Purpose:
The purpose of this position is to engage patients coming out of emergency room and inpatient psychiatric unit settings and assist in linking them to services to support their community stabilization. The 590 Peer Engagement Specialist team is an innovative care coordination pilot program to connect individuals and families with mental health needs upon their discharge from hospitals and provide support with therapy and care coordination services until linked to long term community providers or consumer needs are resolved. In addition, the responsibility of the 590 Peer Engagement Specialist team is to triage crisis needs, dispatch to mobile crisis, provide community outreach, promote services, assist with linkage, conduct follow-up, and support addressing Social Determinant of Health needs. The Peer Engagement Specialist has a unique role in the Rapid Response program and is encouraged to share personal experiences with recovery from mental health.
Location and Work Conditions:
Although the Peer Engagement Specialist will be based at one of C4's sites, services are provided telephonically and remote work is permitted. Peer Engagement Specialist will occasionally be required to report on site for the purpose of trainings and for the shifts out in the community with clinicians on the Rapid Response company van. Shifts are in 10-hour increments with two shifts during the day and two shifts overnight, with some availability for overtime. Work may be stressful due to consumer behaviors, periods of high call and case volume, or tight deadlines.
Duties and Responsibilities
Ability to be respectful of the diverse cultures of the people served and to provide culturally appropriate, competent, and individualized treatment according to each client's age, gender, race, ethnicity, and culture
Engage and Educate clients for participation in treatment and other outside needed resources for all Social Determinant of Health needs.
Attend linkage appointments with consumers.
Develop and maintain relationships with community-based programs.
Conduct outreach and follow up with clients, families, and treatment providers.
Maintain all appropriate documentation and reports needed for program.
Provide a Person-Centered approach to ensure client motivation in recovery.
Assist with ensuring hospital discharge recommendations are understand by consumer and followed through with by the patient.
Ensure interventions are recovery oriented, culturally congruent and developmentally appropriate
Engage natural and family support to strengthen the individuals participation and engagement.
Deliver services within hospital, office, and community settings.
Ensure successful linkage to long-term care providers as indicated.
Work collaboratively with interdisciplinary teams and health care team members both internal and external to the organization to improve patient care through effective utilization and monitoring of health care resources.
Maintain effective communication and coordination with paired hospital and participate in hospital staffings
Minimum Qualifications:
High school diploma
CRSS certification (preferred).
High school diploma
Familiarity and comfortability doing community work throughout Chicago
Valid drivers license, insurable driving record and vehicle insurance
Engagement specialists are expected to partner with clinicians in community outreach and crisis response using the agency vehicle on assigned shifts

The Opportunity The Specialist, Health Initiatives role provides critical programmatic, data and operational support to the Health Initiatives team, advancing efforts to strengthen food bank-healthcare partnerships and expand models that increase access to nutritious food and promote better health outcomes. This role engages across the organization, with the national network of food banks, and with external partners to ensure Feeding America's work is informed by current Food and Health evidence and best practices to support the emerging needs from the network. Responsibilities:
Takes direction from Manager/Director of Health Initiatives to implement project tasks, processes and analysis for Food & Health capacity building initiatives (e.g., food as medicine, and healthcare partnerships focused), progressing key project components and coordinating with team members, appropriate personnel, and/or consultants in a timely and clear fashion.
Collaborates with Health Initiatives manager and director to define project goals, program design and deliverables and to draft and manage project budgets. Required Experience and Qualifications:
3 or more years of experience in non-profit, private sector or healthcare roles at increasing levels of responsibility. Specific experience in the Food as Medicine programming, public health, community health or health policy space strongly preferred.
Demonstrated experience in program implementation and service delivery, technical assistance and capacity building, and continuous quality improvement.
Strong project management skills with the ability to balance multiple priorities and deadlines in a fast-paced, matrixed environment.

Program participants will be trained to engage in outreach and enrollment activities for the Supplemental Nutrition Assistance Program (SNAP), Women Infants and Children Program (WIC) and other resources, thereby enhancing the grant's impact and promoting sustainable solutions to hunger within their communities. Position is part-time. Candidates must live in or within commuting distance to Chicago- or be willing to relocate without relocation assistance -and will spend most days working from a project office in or near the city, with the possibility of working from home on a limited basis. This position will report to HFA's Project Manager, Social Services Job Training Program - Chicago, IL region. Duties may include:

RESPONSIBILITIES

Manage the implementation and logistics of adult drunk driving prevention and underage drinking prevention programs through the design and facilitation of training programs at the State and Local level.
Organize and host orientation meetings, evaluation meetings, facilitator training and facilitation of parent workshops and student presentations.
Source and recruit volunteers, as needed, for applicable programs. Provide training, guidance, support and motivation to volunteers and staff by conducting training meetings, telephone coaching and on-site assistance.

