SPECIFIC RESPONSIBILITIES: * Under overall direction and within limits prescribed by professional policies, codes and ethics: * Performs pre-anesthesia patient interview and evaluates patient's condition including a review of medical history and physical examinations, laboratory, x-ray, ECG reports, nurses records, previous hospitalization records and anesthetics, including a history of anesthesia problems of family members. * Determines the need for timely administration of indicated medications and supportive therapy such as fluids, blood, etc., pre-operatively in addition to anticipated requirements during surgery and requests additional medical examination or consultation when indicated. * Assembles supplies and equipment for anesthetic cases, checking same for safety and proper function. Discusses the anesthetics, drugs and techniques to be used or any other situation relevant to the anesthesia with the anesthesiologist, surgeon and/or obstetrician when indicated. * Helps position patient properly on operating table; applies, attaches and/or inserts devices for monitoring vital signs and establishes baseline vital signs. * Performs technical procedures: veni-puncture, arterial puncture, laryngoscopy, oral or nasal intubation (before or after induction of anesthesia as indicated to insure airway maintenance. * Induces and maintains anesthesia/analgesia at a required safe level and monitors support life junctions during the administration of anesthesia. Detects and manages untoward reactions to anesthesia agents and other drugs used during surgery. Requests anesthesiology consultant when required. * Observes and informs the surgeon of changes in patient's condition as circumstances warrant and institutes resuscitative measures in support of and restorative to the health of the patient. * Accompanies patient to the recovery room and informs the nurse assuming responsibility for the patient's care during anesthesia emergence and in the immediate post-operative recovery period of all essential data necessary for the patient's optimal recovery. * Visits the patient post-operatively and notes any untoward reactions or complications attributable to the anesthesia. Follows up as required. * Maintains accurate, neat, legible and complete anesthesia record. * Inspects anesthesia and ancillary equipment for health and safety hazards; maintains adequate supplies of equipment and is cognizant of the techniques and quality of cleaning and sterilizing of anesthesia supplies and equipment. * Provides emergency resuscitative care in all areas of the hospital. * Pursues professional growth and increased knowledge through the continuing education opportunities available, e.g., attendance at educational meetings, conferences and work-shops sponsored by the Anesthesia Societies and Associations at the national, state, regional and local levels. Attends departmental in-hospital seminars as often as possible. Keeps abreast of current concepts and techniques through journals. REQUIREMENTS: Graduate of an approved school of nursing with a current license to practice in New York State; graduate of a school of nurse anesthesia, accredited by the American Association of Nurse Anesthetists (Dept. of H.E.W. authorized accrediting agent). Current A.A.N.A. membership, active practicing category. (Possession of above is proof that the applicant has passed the national qualifying examination and received initial certification-C.R.N.A.; and is in compliance with the on-going specific requirements for continuing professional education to maintain certification. JOB SPECIFIC DUTIES * 55% Conducts and maintains the anesthetic as evidenced by: * Selects and implements appropriate anesthetic agent. * Initiates and maintains patient ventilation when appropriate. * Provides for protection of the patient's eyes and pressure points when appropriate. * Observes and records the effects of ambient temperature on the patient when appropriate. * Evaluates the adequacy of anesthetic depth. * Evaluates, records, and treats any cardiovascular dysrhythmia. * Evaluates, manages, and documents fluid and electrolyte status. * Identifies and determines the need for blood component therapy. * Effectively adjusts the anesthetic to meet requirements of patient need. * Evaluates the need for drug antagonism and reversal. * Recognizes and reports any abnormal patient responses. * 20% Administers/Induces Anesthesia as evidenced by: * Provides an appropriate general anesthetic (i.e. rapid sequence) * Establishes and maintains patient airway with OETT, while minimizing laryngoscopy time, establishing tube placement within 30 seconds, observes and affirms tube placement (breath sounds, ETCO2 level, capnograph pattern). * Establishes regional anesthesia in a safe and timely manner. * 5% Completes and documents pre-anesthesia patient assessment according to current anesthesia standards for any person requiring any form of anesthesia or IV sedation as evidenced by: * Collaborates with all members of the health care team to ascertain acuity needs of patient. * Chart review, patient interview, and appropriate documentation of patient history. * Physical examination of patient, including airway examination. * Offers psychological support while explaining the anesthetic risk/benefit and obtaining an informed consent for all planned and potential forms of anesthesia. * 5% Selects appropriate equipment and techniques for use in the accomplishment of goals outlined in the anesthesia care plan/management plan as evidenced by: * Evaluation of the functional status of anesthesia equipment (machine, suction, laryngoscope, monitors, etc.) * Assures presence and functioning of emergency equipment prior to initiation of any anesthetic technique. * Calculation of drug doses; preparation and appropriate labeling of syringes. * Appropriate selection of airway management equipment. * Appropriate application and selection of monitoring modalities. * 5% Supervises Student Registered Nurse Anesthetists (SRNAs) in the delivery of anesthesia care. Participates in education program for SRNAs. * 2% Appropriately and effectively administers pharmacological agents. * Administers drugs based on individual patient need, dosage and interaction. * Follows hospital policy on controlled substances security and sign out. * Evaluates patient response to pharmacological agents. * Effectively treats any adverse drug reaction. * 2% Terminates the anesthetic and emerges the patient from anesthesia. * Prepares the patient for emergence. * Demonstrates knowledge of extubation criteria. * Extubates patient and evaluates ventilatory status. * Determines patient stability prior to transport to PACU. * Leaves the OR in a state of readiness prior to leaving for PACU. * Transports patient safely to PACU. * Gives a thorough report to the PACU nurse. * 2% Evaluates the attainment of anesthesia care goals. * Evaluates response to anesthesia care (immediate or delayed). * Evaluates and documents patient status on admission to PACU. * Recognizes current or possible problems/complications/side effects. * Communicates evaluation findings with the pertinent members of the health care team. * 2% Completes departmental documentation and post anesthesia rounds. * Completes patient care log appropriately. * Completes narcotic record appropriately. * Completes and documents post-anesthesia rounds. * 2% Assists with starting difficult intravenous lines, inserting difficult arterial lines, obtaining difficult arterial blood gases, performing emergency endotracheal intubation and cardiopulmonary resuscitation throughout the hospital. Acts as a resource for critical care management in the intensive care unit as needed. Part II. This nursing role independently provides nursing care; identifies and communicates the impact of the research process on patient care; adjusts interventions based on findings; and reports issues/variances promptly to the research team. The CRN administers research interventions; collects patient data according to protocol specifications; evaluates the patient response to therapy; and integrates evidence-based practice into nursing practice to best protect the health and safety of research participants. Our CRN's are developing and changing the way we manage care for clinical trial oncology patients. The CRN role allows you to apply your nursing and oncology expertise without leaving the field of nursing. Excellent communication skills, detail oriented and the ability to work autonomously with multidisciplinary teams are required to be successful in this role. BSN with 1-2 years oncology experience and/or clinical research experience preferred. Oncology nursing certification preferred. Knowledge of computer databases, word processing and spreadsheets required. Qualifications Requires a Diploma, Associate or Bachelor's degree from an accredited Nursing Program and an active New York State RN license in good standing. Applicants will have a minimum of 6-12 months of RN experience. This position assumes the charge nurse role, actively participates in unit/service activities, supports co-workers, and assesses unit needs.
