Responsibilities
The Clinical Manager conducts the IDT meetings, supervises and supports clinical staff through evaluation and field supervisory visits and assures ongoing development of the team's clinical skills. Responsibilities
The RN Care Manager also provides referrals to community resources to meet the psychosocial needs of patients. He/she demonstrates empathy, excellent communication skills and the ability to think critically as a member of the interdisciplinary team. The Case Manager also has technical competency in the use of computer systems and applications, including the Electronic Health Record.
Quality Provision of Services:
Schedules and conducts routine telephonic visits with patient and/or caregiver to perform care management services.
Completes assessments with each patient to identify clinical and psychosocial needs and risks that impact health status.
Works with all stakeholders to create individualized care plans to meet clinical and psychosocial needs while maximizing healthcare responses, quality and cost-effective outcomes.
Facilitates communication and coordination among stakeholders, involving the patient in the decision-making process to minimize service fragmentation.
Assists the client in safe transitions across care settings.
Manage and triage incoming calls from patients or caregivers with back up from other team members as needed.
Completes all required documentation in the electronic health record at the time of service.
Assists with the development and implementation of goals, objectives, priorities and projects for the Applications Team.
Cross trains and assists in other areas of the Innovations HUB as requested.
Qualifications
Experience Requirements
Must have 3 years of RN experience, with 1 year as a Care/Case Manager. Geriatrics or primary care experience desired.
Education Requirements
Must have a degree in Nursing from an accredited nursing program. A BSN degree is preferred.
Required Certificates and/or Licenses:
Must have an active RN license in the state of service. Healthcare Provider BLS (CPR) is required to practice in the State of Maryland and the District of Columbia. Case Management certification is preferred.

Acts as the Company representative at assigned facilities while facilitating referrals to all service lines; works closely with referring hospitals, physicians, facilities, patients, families, and the general public.
Communicates frequently with other members of the IDT.
Provides all necessary clinical communication timely using SBAR.
Discusses any potential needs with after-hours staff.
Develops strong relationships with case managers, physicians, etc. at facilities.

• Educates patient, family, caregivers and other health professionals about disease process and decline, prevention, palliative interventions, care giving, dying process and safety practices.
- Participant visit frequency dependent on risk score/needs to be determined
- Home visits to assess home safety, medication compliance, nutritional compliance, DME compliance- ability to live safely in the community.
• Reports changes in the patient's condition to appropriate members of the IDT or other health professionals.
• Participates with the IDT to evaluate hospice referrals/admissions for level of care appropriateness.
- Attends daily IDT collaboration meetings
• Presents concise and pertinent oral and written reports to IDT; respects and encourages input from all disciplines.
• Communicates accurately and completely to physicians, staff members, patients, families, and supervisors; utilizes positive approaches when working with others.
• Supervises patient care provided by Community Health Workers and Home Health Aides as requested.
• During times of emergencies (i.e. Hurricanes, etc.), the RN, Case Manager may be required to report to work at a location designated by the company, to ensure continuity of services. This may include reporting to work ahead of your scheduled date/time due to planned lock down of unit, and staying overnight(s) based on duration of emergency.

The Registered Nurse (RN) Hospice Case Manager works in collaboration with the Interdisciplinary Discipline Team (IDT) to foster a quality experience in accordance with the patient and family's care plan and wishes. The RN Case Manager manages symptoms of the patient's disease and re-evaluates effectiveness of interventions and adjusts the care plan as needed.
Location: Falls Church, VA - home care
Hours: Monday-Friday 8:00am-5:00pm, occasional on call coverage. 2 holidays/year

Responsibilities
The Registered Nurse (RN) Hospice Case Manager provides, coordinates, teaches and supervises the nursing care of Hospice patients and families/caregivers. The RN Case Manager makes initial and ongoing nursing assessments, judgments and treatment decisions based on patient and family/caregiver needs. The RN Case Manager is responsible for supervising Licensed Practical Nurses, Certified Nursing Assistants, and Patient Care Volunteers. May be asked to rotate to other regions/departments.
Qualifications
Experience Requirements
Must have one year of staff nurse experience in a comparable setting/unit. Hospice and/or palliative care experience is strongly desired.
Education Requirements
Must have a degree in Nursing from an accredited nursing program. A BSN degree is preferred.
Required Certificates and/or Licenses
Must have licensure from the governing body for practice for the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia, based on the state/jurisdiction of practice. Licensure from compact state is acceptable. Healthcare Provider BLS (CPR) is required to practice in the State of Maryland and the District of Columbia. A CHPN certification is a plus.
Driver's License
Must have a driver's license in good standing and access to a vehicle during working hours.

Responsibilities
The Pediatric RN Case Manager provides visits to pediatric patients. He/she arranges for, teaches, and supervises nursing care of pediatric patients in home care and facility settings while collaborating with other members of the Interdisciplinary Group. The Pediatric RN Case Manager makes nursing assessments and uses critical thinking skills to provide the best possible care.
