The PhysicalTherapist is a qualified professional who directs, supervises, evaluates, and provides direct care to patients and provides physical therapy services to patients in the home according to State and Federal regulations under the direction of the attending physician.

Responsibilities
* 1. Comprehensive evaluation of the patient's level of function and development of the therapy plan of care.
* 2. Participation in case management conferences and documentation thereof.
* 3. Instruction of patient, caregivers, Physical Therapy Assistants (PTAs), and other agency clinical staff in the patient's treatment plan, including follow-up supervision.
* 4. Selects and administers diagnostic and therapeutic techniques and materials.
* 5. Supervision of PTAs. Confers with PTAs at least weekly and when there is a change in condition.
* 6. Supervision of Home Health Aides when they are participating in the patient's therapy care plan.
* 7. Documentation is completed in the EMR within 24 hours after visits are made.
* 8. Assists patients and caregivers in obtaining necessary equipment and provides instruction on how to use equipment.
* 9. Responsible for maintaining productivity and appropriately communicates needs for additional visits when necessary.
* 10. Ensures that there is insurance authorization prior to making visits when appropriate.
* 11. Reports incidents that occur to the RN case manager and the Manager, Therapy Services.
* 12. Participates in staff meetings, in-service programs, and other meetings as requested.
* 13. Coordinates and manages the care of patients receiving services in their home.
* 13.1 Participates in case management conferences and documents per guidelines.
* 14. May perform other duties as assigned.
Qualifications
EDUCATION
Bachelor of Science in Physical Therapy required, masters or doctorate desirable.
EXPERIENCE
Minimum of two (2) years of clinical experience in Physical Therapy in an acute or sub-acute care setting. Home care experience is preferred.
LICENSURE
PhysicalTherapist with valid (or eligible) Connecticut license. A valid driver's license and proof of car insurance are required when using a personal vehicle for company business. Access to a dependable vehicle in order to travel to multiple stops a day. CPR certification is required and must be maintained. Candidates who do not have certification are required to obtain it within 3 months (90 days) of hire.
SPECIAL SKILLS
Excellent written and verbal communication. Ability to work collaboratively with multidisciplines. Must be adaptable, flexible, able to learn quickly, use critical thinking skills, and oversee PTAs. Strong computer skills.
PHYSICAL DEMAND
Ability to perform tasks involving physical activity, which includes heavy lifting, extensive bending, administering CPR, patient bracing, standing, and stairs. Requires exposure to blood and body fluids, hazardous materials, and communicable diseases.

Other Information:
Experience delivering comprehensive care in an outpatient setting to patients with physical dysfunction secondary to disability, disease, or injury in accordance with professional standards of practice. Experience with diagnoses including low complex medical patients, orthopedic, oncology, and neurological patients.
Credentials: Current license to practice in Connecticut: Successful completion of minimum educational requirements for licensure including all clinical practice and comprehensive examinations. Temporary licensure may be considered at the discretion of the department manager.

A cardiac rehabilitation Exercise Physiologist utilizes the clinical process to offer a multifaceted and multidisciplinary approach to overall cardiovascular risk reduction, foster healthy behaviors, reduce disability, and promote an active lifestyle for patients with cardiovascular disease. The Exercise Physiologist works with a registered nurse to serve as a liaison between the patient and physician to facilitate expeditious and effective medical care. Participates in running Project Heart Smart when offered and in the Heart Beats Volunteer group as needed. Essential duties and responsibilities include the following. Qualifications
EDUCATION
Bachelors degree in Exercise Science required. Telemetry trained. Exercise training, group dynamic skill, or experience related to primary or secondary disease prevention.
EXPERIENCE
Cardiology, physical rehabilitation, community health, teaching, exercise training or public health training .
LICENSURE
Telemetry trained, Certification by the American College of Sports Medicine preferred.
SPECIAL SKILLS
Advanced knowledge of cardiovascular disease, telemetry trained, exercise physiology, cardiac risk factor modification, counseling techniques, nutrition, group dynamic and cardiovascular health education principles.
PHYSICAL DEMAND
While performing the duties of this job, the employee is regularly required to stand, sit, walk, talk and hear. The employee is also required to demonstrate and model aerobic, strength training and flexibility exercises to classes.
