Licensed Therapists have the primary responsibilities of providing individual therapy, recovery sessions, and facilitating groups. Working in a therapeutic environment, Licensed Therapists lead the culture of recovery by providing strength based, person-centered services to assist people in acquiring and maintaining skills to live successfully in their communities. Each person receiving our services has an Individualized Recovery Plan that guides them through recovery.
ESSENTIAL FUNCTIONS:
Professional and Therapeutic communication with persons served and co-workers.
Ability to submit timely and accurate documentation with use of technology
Provide therapy with persons served in their primary language by self or interpretive services
Physical demands, include but are not limited to long periods of sitting, standing, moving / walking, with occasional squatting, lifting, bending, pushing & pulling with low intensity, driving self or clients
Professional demands include prolonged periods of interacting with the public and making independent decisions
Facilitate Psycho-educational, intensive rehabilitation, counselling, and co-occurring disorder groups; including curriculum development.
Diagnose in collaboration with psychiatrist and clinical team
Collaboration with natural and formal supports.
Monitor treatment outcomes in partnership with consumers.
Provide services on-site or off-site to clients as needed.
Promote health & wellness and recovery-oriented initiatives in response to community needs.
PRIMARY RESPONSIBILITIES AND PERFORMANCE EXPECTATIONS:
Facilitate psycho-educational, rehabilitation, and recovery groups. Facilitate counseling groups and provide individual counseling sessions. Conduct intakes and complete psychosocial assessments. Responsible for caseload and all required documentation including notes, individual recovery/ treatment plans, and required assessments.
PROS is a community rehabilitation and support program to assist individuals in acquiring and maintaining skills and supports to live successfully in their communities. These include wellness self-management, benefits and financial management, leisure planning, and skill development to manage symptoms and practice life skills. This program offers intensive rehabilitation services to assist individuals in obtaining specific life roles, and to reduce the risk of relapse, hospitalization, loss of housing, or involvement with the criminal justice system.
EDUCATION:
Master's Degree in Mental Health Counseling or Social Work
QUALIFICATIONS:
NYS Professional Licensure - Licensed Mental Health Counselor, Licensed Master of Social Work, or Licensed Clinical Social Work
Previous experience working with adults who have Severe and Persistent Mental Health (SPMI) issues and be comfortable in both individual and group settings
Computer skills and experience working with electronic medical records
Perform other duties as assigned
Bilingual, English-Spanish speaking, is a plus

This Opportunity:The primary responsibility of the Behavioral Health Therapist is to provide outpatient behavioral health services to persons of all ages experiencing psychological difficulties in order to help them interact appropriately and develop meaningful relationships within community.What You'll Do:
Perform interviews with individuals and/or families to aid in identifying presenting problems, assessing mental status and dangerousness, gathering relevant facts of psychosocial history, and assigning accurate diagnoses (using all five Axes of the current DSM manual) that will help determine the best intervention to help the individual and his or her family achieve stated goals.
Provide individual, group, and family therapy to individuals of all ages using person-centered philosophies and best practices in all service delivery.
Provide expertise in assessing psychiatric crises and crisis intervention while working closely with psychiatrists.

Provide crisis assessments, stabilization, and service coordination in order to de-escalate crisis and potential crisis situations in an effort to maintain lowest level of care that promotes safety.
Provide coverage or back-up for Emergency Services during clinical hours and on-call coverage after hours and on weekends as needed.
Maintain licensure as required by licensing board and attend educational trainings/seminars to remain up-to-date on current best practices.
Maintain positive working relationships within the communities served, including with individuals, families, staff, monitoring and licensing agencies, organizations, funding resources, and other human service agencies.
Be an active participant in the ongoing continuous quality improvement activities of the agency.
Demonstrate knowledge of and comply with all agency policies and procedures.
Provide clinical consultation as required or requested by other professionals within Monarch to provide ongoing professional development and to process difficult client situations that may arise.

