Position Qualifications Required / Experience Required: Must be a graduate of an accredited school of nursing and have valid NJ RN License. Refer to the Educational/Regulatory Requirement (Department Specific) policy in the Nursing Administrative Manual. Demonstrates knowledge of nursing skills, hospital practices, procedures and standards. Perfects skills and stays abreast of current nursing practice through on-going education, journals, etc. Must have strong communication skills. Must be able to coordinate efforts of a team of care providers. Short Description The Clinical Coordinator will be responsible for patient enrollment services associated with the digital remote monitoring programs (COVID-19, Heart Failure, COPD, Diabetes). Enrolls patient in platform partners electronic health record and updates as needed Monitors digitally transmitted vital sign results daily (M-F, excludes holidays) 10. Supports patient with activation of patient's MyChart account if not active 11. Assists patient with remote patient monitoring adherence and program participation 12. Outreaches to patients when vital signs are out of range or not transmitted telephonically or via video conference and follows escalation pathway 15. Documents all events and communications in patients CUHC electronic health record 16. Collaborates with RPM Population Care Coordinator and clinical care team as needed 17. Reinforces provider plan of care and patient centered care plan 18. Promotes chronic disease self-management 19. Coordinates with RPM program manager and digital platform support services as needed for technical support 20.
