Minimum Qualifications

Initiate contact with previous provider, Primary Care Physician, Psychiatric Hospital and/or collateral resources, to obtain assessment information and provide case management during transition as necessary.
Ensure member's discharging from psychiatric hospital are linked to agency Psychiatrist within 30 days. Ensure member's transferring to services with medication already in place are linked to agency psychiatrist.
Develop initial safety plan and monitor safety planning in high risk cases during assessment period.
Complete Service Requests and links members and their families/identified support systems to community resources as appropriate.
Facilitate groups and provide individual and family therapy.
Supports Family Advocate with engagement of new referrals to program.
Schedule: Monday - Friday 8:30 am - 5:00 pm (various shifts available, full-time or part-time)
Compensation: $34.00 - $36.08 per hour
