Job Summary:
Responsible for patient care under the direction and supervision of a licensed physicaltherapist as mandated by State Practice Act. Possesses training and education which allows him/her to assist the PhysicalTherapist in carrying out prescribed treatment program.
Essential Functions:
* Understand applicable state Practice Act and adhere to the laws which govern how PhysicalTherapistAssistants may practice under the direction and supervision of a licensed PhysicalTherapist.
* Review the VibrantCare Clinical Operations Policies & Procedures Manual on a yearly basis and adhere to the policies and procedures set forth.
* Complete all Medicare compliance trainings as required by regulations and/or VibrantCare policies.
* Complete Occupational Safety and Health Administration (OSHA) training on a yearly basis and adhere to standards set forth in OSHA standards and other governmental regulations.
* Clean and prepare treatment areas and equipment following OSHA guidelines.
* Carry out skilled treatment programs as directed by a licensed PhysicalTherapist.
* Communicate patient comments and activities to therapists clearly via verbal or written word.
* Document in charts using accepted medical terminology and format.
* Complete written documentation (daily notes, charge tickets, etc.) legibly, appropriately and in a timely fashion in order to meet facility, physician and payor deadlines.
* Maintain open and respectful communication with co-workers, physicians, patients, family members, third party payors and others at all times.
* Attend and/or participate in facility meetings as directed by Center Manager (may include utilization review, infection control, disaster preparedness, etc.)
* Completes sufficient ongoing continuing education coursework to meet state licensure requirements for the defined reporting period
* Maintains a current CPR certification that meets state licensure requirements
Additional Responsibilities:
* Utilize time efficiently to meet productivity requirements of the facility.
* Participate in and present clinical in-services, as directed
* Maintain current awareness of general changes in health care field and specific issues impacting the practice of physical therapy.
* Play a significant role in specific area of center operations as agreed upon between employee and Center Manager.
* Performs other duties or special projects as assigned.
Required Skills & Abilities:
* Ability to work effectively as a team member.
* Effective organizational, time management, and planning skills.
* Active listening, reading and comprehension skills for analyzing reports, charts and correspondence
Physical Requirements:
* Ability to lift patients using biomedical techniques frequently throughout the course of a work day/shift; must be capable of easily lifting fifty (50) pounds of weight alone.
* Ability to walk, bend, stand, and reach constantly during a work day/shift.
* Visual acuity (near and distant) sufficient to maintain accurate records, recognize people, and understand written direction.
* Ability to speak and hear sufficiently to understand and give directions.
* Ability to push wheeled equipment throughout the facility and in the community.
* Fine motor skills adequate for scheduling and preparing patients, equipment, and supplies for treatment.
* Ability to participate in sustained activities for many hours in duration in accordance with state labor laws.
Required Credentials:
* Graduation from a physicaltherapistassistant or physicaltherapist education program accredited by CAPTE or in the case of a graduate of an international physicaltherapistassistant program or international physicaltherapist program from a non-CAPTE accredited program seeking licensure, certification, or registration as a physicaltherapistassistant, a substantially equivalent education
* Current valid State license/registration/certification

Conduct a psychosocial assessment and develop an initial discharge plan.
Provide initial treatment planning for patient based on assessment and coordinate any additional services needed and revise as necessary.
Maintain documentation of therapy services provided to each patient according to facility and professional standards, revising as necessary.
Maintain a caseload as assigned.
Participate in treatment team meetings and collaborate with other staff to facilitate services for the patient as appropriate to their treatment plan.
May provide these services in an inpatient or outpatient setting.
Provide case management, discharge planning or after-care planning according to treatment plan.
May assist in educating and training staff on behavior management or therapeutic crisis intervention.
May coordinate various clinical, psychosocial or educational activities with patient and their families.
Evaluate patient's need and determine if referrals to other programs or facilities are needed.
May assist in the training of new clinical staff and interns.
Demonstrate a positive, empathetic and professional attitude towards customers always. When patient needs are not met, acknowledge and work to resolve complaints. Recognize that patient safety is a top priority.

EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:
* Master's degree in Psychology, Social Work, Counseling or other therapeutic field is required.

The Home Health PhysicalTherapistAssistant (PTA, LPTA) performs treatment and provides exercise instruction and patient education aimed at improving/enhancing the patient's well-being while following the established plan of care.
• Responsible for following all state specific laws governing the provision of physical therapy in home care, to follow the treatment set only as defined by the supervising PT; to perform only those procedures that he/she is qualified and trained to perform and to make all recommendations for changes in the plan of care to the supervising PhysicalTherapist.
• Instructs and aids patients in active and passive exercise, muscle re-education, gait, functional, ADL, transfer, safety, and prosthetic training.
• Observes, records, and reports to the supervising PT, the nurse supervisor, and/or the physician the patient's response to treatment and changes in the patient's condition. Coordinates care with the other members of the healthcare team as appropriate.
• Instructs patient, family, caregiver, and other members of the health care team in the areas of therapy within the scope of the Physical Therapy Assistant.
License Requirements
• Current licensed, registered, or certified to assist in the practice of physical therapy in state of practice.
• Current CPR certification is required.
• Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
Additional State Requirements
• TX: Must have one (1) year of experience as a licensed PTA.

* Clean and prepare treatment areas and equipment following OSHA guidelines.
* Carry out skilled treatment programs as directed by a licensed PhysicalTherapist.
* Communicate patient comments and activities to therapists clearly via verbal or written word.
* Document in charts using accepted medical terminology and format.
* Complete written documentation (daily notes, charge tickets, etc.) legibly, appropriately and in a timely fashion in order to meet facility, physician and payor deadlines.
* Maintain open and respectful communication with co-workers, physicians, patients, family members, third party payors and others at all times.
* Attend and/or participate in facility meetings as directed by Center Manager (may include utilization review, infection control, disaster preparedness, etc.)
* Completes sufficient ongoing continuing education coursework to meet state licensure requirements for the defined reporting period

* Graduation from a physicaltherapistassistant or physicaltherapist education program accredited by CAPTE or in the case of a graduate of an international physicaltherapistassistant program or international physicaltherapist program from a non-CAPTE accredited program seeking licensure, certification, or registration as a physicaltherapistassistant, a substantially equivalent education
* Current valid State license/registration/certification
* Current CPR certification required; employee is responsible for maintaining active certification for the duration of employment.
* Current tuberculosis (TB) test required; employee is responsible for maintaining in accordance with state guidelines for the duration of employment.
