About the Opportunity
We are seeking a full-time Bilingual Family Nurse Practitioner to support Pair Team's Texas Medicaid Care Program and deliver relationship-centered, comprehensive care to patients with complex medical, social, and behavioral needs.
You will work within a multidisciplinary team of community health workers and nurses to provide continuous preventive, chronic, and acute care support to Medicaid members across Texas. Many of our patients struggle with chronic illness, behavioral health needs, SUD, or housing instability, and benefit greatly from consistent, proactive, tech-enabled medical care.
While this role is primarily remote,
candidates must reside in Travis County (Greater Austin, TX) for occasional patient visits and periodic in-person care team meetings.
Key Responsibilities:
Deliver virtual primary and preventive care to underserved patients across Texas
Manage chronic conditions, acute concerns, and routine follow-ups through telemedicine
Build trusted relationships with patients who have limited access to consistent medical care
Support and refine clinical programs focused on hypertension, diabetes, medication adherence, and reducing preventable hospitalizations
Partner with care coordinators, CHWs, and RNs to close care gaps and ensure seamless follow-up
Provide clinical guidance during case reviews and escalations
Collaborate with product and operations teams to improve care delivery workflows

What You'll Need
2+ years of clinical experience, preferably serving medically or socially complex populations
Active Texas NP license
Bilingual - Spanish and English fluency required

Experience supporting patients with chronic disease, behavioral health needs, SUD, homelessness, or Medicaid coverage
Comfort working within a multidisciplinary care model

What is the Telehealth Nurse Practitioner role?
We are looking for a full-time Nurse Practitioner specializing in cardiology and heart failure with experience overseeing complex chronic care programs. Bilingual candidates (English and Spanish speaking) are highly preferred. You will manage a growing panel of patients and contribute to the growth and development of a telemedicine program that leverages first in class medication management titration algorithms combined with patient management principles.
We are seeking full-time coverage Monday through Friday from 8:00 AM to 5:00 PM PST.
What you'll be doing:
Virtually evaluate patients entering the program and assist in creating a care plan based on patient specific needs and predesignated protocols.
Respond to alerts escalated from the nursing team related to patients' deviations in vitals and physiologic data; symptoms; weight changes.
Independently manage changes in clinical status that are within the APP's scope of practice (e.g. dose change in diuretic after 3 pounds of weight gain over 24 hours).
Escalate out of scope clinical changes to lead MD.
Liaise with the nursing team to ensure proper medications, labs and virtual appointments are ordered and communicated to the patient.
Partner with the nursing team to ensure that medications are reconciled on program entry and exit.
Serve as the point of contact for each patient's primary care teams, relaying information to them and tailoring this communication to individual providers' needs.
Provide education to patients about healthy behaviors and self-management.
What you'll bring:
2+ years as a Board-Certified Nurse Practitioner specializing in heart failure and/or internal medicine in an ambulatory or inpatient setting.
Actively licensed in California, multiple state licenses strongly preferred.
Willingness to obtain additional state licenses as needed.
Fluency in English and Spanish is highly preferred.
Previous experience overseeing complex chronic care programs strongly preferred.
Previous telehealth experience strongly preferred.
Willingness to work in a changing, dynamic environment with a growing company
Ability to work from home in a HIPAA compliant environment is required.

Person-Centered Care Planning: Individualized roadmaps addressing all identified needs
Care Coordination: Interdisciplinary team communication across all providers

24/7 Helpline: Human support (not AI) for patients and caregivers during crises
Caregiver Education & Support: Training programs, support groups, one-on-one guidance

Health-Related Social Needs: Screen and address food insecurity, housing instability, transportation barriers, utility difficulties

We are looking for a highly motivated full-time Bilingual Family Nurse Practitioner who is willing to think creatively and empathically to help our team change the way people access healthcare.
We are excited to partner with Federally Qualified, Non-Profit Health Centers in California to enable their participation in CalAIM's new Enhanced Care Management Medi-Cal benefit program, which provides long-term, whole-person care coordination, inclusive of behavioral health and social needs supports.
We seek a full-time Bilingual Family Nurse Practitioner to play a critical role in our whole-person, interdisciplinary care model. This person would be responsible for directly engaging and caring for individuals living with Serious Mental Illness/ Substance Use Disorder, experiencing homelessness, and/or those who have high medical needs. We believe in the power of trust and relationships to successfully engage those who may have never received the kind of whole-health care that Pair Team can provide. Focused on building relationships with and providing support to individuals whose quality of life can be improved with the Enhanced Care Management benefit, the Lead Care Manager has lived experience working with these populations, is an empathetic problem-solver, and works closely with our partner clinics, community organizations, and Pair Team's Lead Care Managers and Clinical Team.

What You'll Need

Job Duties:
