Responsibilities The clinical pharmacist optimizes patient care by providing appropriate, safe, and cost-effective pharmaceutical care including drug dispensing, IV admixture program, and providing drug information. Works under the direction of the Director of Pharmacy and/or the Pharmacy Operations Manager and will oversee the work of pharmacy technicians, students, and support staff. Qualifications Job Requirements: * B.S. or Pharm D. from an ACPE-accredited School of Pharmacy. * Active Pharmacist license in good standing with the appropriate State Board of Pharmacy. * Other applicable state requirements - i.e., sterile compounding, etc. * BLS certification must be obtained within 14 days of hire or transfer into the role and prior to providing direct patient care. * ALS/ACLS certification (within 90 days of hire; ALS/ACLS required for attending Code Blue). Preferred Job Requirements: * Doctor of Pharmacy from an ACPE-accredited college of pharmacy. * 1 year of inpatient hospital pharmacy experience.

Hours are typically 8am-5:00pm * The Advanced Clinical Pharmacist Heart Transplant is responsible for providing all levels of pharmacy clinical services for the Heart Transplant patient. Job Summary and Qualifications As a Clinical Pharmacist you will demonstrate proficiency in all pharmacist functions by consulting on medication orders with nursing and medical staff, validating patient histories and current medication profiles. You will identify medication variances and adverse drug reactions using formulary alternatives and handling complaints.
