1. Record Review:
* Completes initial medical records reviews of patient records within 24-48 hours of admission for a specified patient population to: (a) evaluate documentation to assign the principal diagnosis, pertinent secondary diagnoses, and procedures for accurate MS-DRG assignment, risk of mortality and severity of illness; and (b) record in business partner designated CDI tool and/or host medical record system.
* Conducts follow-up reviews of patients every 24-48 hours or as needed up through discharge to support assigned working MS-DRG assignment upon patient discharge, as necessary.
* Formulate physician queries regarding missing, unclear, or conflicting health record documentation by requesting and obtaining additional documentation within the health record, as necessary.

2.CDI

The primary role of the **Home Infusion Nurse** is to provide clinical care and education to home-based patients receiving a wide range of parenteral medications dispensed from Optum Infusion Pharmacy (the "Company"). The most common route of administration is intravenous, however medications may also be given via subcutaneous infusion or injection and intramuscular injection. Service delivery takes place in patient homes and also in ambulatory infusion suites, where available. All infusion services and medication delivery is provided in accordance with Optum Infusion Pharmacy policies, procedures and therapy-specific protocols, and based on comprehensive orders received from a physician or licensed practitioner. **Schedule will be flexible Monday-Friday, 40 hour work week. This role includes travel throughout Auburn and surrounding cities, with drive time included in the workday. **Primary Responsibilities:**
+ Establish and maintain positive clinical rapport with patients and their families, working closely with them in the provision of infusion therapy, education related to self - care and self - administration, and clinical monitoring of patient response to therapy
+ Assess patient appropriateness for infusion therapy, including safety of the home environment, availability of caregiver support, and willingness of patient and / or caregivers to become as independent in therapy administration and monitoring of response to therapy as possible
+ Obtain and document patient's medical history at the start of care and ongoing, particularly as related to the diagnosis being treated with the prescribed infusion therapy
+ Complete all required clinical documentation thoroughly and timely, including nursing plans of care (POC), plans of treatment (POT), visit notes, clinical summaries, medication profiles, discharge summaries and any updates provided to members of the patient's care team, as well as obtaining patient and / or caregiver signatures on relevant consents and required forms

**Primary Responsibilities:**
+ Assesses the health status of the patient in order to develop an individualized plan of care by collecting, categorizing and interpreting data (physical and psychosocial) from documented information, observation, patient/family interview and other health care team members
+ Develops a nursing care plan based on the assessment of the patient that included nursing diagnosis, patient goals and nursing interventions necessary to achieve these goals
+ Implements the nursing care plan to provide optimal individualized care for the patient
+ Instructs the patient and family to assist the patient in the achievement of optimal health status
+ Documents nursing activities to provide a permanent record, for continuity of care, quality improvement and professional accountability
+ Identifies the patient to ensure that the patient, procedure and operative site correspond
+ Plans, supervises, and assists with the transfer and transport of the patient to protect the patient and personnel from injury
+ Positions the patient to provide access to the operative site while maximizing patient safety and comfort
+ Establishes and maintains a surgically clean environment to reduce the potential for infection by applying the principles of antisepsis, decontamination, disinfection, sanitation, sterilization, and environmental monitoring
+ Establishes and maintains a sterile field to reduce potential for infection by teaching and supervising the application of principles of aseptic technique and continual monitoring of the sterile field
+ Establishes and maintains a safe environment to protect the patient and the health care team by monitoring the patient (physiologically and psychologically) and the environment and by implementing safety procedures
+ Coordinates support services needed during surgery by anticipating and communicating patient and surgical team needs
+ Communicates information to patient/family
+ Organizes and coordinates supplies, equipment, and team members to maintain an efficient, effective and safe intraoperative environment through ongoing patient and situation assessment, priority setting, procurement of supplies and arrangement of equipment, by teaching, directing, communicating and collaborating with health care team members and support service personnel
+ Communicates information to recovery room personnel to promote continuity of care
+ Evaluates effectiveness of nursing care to determine degree of goal attainment by comparing patient outcomes with patient goals
+ Evaluates products to determine their safety, effectiveness, use and cost effectiveness
+ Executes nursing activities to provide life support in an emergency situation by taking appropriate action and/or providing direction/assistance to health care team members
+ Initiates appropriate intraoperative behaviors as a circulator/scrub
