Responsibilities
Check each resident routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes
Review patient care plans daily to determine if changes in the resident's daily care routine have been made on the care plan
Inform the nurse supervisor of any changes in the resident's condition so that appropriate information can be entered on the resident's care plan
Assist resident's with daily needs, including preparation for activity and social programs, and transporting residents to/from appointments
Create and uphold an atmosphere of warmth, patience, enthusiasm, and foster a calm and cheerful environment
Ensure residents' rooms are ready for receiving and help residents feel comfortable
Participate in facility surveys by authorized government agencies
Maintain confidentiality of all resident care information in accordance with HIPAA guidelines
Effectively communicate necessary resident information to charge nurses, director of nursing, and/or administrator
Qualifications
Have a 10th grade education or above
Be a licensed certified nursing assistant in this state
Must be able to read, write and speak English fluently.
Must have an active CPR/BLS Certification.

POSITION DESCRIPTION:
The incumbent of the Senior Certified Nursing Assistant (SCNA) / Activities position will be responsible for ensuring that the residents Activities of Daily Living (ADLs), daily assignments and an ongoing program of activities in accordance with the comprehensive assessment are completed during the shift while identifying and resolving needs. Reports directly to the Activities Director (Therapy Aide Supervisor).
* Assist with developing and implementing an activities program, which improves or maintains physical, mental, and social well-being with a varied work schedule that may include weekends, evenings, and holidays.
* Assist with developing activities plan for each resident in accordance with the inter-disciplinary team process.
* Responsible for the documentation of that plan in the resident's medical record.
* Responsible for initial interview with new residents and input of resident data into the medical record.
* Performing duties of the Activities Director when the Activities Director is unavailable.
* Responsible for maintaining the activity space and requesting equipment and supplies for the program and ensuring resident safety for individual and/or group activities.
* Provide the assistance, when necessary, by giving residents the following Activities of Daily Living nursing care: oral hygiene; bathing; dressing and grooming; hair and nail care; shaving; toileting; assistance with meals, nourishment and transportation to meal service areas, and mobility.
* Routinely observe, identify, and report resident concerns/needs to supervisor.
* Provide escort to resident appointments when required.
* Complete assignments timely, completely, and accurately.
* Accept assigned duties in a cooperative manner.
* Perform restorative nursing measures.
* Document in the nursing assistant notes the care and treatment provided to the resident and the response or lack of response to care provided.
* Perform simple nursing measures such as but not limited to applying splints and positioning devices, making occupied and unoccupied beds, changing linens, taking vital signs, weights, recording intake and output, etc.
* Follow established performance standards and perform duties according to nursing service policies and procedures.
* Review care plans and perform nursing care as outlined.
* Participate in the development of an individualized plan of care of assigned residents.
* Respond to residents' requests/needs and assure residents have call lights at hand and answer call lights promptly.
* Observe all facility safety, infection control policies and procedures.
* Complete required documentation.
* Consistently working cooperatively with residents, resident's representatives, facility staff, physicians, consultants, and ancillary service providers.
* Maintain confidentiality of residents.
* Performs all other related duties in an effective, timely, professional manner as assigned.
* Attend all in-service programs classes as assigned and complete assignment.
* Participates in interdisciplinary team and other facility meetings as assigned.
* Always Follow Residents Rights' policies.
* Assume accountability for data contained in the employees' handbook.

• Record all entries on flow sheets, notes, charts, etc., in an informative, descriptive manner.
• Report all accidents and incidents you observe on the shift that they occur.
• Agree not to disclose resident's protected health information and promptly report suspected or known violations of such disclosure to the Administrator.
• Report any known or suspected unauthorized attempt to access facility's information system.
• Perform only those nursing care procedures that you have been trained to do.
• Ensure that the resident's room is ready for receiving the resident (i.e., bed made, name tags up, admission kit available, etc.).
• Greet residents and escort them to their room.
• Introduce resident to his/her roommate, if any, and other residents and personnel as appropriate.
• Make resident comfortable.
• Inventory and mark the resident's personal possessions as instructed.
• Store resident's clothing.
• Assist residents with packing their personal possessions when they are being transferred to a new room, or when being discharged.
• Transport residents to new rooms or to the receiving area.
• Assist with loading/unloading residents' to/from vehicles as necessary.
• Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift.
• Meet with your shift's nursing personnel, on a regularly scheduled basis, to assist in identifying and correcting problem areas, and/or the improvement of services.

• Keep residents' water pitchers clean and filled with fresh water (on each shift), and within easy reach of the resident.
• Perform after meal care (i.e., remove trays, clean resident's hands, face, clothing, etc.).
• Participate in and receive the nursing report upon reporting for duty.
• Perform all assigned tasks in accordance with our established policies and procedures, and as instructed by your supervisors.
• Follow work assignments, and/or work schedules in completing and performing your assigned tasks.
• Cooperate with inter-departmental personnel, as well as other facility personnel to ensure that nursing services can be adequately maintained to meet the needs of the residents.
• Notify the facility when you will be late or absent from work.
• Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility's policies and procedures governing accidents and incidents.
• Assist residents with daily functions (dental and mouth care, bath functions, combing of hair, dressing and undressing as necessary).
• Keep residents dry (change gown, clothing and linens, when it becomes wet or soiled).
• Assist residents in preparing for medical tests (i.e., lab work, x-ray, therapy, dental, etc.).
• Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
• Perform restorative and rehabilitative procedures as instructed.

• Observe and report the presence of pressure areas and skin breakdowns to prevent decubitus ulcers (bedsores).
• Report injuries of an unknown source, including skin tears.
• Observe disoriented and comatose residents. Record and report data as instructed
• Perform special treatments as instructed.
• Immediately notify the Nurse Supervisor/Charge Nurse of any resident leaving/missing from the facility.
• Use only the equipment you have been trained to use.
• Report defective equipment to the Nurse Supervisor/Charge Nurse
• Maintain the confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information.
• Report all allegations of resident abuse and/or misappropriation of resident property.
• Honor the resident's refusal of treatment request. Report such requests to your supervisor.

Job Description
**This recruitment is for multiple positions at all DPH facilities, including Laguna Honda Hospital and Rehabilitation Center and Zuckerberg San Francisco General Hospital and Trauma Center.**
Under direct supervision of a licensed nurse, the Certified Nursing Assistant (CNA) performs a variety of routine duties involving care of medically or mentally ill acute or sub-acute patients; performs direct patient care; observes and reports changes in the behavior and physical status of patients; provides a clean and safe environment; and performs related duties.
The essential functions include, but are not limited to:
Under direct supervision, and as part of the treatment team, provides routine patient care in accordance with established policies, procedures and standards.
Establishes and maintains a therapeutic and/or medical relationship with patients.
As appropriate, reviews patient's admission documentation and notes pertinent facts regarding diagnoses and treatment history. Documents care according to department standards.
Maintains constant and continuous communication with medical providers and staff.

Observes, records and reports patient medical, social and psychiatric behavior and physical conditions.
Participates in multidisciplinary team meetings and treatment planning by reporting observations and concerns and by asking questions.