Position Summary
The Advance and Earn Program is a career pathways program for youth and young adults ages 16-24 that provides work readiness support, occupational skills training, and employment services, along with supportive services such as High School Equivalency (HSE) classes, paid internships, comprehensive case management, and access to post-secondary education and sector-based credential training.
As a member of the direct service teams, the Licensed Therapist will complete contractually required enrollment steps, a comprehensive assessment and corresponding service planning. This work will be done in collaboration with each youth participant to elevate their experiences, their strengths, their interests, and their voice. The Therapist will build their caseload with those students presenting the most significant barriers, meeting with their participants regularly to work on barriers to program persistence and success, including the provision and follow through with referrals to other service providers. The Licensed Therapist will also complete all necessary data entry related to this work and support the larger team with other program activities, such as outreach and recruitment, workshops, post-exit placements, etc. The Licensed Therapist will serve as the immediate supervisor to the Case Managers and Retention Specialist, including any that need clinical supervision hours for licensure, and serve in a leadership role within the larger organization's Case Management Team.
The salary range for this position is $80-85,000 annually.
Essential Job Functions and Responsibilities
Assess each applicant for program fit and match with contractual enrollment criteria.
Complete a thorough assessment and design a corresponding service plan in collaboration
with each qualifying applicant. Complete necessary enrollment paperwork.
Meet with program participants regularly to discuss barriers to program persistence and
persistence in a post-exit outcome. Make referrals to other service providers as necessary
and ensure participants follow through with all referrals.
Offer crisis counseling and group work when appropriate.
Assure quality, timely data entry into both funder-mandated databases and the agency's
Salesforce instance.
Complete exit steps at the end of each cohort and assist with the placement into positive
outcomes for all youth.
Support a culture of professionalism designed to teach and cultivate a real-world working
experience for the youth and young adults served.
Assist in identifying and developing services and programs not currently being offered.
Collaborate with balance of program team to fulfill all program functions and services that
may need additional support.
Provide leadership to agency's Case Management Team and provide direct supervision
weekly to Case Managers and Retention Specialist.

Provides individual and group counseling services to clients within residential, offsite programs, and the surrounding community served by the Counseling & Resiliency Center. Education and Experience:

Must be knowledgeable about homeless issues, experience working with individuals with mental health/substance abuse issues and/or be willing to seek continuing education related to the homeless population. Trauma informed professional knowledge preferred. Bilingual is a plus.

Requires strong counseling and teaching skills for individuals and groups, along with maturity, dependability, and sensitivity to cultural and individual differences. Provide social work and therapy to individuals who are served by the Assertive Community Treatment Team.
Facilitate training of independent living skills, establish access to community services and agencies, assist individuals in establishing support networks in the community and assess individual needs on a daily basis.
Coordinate services with the program, staff, inpatient facilities, and detoxification facilities and establish linkage to residential programs, self-help programs and community resources.
Review treatment progress and monitor and provide continuous consultation with other Assertive Community Treatment Team staff and community agencies to ensure the consistent therapeutic management of the individual's needs.
Act as liaison with the program after hours staff on a daily basis through exchange of participant information; briefly detailing the presenting problem and after hours plans for the individuals being served by the Assertive Community Treatment Team.
Meet regularly with the other Assertive Community Treatment Team members to formulate and disseminate information and be available by telephone and/or pager.
Complete an intake assessment including: medical history, general physical condition, alcohol and drug histories and current use, educational issues, family structure and relationships, vocational history and goals, legal history and issues, psychiatric history (including, onset, course, effect of illness, treatment history, medication history risk behaviors).
Ensure immediate and appropriate interventions are provided in response to changes in mental status which may put individual at risk.
Document participant activities consistent with the service records manual. Write an event note that reflects participant activities, goals, interventions, and outcomes.
Develop service plans and/or person-centered plans as appropriate, and screens referrals in coordination with the other Assertive Community Treatment Team members.
Meet regularly to review the total caseload, problems-solve, develop plans, and determine daily duty assignments.
Attend and actively participate in meetings and training as required. Maintain certification in all agency, state and federal training requirements.
Demonstrate knowledge of and comply with all agency policies and procedures, as well as state and federal statutes and regulations related to specific program areas.
Follow service definition guidelines for services being provided.
Complete all other relevant responsibilities as assigned by the supervisor.

Position Details:
A Physical Therapy Assistant (PTA) is a registered professional who provides therapeutic techniques for the rehabilitation of patients' service disorders. Plans coordinated care to patients of all age groups. Plans, implements, and evaluates patient care plans to restore or maintain patient well-being. Collaborates with an interdisciplinary team, including physical therapists and other healthcare professionals.
Essential functions:
Design and administer the therapy treatment plan.
Applies concepts of infection control and universal precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.
Accept clinical assignments that are consistent with education and competence to care for patients.
Uses effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes.
Meets mandatory continuing education requirements of the agency/licensing board.
Demonstrates commitment, professional growth, and competency.
