JOB SUMMARY AND PURPOSE
Under the direct supervision of the Program Manager, and working in collaboration of the Medical Director, Clinical Director and MAT AOD Specialist, the Licensed Vocational Nurse (LVN) provides professional nursing services for this residential substance use program. As a patient advocate, the LVN will also provide limited physical examinations, medication support, detox withdrawal management care, and MAT educational services
Clients are assessed using the full American Society of Addiction Medicine (ASAM) assessment and receive the following services: Education and counseling groups, individual counseling sessions with Substance Abuse Specialist and/or a therapist, case management, transportation, and random drug screening.
DUTIES AND RESPONSIBILITIES
Provides nursing services including: monitoring of withdrawal symptoms, monitoring of medications including MAT, monitoring vitals, direct communication with treatment medical providers on or off-site for referrals to other provides and community health resources, health instruction to patients, and instruction and in-servicing to counseling line staff including monitoring of self-administration of prescribed medication.
Designs and provides a comprehensive health services plan for each client that includes a broad range of medical services including assisting clients with physical and emotional needs, and referral to appropriate service providers when needed.
Documents patient's current medication list, including: name of prescriber, names of medication, dosages, frequency, duration and start date
Participates in weekly case review with Program Staff to ensure best quality care for clients, and continuous improvement of services.
Acts as liaison between Program Staff and Medical Providers, in order to coordinate best care of clients
Maintain electronic health records and hard copy documentation of all services provided
Other Tasks that may be required by the Program Manager
ESSENTIAL DUTIES:
Assign case load of LVN Team, mentor, monitor services
Provide on-the-job training, facilitate ongoing professional training of LVN team
Provide nursing care to program clients
Design health services plan for program clients, coordinate care with primary and other healthcare providers
Monitor clients self administration of prescribed medication, maintain documentation of all medications
Weekly review of clients care with program staff, coordinate reviews with primary care providers
Liaison between Program and primary care providers
Maintain accurate health records of clients
Other duties as required and is subject to change at any time.
Qualifications
REQUIREMENTS:
Must be able to pass a fingerprint clearance, background check, including criminal history, personal references, employment and education verifications
EDUCATION:
Current and valid California Licensed Vocational Nurse
Valid Cardio Pulmonary Resuscitation (CPR) certification
Must have completed ASAM I and ASAM II for access to EHR
EXPERIENCE:
One year of work experience as an LVN
Work experience serving substance abuse clients
PREFERRED QUALIFICATIONS:
Culturally sensitive and demonstrated ability and effectiveness working with mental health and substance use disorder

ESSENTIAL DUTIES AND RESPONSIBILITIES
Reports to the Nurse Supervisor, this position provides the appropriate nursing services within the scope of practice by the California Board of Vocational Nursing and Psychiatric Technicians or California Board of Registered Nursing.
Responds to life saving situations based upon nursing standards, policies, procedures, and protocol.
Provides direct and indirect patient care services including, but not limited to the following: distribution of prescribed medications and treatments in accordance with approved nursing techniques. Maintains awareness of comfort and safety needs of clients. Ensure the personal hygiene and the performance of disease prevention and takes restorative measures.
Perform basic health care, testing and prevention procedures, including skin tests and immunization techniques according to facility policy and procedures for both employees and clients.
Ensure SDCC policies and procedures are always utilized in dispensing and monitoring effects of children's medication, including distribution of routine and PRN medication when deemed necessary and clinically appropriate and approved.

Provides direct support and assistance to counselor staff in a crisis situation when not involved in performing required medical functions.
Occasionally may be called to assist NPS when a crisis or injury presents to provide basic first aid.
Engages in professional conduct (role model behavior and appropriate boundaries) and abides by all SDCC policies and procedures and employee handbook guidelines at all times.
Provides direct support and assistance to counselor staff in a crisis situation when not involved in performing required medical functions.
Responsible for placing client on precautions and discontinuing precautions when deemed necessary.
Attends treatment team milieu meetings and treatment reviews as assigned.

JOB SUMMARY AND PURPOSE
Under the direct supervision of the Program Manager, the Registered Nurse (RN) provides professional nursing services for this residential Recuperative program. As a patient advocate, the RN will also provide limited physical examinations, medication support, and work with the primary healthcare providers for the clients. Schedules medical and mental health appointment for clients as appropriate. Conducts group education for clients related to relevant medical conditions, and in-service training for program staff.
DUTIES AND RESPONSIBILITIES
Provides nursing services including: monitoring of medications, monitoring vitals, direct communication with medical providers on-site for referrals to other providers and community health resources, health instruction to patients, and instruction and relevant training for program staff including monitoring of self-administration of prescribed medication.
Designs and provides a comprehensive health services plan for each client that includes a broad range of medical services including assisting clients and disabled veterans with physical and emotional needs, and referral to appropriate service providers; schedules medical and mental health appointments.
Documents patient's current medication list, including: name of prescriber, names of medication, dosages, frequency, duration and start date
Participates in weekly case review with Program Staff to ensure best quality care for clients, and continuous improvement of services.
Acts as liaison between Program Staff and Medical Providers, in order to coordinate best care of clients
Maintain electronic health records and hard copy documentation of all services provided
Provides group education on medical conditions for clients and in-service training for program staff
Other Tasks that may be required by the Program Manager

ESSENTIAL DUTIES:
Consult with Program Staff and leadership regarding medical care to clients; oversee provision of medical services to clients
Design health services plan for program clients, coordinate care with primary and other healthcare providers

Qualifications
REQUIREMENTS:
Must be able to pass a fingerprint clearance, background check, including criminal history, personal references, employment and education verifications
EDUCATION:
Current and valid California license Registered Nurse
Valid Cardio Pulmonary Resuscitation (CPR) certification
EXPERIENCE:
One year of work experience as an RN
PREFERRED QUALIFICATIONS:
Bachelor s degree in Nursing, Nurse Science, or Public Health Administration (or closely related field)
Experience with Veteran population
