Department: School Health Services Program: 1:1 Nursing - On Call Reports to: Nursing Supervisor, School Health Services Classification/Range: Licensed Schedule: Varies per student needs The ONCALL 1:1 RN provides direct nursing services to one student to allow the student with complex health needs to fully access their education.
NATURE & SCOPE:
This position is subject to board policies, administrative rules and procedures, department regulations, and applicable state and federal statutes. This position carries/does not carry administrative authority. All commitments outside of authorized programs and services are subject to prior approval by the supervisor of this position.
ESSENTIAL JOB FUNCTIONS:
* Facilitate health services for identified student by:
* Developing a Student Health Management Plan (SHMP) and providing a summary of health information including recommendations for the school program, including:
* Nursing care plan
* Emergency plans as indicated
* Delegation and assignment of nursing procedures to Unregulated Assistive Persons (UAPs) as indicated
* Transportation plans
* IEP goals
* Collaboration with the Complex Needs Nurses as indicated
* Providing continuous observation and assessment of student health needs
* Periodically re-evaluating student health status, identified health concerns, goals and placement to determine if modifications could be necessary.
* Act as liaison between the family, school staff and medical providers to ensure effective communication and to make sure the student is being provided the least restrictive and safe school environment.
* Participate as a member of the multidisciplinary team and make appropriate recommendations regarding how the student's health needs fit within the IEP.
* Demonstrate commitment to maintain professional practice competency.
* Contribute to a positive and productive work environment.
* Consult with the Nursing Supervisor for assistance and/or direction when needed.

WORKING CONDITIONS:
Work is performed primarily on a school campus, both indoors and outdoors, including classrooms, gyms, cafeterias, and community locations. Duties involve standing, walking, climbing, bending, pushing, pulling, and lifting. The role requires the use of durable medical equipment and standard office equipment (e.g., computer). Accompanying the student on the bus and occasional home visits, travel to health-care or social-service providers, are also required. The position frequently involves working with students, families, or staff in high-stress situations.

This 12-bed unit serves as a critical step between the Neuroscience Intensive Care Unit (NSICU) and the Acute Care setting, supporting early recovery, close neurological observation, and safe transitions along the continuum of care. The IMCU emphasizes patient safety, early mobility, multidisciplinary collaboration, and evidence-based neuroscience nursing within a supportive and professional work environment. This unit utilizes a multidisciplinary team approach to the care we provide to our patients: 40kg weight minimum, age generally not less than 16 years with no upper age limit.

Function/Duties of Position
The Comprehensive Pain Center (CPC) is an outpatient specialty clinic treating patients with chronic, acute, and cancer pain. We focus on diagnostic and therapeutic pain procedures. Top diagnoses we see include back pain, neck pain, cancer pain, spinal issues, neuropathy, headaches and migraines, pain that can affect joints (knees, hips, shoulders) or other body parts (abdominal, pelvic). We have a team of Physicians, Physician Assistants, Psychologists, and a Naturopath.
The RNs at the CPC provide care to our procedure patients before, during, and after procedures. We have two procedure rooms and staff the area with 5-6 nurses per day. Most procedures are done under moderate sedation and using fluoroscopy or ultrasound guidance. Nurses will wear a lead vest when in the procedure room, and need strong skills placing IVs.
The workflow functions like a procedural care unit. Nurses with previous experience in a PACU, surgical unit, or procedural setting will find the workflow familiar.
Our nursing team is closely integrated with each other and our providers, which includes physicians, fellows, and a Certified Nursing Assistant.

The Cardiology Clinic is a multidisciplinary practice with primary locations at the Center for Health and Healing and Beaverton Clinics with some programmatic aspects on Marquam Hill. The RN will partner with the Electrophysiology (EP) team and other clinical staff throughout OHSU to coordinate the care of cardiovascular patients with CIEDs. The RN will coordinate care in the outpatient setting for long term in person and remote monitoring. The RN will provide and improve quality patient outcomes for the patient and families for the Cardiology Clinic in collaboration with the multidisciplinary team.
Function/Duties of Position
CLINICAL SERVICES: Working with the various members of the multi-interdisciplinary team in delivering outpatient care to patients with CIEDs:
Conduct interviews with patient, family, and other health care agencies to obtain past medical records and social and medical history to assess their understanding of the plan of care regarding cardiac monitoring devices.
Independently perform CIED interrogation in the outpatient setting
Problem solve device function, settings, arrhythmia episodes, and interventions.
Work closely with EP team and other multidisciplinary teams, ensuring quality CIED and medical follow up care.
Analyze clinical data and compose written CIED management summaries, clinic visit notes, and letters.
Triage patient clinical concerns, symptoms, or other relevant data in relation to their cardiac device care.
Communicate and problem solve with multidisciplinary team to solve complex patient care issues and CIED function.
Educate patient and family regarding all aspects of disease management and diagnostic and device management follow up.
Proactively review patient schedules to ensure efficiency in clinical workflows and identify and resolve potential conflicts.
Ensure appropriate resources are available to support patient care and procedures in the outpatient setting.
Educate patient and family on signs and symptoms that require a follow up telephone call or clinic visit.
Collaborate with medical faculty, nursing staff, and other health care agencies to facilitate patient and family needs.

Acute Care (5A) and Family Medicine (5C) is a conceptually contiguous Family Medicine and General Medicine Acute Care unit with 14 beds on the Family Medicine unit and 23 beds on the Acute Care unit.
The patient population is adults who have a variety of medical diagnoses such as Diabetes, End Stage Renal Disease, End Stage Liver Disease, Gastro-Intestinal Disorders, Adult Cystic Fibrosis, chronic and acute pain, infection, diagnostic testing, and psychiatric patients with a medical overlay. Patients require acute medical and nursing interventions such as intravenous fluids, nutritional support, pain management, antibiotics administration, diabetic management, respiratory care, remote telemetry, prepping for procedures, and education to optimize self-care. 5A/5C admits behavioral/psychiatric patients with medical issues that may have suicidal ideation or other behavioral risks that necessitate a court hold. Creating a therapeutic environment for this population includes establishing a therapeutic nurse-patient relationship that facilitates gathering critical patient data and implementing the interdisciplinary plan of care.

This 12-bed unit serves as a critical step between the Neuroscience Intensive Care Unit (NSICU) and the Acute Care setting, supporting early recovery, close neurological observation, and safe transitions along the continuum of care. The IMCU emphasizes patient safety, early mobility, multidisciplinary collaboration, and evidence-based neuroscience nursing within a supportive and professional work environment. This unit utilizes a multidisciplinary team approach to the care we provide to our patients: 40kg weight minimum, age generally not less than 16 years with no upper age limit.

The Hermiston School District is the largest district in eastern Oregon, exceeding 5,600 students in grades kindergarten through 12, with the mission of serving the needs of all children with rigorous program choices, high expectations, mutual respect, and excellence.