The Wounded Warrior Project (WWP) Physical Health & Wellness (PH&W) Screening Specialist recruits and screens warriors for multi-week programs and events that educate, coach, and empower wounded service members in pursuit of achievement of their physical health and wellness and/or adaptive sports goals. The PH&W Screening Specialist provides warriors with comprehensive screening, needs assessment, and coordination of internal and external resources to support warriors and enhance their well-being.
DUTIES & RESPONSIBILITIES
* Identify, screen, and recruit warriors for participation in the virtual and in-person PH&W Coaching Program, Soldier Ride, and Adaptive Sports Program. The PH&W Program provides fitness, nutrition, mobility, sleep, and health-focused education to warriors. The Soldier Ride program provides all-ability events that include single and multi-day rides of varying modalities with adaptive and standard equipment. The Adaptive Sports Program provides opportunities for warriors to participate in modified athletic events designed for their individual abilities.

Due to an internal promotion, we are looking for a Special Events Manager who will meet fundraising goals and objectives and engage our donors across the Illinois region in meaningful events. In this role, you will lead a team of staff and volunteers to plan, fundraise for, and execute four major fundraising events across the region. This position is part of the development team who support the work within the Illinois region and will report to a Director of Development.
This is a hybrid role where you will support events in-person throughout the Illinois region, and work from our Chicago office (2200 W Harrison Street) on a weekly basis. Flexibility to work remotely when possible.

Event Management Responsibilities
* Responsible for the execution of four major fundraising events across the Region raising $2M+ in revenue.
* Collaborate with senior paid and volunteer leadership to secure event sponsorships by supporting the development of prospect lists, proposals, stewardship reports and preparatory meeting and solicitation materials.
* Ensure the distribution of relevant special events information by providing support and counsel to departmental staff and volunteer leadership including serving as staff advisor for all fundraising events related committees.
* Day-to-day supervision of two special events staff who will provide programming, logistical, and administrative support to special events. Additionally, provide project-based management to other departments to ensure effective execution of special events.
* Responsible for the management of all fundraising events from conception to execution, including but not limited to: managing event timelines, developing and maintaining event budgets, managing event committees, ensuring that event fundraising goals are met, developing sponsorship packages, ensuring sponsor benefits are fulfilled, coordinating event collateral, managing event logistics, set-up and day of execution.
Donor Account Management Responsibilities
* Develop and execute ongoing regional strategy for qualifying donors to ensure retention, growth of donor contributions as well as recapture from previous donors.
* Manage assigned portfolio of donors and prospects with intent to form deep relationships. Responsible for minimum annual fundraising goal, including both renewable gifts and new incremental revenue. Develop donor management plans for entire portfolio resulting in strategic, knowledgeable and complex asks.
* Implement a program/activity to identify, cultivate, solicit and steward donors at the $5,000 level or higher, with an emphasis on maximizing revenue for the American Red Cross. Update donor records in Salesforce following donor contacts.
* Carry out any additional assignments required to fulfill the mission of the American Red Cross.
Compensation:
The salary range for this position is: $86,000 - 100,000. You will be eligible to participate in an incentive plan based on annual individual and organization performance.
Note that American Red Cross salaries are aligned to the specific geographic location in which the work is primarily performed. Other factors that may be used to determine your actual salary may include your specific skills, how many years of experience you have, and comparison to other employees already in this role.
This role is not eligible for relocation assistance.
What you need to succeed:
* Education: Bachelor's degree required. Advanced degree highly desirable.
* Experience: Minimum 7 years of sales and/or fundraising experience or equivalent combination of education and related experience required.
* Event planning experience preferred.
* Given the realities of event management, operational flexibility is required to meet sudden and unpredictable needs. Physical conditions may include lifting up to 30 lbs. unassisted, squatting, pushing or pulling heavy weights up and down ramps and stairs. Good manual dexterity, the ability to sit or stand for long periods of time, and ability to adapt to long, irregular hours is required.
* Skills & Abilities: Ability to relate well and work effectively with multiple constituencies and audiences. Excellent interpersonal, verbal, and written communication skills. Knowledge of office systems: MS-Office preferred and fundraising database systems (e.g., Raiser's Edge, Salesforce). Ability to work on a team within a collaborative environment and ensure the highest customer service orientation. A current valid driver's license and good driving record is required.
