The Home Health ICM Coordinator plays a key role in supporting our Utilization Management team to ensure safe, high-quality, and cost-effective care for patients enrolled in UCLA Medicare Advantage and Intensive CaseManagement programs. RESPONSIBILITIES:
Under the supervision of the Guardian Scholars Supervisor, support a caseload of approximately 25-35 high need youth requiring daily driving to meet youth where they are in the community.
Implement evaluation and assessment tools to ensure programmatic data and outcomes can be collected and analyzed for outcome management.
Support youth trainings and workshops, as needed.
Ensure Salesforce is up to date with accurate files, notes, and documentation. Salesforce must be updated daily per phone calls, emails, and visits.
Conduct monthly in-person meetings with youth to address challenges, track progress, and provide support, supplemented by additional check-ins via phone, text, or email as needed.
This position requires regular evening and weekend hours to meet participant needs.
Address any youth issues as they arise including career navigation, homelessness, and academic needs.
As part of the team, support youth development program components including but not limited to college and career tours, Women's and Men's Leadership Network, Advocates for Change, care packages, Mentoring Department, internships, Guardian Scholars Resource Fair, and youth training and events.

Minimum Qualifications:
Bachelor's degree in social work, public health or related field and/or 2+ years providing MOUD/MAT navigation and harm reduction focused casemanagement services to high-risk populations, including knowledge providing prescriptions for suboxone, naltrexone and other medications (and methadone dispensaries) used to support treatment adherence for clients who access HEAT MOUD services.
Must have experience and training in overdose reversals, de-escalation techniques, motivational interviewing. And experience providing street-based outreach services in urban settings and homeless encampments.
Must have an understanding of the social determinants of health and how they impact people experiencing homelessness, actively engaged in drug use with proximate social stressors.
Ability to establish and maintain effective collaborative relationships across Glide wide programs as well as with city agencies across San Francisco and the wider Bay Area.

Develop and prepare outreach and educational materials targeted to engage the broader community in HEAT MOUD programs. Assist the leadership team in coordinating special events that promote HEAT MOUD efforts as well as the full array of HEAT and Glide wide programming.
Participate in and support the full range of HEAT program services such as SAS, HIV/HCV/STI testing and navigation, mobile outreach services and community outreach programming when necessary.
Participate and engage in regular professional engagements including individual supervision, team meetings, community meetings, trainings, and other professional develop opportunities that support GLIDE's overall organization goals and strengthens HEAT's service delivery systems and client outcomes.
If needed, the MOUD casemanager will be eligible for jail clearance in San Francisco and other Bay Area counties in order to enter city and county jails to advocate on behalf of incarcerated MOUD clients.
Minimum Qualifications:
Bachelor's degree in social work, public health or related field and/or 2+ years providing MOUD/MAT navigation and harm reduction focused casemanagement services to high-risk populations, including knowledge providing prescriptions for suboxone, naltrexone and other medications (and methadone dispensaries) used to support treatment adherence for clients who access HEAT MOUD services.
Must have experience and training in overdose reversals, de-escalation techniques, motivational interviewing. And experience providing street-based outreach services in urban settings and homeless encampments.

ABOUT THE ROLE:
The CaseManager works out of the Lighthouse Transitional Living Program in West Los Angeles. Lighthouse TLP is a housing first low-barrier, high tolerance program providing housing and comprehensive services to youth ages 18-24 who are experiencing housing instability. The CaseManager is responsible for oversight and safety of the property and residents. This includes, but is not limited to, monitoring and maintaining cleanliness of the site and living spaces, carrying out day-to-day safety protocols, responding and de-escalating crises, conflict management among residents, and promoting a holistic approach to the wellness of our residents. This position requires some driving and lifting.
WHAT YOU'LL DO:
Process incoming referrals and screen for eligibility and manage referrals waitlist
Coordinate and assist with youth move-ins, and exit and sustainability planning.
Complete intake and other program documentation requirements, ensuring that charts meet quality assurance standards
Utilize culturally-relevant active listening and motivational interviewing skills with participants to identify possible supports in their network
Ensure that youth receive a Next Step Tool assessment
Work with youth on creating a housing and service plan and provide intensive casemanagement services to support youth
Meet with youth on a weekly basis to ensure they are progressing towards their plan goals
Connect youth resources to address their basic, physical, economic and psychosocial needs and follow-up to ensure the participant successfully accesses and initially retains housing
Provide housing navigation, as needed, to connect youth to housing resources
Ensure health and safety of youth including crisis intervention, risk assessment and safety planning
Ensure services are documented (e.g. face to face, phone and collateral contacts) in all relevant databases in a timely and thorough manner
Work with participants on budgeting, connecting to benefits, employment and determining what level of assistance is necessary to be stably housed
Participate in trainings, meetings, and technical assistance such as learning collaboratives
Other duties as assigned to support the mission of the organization
Requirements
WHAT YOU'LL NEED:
Bachelor's degree in social work, psychology, or any related field is preferred. Related work or lived experience will be strongly considered in place of or in addition to a Bachelor's degree.
2+ years in experience providing direct services to high-risk transition age youth is preferred.
Working knowledge of child and adolescent social, emotional, and cognitive development, trauma informed care, and harm reduction service models
Work within legal and ethical guidelines
Flexibility with schedule based on program needs
Commitment and passion for the mission, vision, values and culture of SPY
A self-starter who is prepared to drive innovative solutions
Excellent communication and organizational skills
An ability to work in a fast paced environment that requires effective collaboration
A valid California driver's license, auto insurance, and the ability to transport youth

YOUTH CASEMANAGER ESSENTIAL FUNCTIONS:
Provide comprehensive casemanagement services for the Hope Gardens (HG) Youth Program.
Create, implement, and monitor individual service plans for each enrolled youth at HG.
Follow the Trauma Informed Care philosophy when supporting and addressing the needs of the youth and families served at HG.
Use Crisis intervention therapeutic effective strategies and techniques in and out the classroom such as collaborative helping strategies and harm reduction practices related to children and youth.
Connect the youth to resources in the community.
Conduct periodic assessments to evaluate social, spiritual, emotional, physical, and cognitive needs and development.
Work alongside Family CaseManagers to ensure the youths' needs are being met through CFT meetings, school, IEPS, the Regional Center, DCFS Parent Partners, and other outside resources.
Ensure that services, including counseling, casemanagement, education, personal development, and job training, are being provided to apprentices and community assistants under the Youth Program.
Collaborate with community leaders, organizations, and public agencies to promote the organization's community service programs to benefit the children and youth at Hope Gardens.
Works alongside the Executive Program Director, Program Manager, and Chaplains to ensure students' needs are being adequately met while at Hope Gardens.
Maintain accurate case logs and files for youth enrolled in the Youth Program.
Meet monthly with parents/guardians regarding their child's Individual Service Plan and goals.
Help coordinate and assist with activities for youth trips and summer camps.
Local travel, as required.
Conduct other tasks and projects as assigned by the Executive Programs Director.
