About the Role:
The Inpatient RN Case Manager at Sinai Hospital works with the clinical team and medical provider to coordinate and implement safe discharge plans for patients. Their main goals are to improve patient well-being, outcomes, and ensure healthcare services are used efficiently and timely.
This position is scheduled Every Weekend Sat. & Sun. for 12-hour shifts.*

Key Responsibilities:
Assessment & Planning: Conducts initial and ongoing assessments to determine patient needs for care coordination and discharge, then develops a focused discharge plan, especially for high-risk patients.
Intervention & Collaboration: Works closely with the clinical team and medical providers to put the discharge plan into action.
Continuous Improvement: Stays current with healthcare trends, regulations, and payer requirements related to patient care, discharge planning, and benefits.
Requirements:
Basic professional knowledge; equivalent to a Bachelor's degree; working knowledge of theory and practice within a specialized field
BSN preferred; ADN required
3-5 years related experience; Prior experience with inpatient case management/discharge planning required
Registered Nurse License - Current Maryland license or eligibility to obtain Maryland license
Case Mgmt cert preferred within 3-5 yrs of hire

* Assists chairs and department heads with recruitment, hiring, and development of part-time faculty for the division; assists the dean with recruitment and selection of full-time faculty.
* Participates in the FT faculty evaluation process, at the direction of the dean.
* Works with chairs and department heads to ensure appropriate faculty assignments, schedules and workload.
Community Relations/Outreach
* Serves on college-wide committees and works with other administrators in a team environment.
* Serves as the division liaison to other areas and divisions within the College, supporting day-to-today operations of the division, as directed by the dean.

Salary: $16.00-$18.00 per hour, based on the candidate's skills, experience, and qualifications.
Schedule: Monday-Friday: Shifts start between 7:30-8:00 am and end at 6:00 pm, and every other Saturday: 8:00 am-1:00 pm.
Schedule Options:
* Full-time: Four 10-hour shifts per week with a consistent schedule
* Part-time: Approximately 20-25 hours per week
Key Responsibilities:
* Assist veterinarians and vet techs during examinations, treatments, and surgeries
* Handle and restrain animals safely and compassionately
* Prepare examination rooms and surgical areas by cleaning and sterilizing equipment and surfaces
* Perform basic laboratory tests, such as collecting blood, urine, and stool samples
* Administer medications and vaccinations under the supervision of a veterinarian
* Feed, bathe, and exercise animals as needed
* Maintain accurate medical records
* Communicate with pet owners about their pets' care, treatment plans, and follow-up visits
* Clean and maintain the clinic, including stocking supplies and ensuring a safe and sanitary environment
* Greet clients and their pets, complete the appointment sign-in process, confirm appointment details, and escort clients to exam rooms.
* Ensure accurate service and product sales accounting, handle payments, process credit/debit cards, and schedule future appointments.
* Answer client calls for appointments, hospital inquiries, medication refills, and other requests. Make outbound calls for appointment reminders and follow-ups.
Qualifications:
* Previous experience working with animals in a veterinary setting.
* Technical know-how in areas such as disinfection, administering medication, and using diagnostic equipment.
* Physical ability to handle animals safely and compassionately.
