Responds to team suggestions and integrates recommendations in treatment plans. Participates in clinical team meetings to discuss individual cases and general client progress. Communicates with all collateral agencies to ensure participants compliance with service plan. Maintains all required documentation including progress notes, treatment plans, discharge summaries, Service Participation, Homeless Management Information Systems and submits all in a timely manner. Identifies program needs and makes recommendations. Provides crisis intervention and stabilization as needed. May require some after hour's follow-up to emergency situations. Performs related tasks as assigned. EXPERRIENCE, EDUCATION AND SPECIAL REQUIREMENT: Current NYS certification as a Credentialed Alcoholism and Substance Abuse Counselor required. Bachelor's degree in Human Services field preferred, with 2 years' experience working in a similar setting and/or working with the substance abuse, mental health and homeless populations. Bi-lingual Spanish a plus. Ability to work some evenings and occasional weekends. Excellent and demonstrated writing ability and strong oral communication skills. Must have knowledge and skill in the use of computer software applications for drafting documents, data management and tracking. Experience in crisis intervention techniques and strength-based interventions. Sixty-seven units are dedicated for families, and 203 units are for single individuals. The program has two components to assist clients succeed in housing, socials services and operations. Case management staff focusses on removing barriers to maintain medical care and achieve viral suppression by ensuring that all clients' basic needs are met and that clients are connected to needed services, such as mental health and substance abuse. The operations component focuses on the housing units meeting habitability standards by working with landlords and management companies to address major repair issues and complete minor repairs, as well as identifying new units, negotiating leases, and making units ready when there is a vacancy. Position: Case Manager - Bilingual Reports To: Assistant Program Manager Location: 19 Winthrop Street, Brooklyn NY 11225 What The Case Manager -Bilingual Does: The Case Manager is responsible for ensuring that clients are connected to needed services to address immediate needs, as well as, ensuring that clients obtain stability to maintain permanent housing. Maintain clear, organized, and audit ready records. Report all services provided to clients into program database monthly. This work is in support of the mission and goals of CAMBA. The Case Manager will be responsible for providing all contract funded activities. The third program provides 7 units of housing to individuals who are SPMI and have been residents of either OMH Psychiatric Centers or OMH operated residential programs for one year or longer. All apartments are located in Brooklyn. The program offers Case management services focusing on removing barriers to maintain housing by ensuring that all clients' basic needs are met and that clients are connected to needed services, such as mental health and substance abuse. The operations component focus on the housing units meeting habitability standards by working with landlords and management companies to address major repair issues and complete minor repairs, as well as identifying new units, negotiating leases, and making units ready when there is a vacancy. Clients contribute 30% of their income towards their rent. Position: Case Manager Reports To: Program Supervisor Location: 19 Winthrop Street, Brooklyn NY 11225 What The Case Manager Does: The Case Manager will be responsible for providing all contract funded activities. These activities include conducting and completing Intakes, Assessments, Service Plans, Reassessments, Service Plan Updates, Accompaniment, Referrals, Advocacy, Housing Inspections, Case Conference, etc. In addition, the Case Manager will be responsible for completing all program documentation and entering all client services into to Client Track. Compensation: $46,350 annually When salary ranges are listed, the range would represent the low and high end for the applicable position & program. The salary offered would be based on various factors unique to each program and candidate. This includes but is not limited to experience, education, budget and/or program size, internal equity, skills and other factors that may be required for the position and organization.
