Role Overview
The Mental Health Intake Clinician is often the first point of contact for individuals seeking treatment at our outpatient center. This role is responsible for conducting comprehensive diagnostic assessments, determining appropriate levels of care, and ensuring a smooth transition into services.
This position is vital to ensuring clients feel heard, supported, and connected to the right resources from day one.
Key ResponsibilitiesClinical Assessment & Intake
Conduct initial mental health and substance use diagnostic assessments using evidence-based tools.
Evaluate client history, presenting concerns, risk level, medical needs, and social determinants of health.
Determine level-of-care recommendations and collaboratively develop initial treatment plans.
Facilitate crisis assessments as needed, including safety planning and referrals for higher-level care.
Care Coordination & Communication
Communicate assessment results and clinical recommendations with clients, families, and treatment teams.
Complete referral coordination for internal services (therapy, groups, psychiatry) and external supports when indicated.
Ensure accurate, timely documentation in accordance with Medicaid, HIPAA, state, and federal guidelines.
Participate in interdisciplinary staffing and treatment meetings to support client engagement and retention.
Professional Standards
Maintain current knowledge of outpatient behavioral health regulations, TJC standards, and ethical guidelines.
Uphold confidentiality, trauma-informed care principles, and culturally responsive engagement.
Foster a welcoming, compassionate environment for individuals entering treatment.
Education Requirements
Bachelor's degree in Social Work with LSW and relevant clinical or assessment experience
OR
Master's degree in Social Work, Counseling, or a related behavioral health field
Candidate must be 21 years or older.

The Role Itself
Conducts bio-psychosocial assessments under supervision.

Position Summary:
This position provides direct care services to clients with mental health crisis needs. These services are provided through individual, group and family therapy, through the diagnostic assessment process or through individual community based rehabilitative services and/or group rehabilitative services including linkage and advocacy as necessary. The Individualized Treatment Plan is measurable and specific. Provides individual therapy as needed.
Provides restorative interventions and training to improve interpersonal, community integration and independent living skills when the mental illness impacts the client's ability to function in and adapt to home, school, work and community settings.
Makes referrals, both internally and externally, according to the ITP and monitors outcome.
Monitors and records symptoms of decompensation. Reports symptoms to client's provider.
Coordinates with other service providers and follows through with client needs.
Obtains and serves as health officer, as needed.
Education & Experience Requirements:
Requires a bachelor's degree in social work or a master's in counseling. Master's in social work preferred.
Must be at least 21 years of age.
