Job Description Summary:
The RegisteredNurse (RN) provides hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. Job Duties:
Patient Care
Responsible for providing comprehensive care to hospice patients.
Completes an initial assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). Admits to appropriate level of service/ care.
Provides professional nursing care by utilizing all elements of nursing process.
Assesses and evaluates patient's status by:
Writing and initiating plan of care
Regularly re-evaluating patient and family/caregiver needs
Revising the plan of care as necessary
Uses health assessment data to determine nursing diagnosis.
Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician's plan of care.
Counsels the patient and family in meeting nursing and related needs.
Provides health care instructions to the patient as appropriate per assessment and plan.
Assists the patient with the activities of daily living and facilitates the patient's efforts toward self-sufficiency and optional comfort care.
Has working knowledge of state, federal, local and accreditation regulations for the delivery of hospice services.
Communication
Completes, maintains and submits accurate and relevant clinical notes regarding patient's condition and care given. Records pain/symptom management changes/outcomes as appropriate.
Communicates with the physician regarding the patient's needs and reports changes in the patient's condition; obtains/receives physicians' orders as required.
Communicates with community health related persons to coordinate the care plan.
Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the other hospice Interdisciplinary Team Members.
Provides and maintains a safe environment for the patient.
Assists the patient and family/caregiver and other team members in providing continuity of care.
Works in cooperation with the family/caregiver and hospice Interdisciplinary Team Members to meet the emotional needs of the patient and family/caregiver.
Additional Duties
Participates in on-call duties as defined by the on-call policy.
Ensures that arrangements for equipment and other necessary items and services
are available.
Supervises ancillary personnel and delegates responsibilities when required.
Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and inservice classes.
Fulfills the obligation of requested and/or accepted case assignments.
Actively participates in quality management program.
Meets daily average productivity standard set by the organization.

Qualifications:
Current and active California RN license.
Bachelor's degree, preferred.
Minimum one year acute care experience required.
One year recent hospice experience preferred.
Current CPR certification.
Licensed driver with insured automobile - in accordance with state and/or organization requirements.
Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
Ability to respond independently to life threatening situations.
Ability to reason, deduce and carry out complex multiple step processes.
Bilingual - Spanish preferred, but not necessary.

Job Description:
The Inpatient RegisteredNurse provides hospice care and is skilled at assessing pain and other complex symptoms at the end of life. The professional nurse works with the interdisciplinary hospice team to plan and direct services to meet the needs of individuals and families in the General Inpatient setting. Provide nursing services, treatments, diagnostic and preventive procedures requiring the skills of a registerednurse according to in-patient facility protocols.
Attend and participate in interdisciplinary group meetings.
Assists in development, implementation and revision of the interdisciplinary team care plan for each patient and family. Establishes goals, interventions including the patient and family in the process.
Assess patient and report to the attending physician and appropriate interdisciplinary team members, response to treatments and medications as well as changes in the patient's physical or emotional condition.
Coordinate care with other interdisciplinary team members and with community resources as needed.
Teach, supervise and counsel the patient/family regarding pain and symptom management and psycho-social aspects of care for the terminally ill.

Patient Care
1. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of the nursing process and as defined in the state Nurse Practice Act. 3. Assesses and evaluates patient's status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. Participating in revising the plan of care as necessary 4. Initiates the plan of care and makes necessary revisions as patient status and needs change. 5. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 7. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician's plan of care. 8. Counsels the patient and family in meeting nursing and related needs. 9. Provides health care instructions to the patient as appropriate per assessment and plan. 10. Assists the patient with the activities of daily living and facilitates the patient's efforts toward self-sufficiency and optional comfort care. 11. Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.
Communication
1. Completes, maintains and submits accurate and relevant clinical notes regarding patient's condition and care given. Records pain/symptom management changes/outcomes as appropriate. 2. Communicates with the physician regarding the patient's needs and reports changes in the patient's condition; obtains/receives physicians' orders as required. 3. Communicates with community health related persons to coordinate the care plan. 4. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. 5. Provides and maintains a safe environment for the patient. 6. Assists the patient and family/caregiver and other team members in providing continuity of care. 7. Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver. 8. Attends interdisciplinary group meetings.
Additional Duties
1. Participates in on-call duties as defined by the on-call policy. 2. Ensures that arrangements for equipment and other necessary items and services are available. 3. Supervises ancillary personnel and delegates responsibilities when required. 4. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes. 5. Fulfills the obligation of requested and/or accepted case assignments. 6. Actively participates in quality assessment performance improvement teams and activities. Qualifications
1. Graduate of a National League of Nursing accredited school of nursing. 2. Current license in California and current CPR certification. 3. Registerednurses shall have a minimum of (a) one (1) year of experience as a professional nurse within the last three (3) years OR (b) have a baccalaureate degree in nursing from a program accredited by the National League for Nursing and a current RN license. Minimum of two years' experience, at least one of which is in the area of public health, home care, or hospicenursing is preferred. 4. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. 5. Management experience not required. Responsible for supervising hospice aides. 6. Self-directed and able to work with minimal supervision. 7. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. 8. Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping bending, kneeling and/or crouching.
