visits;
Collaborate with the Family Services and Clinical teams to refer campers to outside resources as needed;
Communicate with the Health Care Consultant as needed;
Support the development and growth of a positive camp culture by providing additional support to campers and colleagues as needed and at community-wide events, including, but not limited to, performances, lunch, and the ferry; and
Other duties that support the Camp Harbor View community as needed.
QUALIFICATIONS
Current Massachusetts license as a Registered Nurse, Nurse Practitioner, LPN,
or Physician's Assistant;
Current Basic Life Support and Basic First Aid certifications required;
Experience working in a camp or school setting preferred;
Experience working in pediatric medicine preferred;
Strong interpersonal and communication skills required;
Ability to work with youth and families from a variety of backgrounds; and
Ability to work independently and as part of a team.
