GENERAL SUMMARY:
The CertifiedNurseAssistant (CNA) assists the licensed nursing staff by performing routine nursing duties and activities of daily living.
PRINCIPLE DUTIES:
* The CNA performs personal care services, within the framework of the individual residents activities of daily living and functional abilities.
* The CNA documents all information pertaining to each assigned resident on each shift accurately and timely. Meal percentages, intake and out put recordings will be documented and reported accurately to the responsible charge nurse every shift. Responsible for reporting any significant changes in resident condition, incident/accidents, injuries and/or skin integrity issues noted, immediately to the charge nurse. Responsible for transportation of their respective residents to and from activities, meals and therapy as indicated.
* The CNA participates in all required/mandatory in-services; as well any additionally offered in-service programs as evidenced by their full knowledge of all required policies/procedures and ability to perform their job duties accurately and timely. Meets all state requirements for recertification, completes and submits all required documentation to the state to ensure timely certification renewal. Works as a team player, as evidenced by consistently assisting other staff members when help is needed.

Barrier Free Living Apartments (BFLA) is a supportive housing program. BFLA's program is physically designed and equipped to serve people with physical disabilities. The facility is located in the South Bronx. Role Summary

Education and Experience Requirements:

Position Summary The Certified Medication Technician (Qualified Medication Administration Personnel/QMAP) delivers quality assisted living services focused on maximizing resident's individual capabilities in accordance with all laws, regulations and Century Park standards. Primary Job Responsibilities
Follows all policies and procedures for medication administration, medication ordering, and delivery
Immediately reports med errors, or missing medications
Provides quality care to residents as indicated on the care plan
Promotes resident independence in accordance with service plan
Compliant with all infection control guidelines
Ensures proper transfer of medication cart between shifts, including narcotic count
Documents medication administration in EHR (electronic health record) per policy
Participates in community Lifestyles activities, events and outings, and encourages residents to attend
Demonstrates patience, compassion and integrity with residents
