Develop and prepare weekly group plans and activities for youth in residential services.
Facilitate Aftercare Youth Support Group for youth in order to connect youth with educational, career, job, social, relational, other life skills, and community resources.
Monitor and maintain all requested and required documentation.
Work with residential staff and counselors to ensure communication regarding youth, and actively participate in staffing/meetings regarding youth program goals
Interact with respective school district personnel and community partners to maintain communication regarding the coordination of services for youth in programs.
Participate in discharge and service planning with youth and parents/case managers.
