Primary Job Functions
1. Assess, secure, coordinate and monitor satisfaction of services and progress toward outcomes.
* Evaluate client needs by securing information on medical, psychological and social factors contributing to client's situation.
* Develop support plans with outcomes and action steps toward achievement.
* Provide case management services for program clients.
* Coordinate linkage of needed services and advocate for client needs.
* Conduct home visits.
* Prepare case notes, reports, statistical reports and other materials.
* Complete invoicing process as assigned by supervisor for specific clients.
* Maintain accurate file documentation.
* Review support plans; perform follow-up to determine quantity and quality of service provided to client and status of client's case.
* Provide crisis management assistance as necessary.
* Verify client's eligibility for Medicaid and obtain Medicaid number.
* Submit monthly program billing.

1. Coordinate the daily operations of the Family Services Team program.
* Review Family Services Planning Team (FSPT) applications for eligibility.
* Perform data entry of client data into client information systems as required.

1. Provide crisis intervention services to victims of crime or trauma, including emotional support, safety planning, and resource referrals.

* Identify and assess client and family needs; conduct intake screening and assessments to determine appropriate services.

Primary Job Functions
1. Facilitates, coordinates and tracks the school health care programs referral network and processes.
* Serve as the point of contact between CPS administration, school nurse, medical clinic, behavioral health department and other health related agencies/departments providing services.
* Serve as entry point to the Community Partnership School for students and parents.
* Supervise and coordinate all medical clinic referrals, including health, dental, vision and mental health referrals.
* Participate in Intervention Team meetings to provide information on behavioral interventions and relevant referrals as needed.
* Track and follow up on all referrals made in and out of school.
* Establish referral network/linkages with outside agencies.
* Record professional assessments, treatment plans, and progress notes.
* Utilize data management system for proper documentation.
* Participate in crisis intervention.
* Implement and oversee eligibility, billing and other reimbursement procedures.
* Participate in the quality improvement process.
* Coordinate Health & Wellness events for students, staff & community.
* Submit all documentation in a timely manner (data, trackers, etc.)
* Possible direct transport or arrange for transportation of clients.
2. Contribute to a positive, engaging work environment.
* Develop a strong knowledge base and stay current on job-related issues and trends.
* Participate actively in departmental meetings, training and education, as well as the quality process.

Primary Job Functions
1. Provide high quality care and supervision of children and adolescents in CHS residential care, focusing on their safety and security while supporting individual growth and development in all facets of daily life, including their physical, mental, emotional and social well-being.
Residential Facility Operation
* Develop and execute household management operations, including maintaining the order, safety and cleanliness of the residential facility and grounds (e.g., domestic cleaning, laundry, yard maintenance, etc.).
* Prepare and serve meals using provided menus.
* Supervise residents in assisting with chores and maintaining their personal hygiene and belongings.
* Dispense medication as instructed.
* Coordinate and execute in designated timeframes, resident services, such school enrollment, medical care and other service referrals, such as parenting and counseling.
* Provide resident transportation to school, medical appointments and other activities.
* Operate and maintain company vehicle in clean condition and good repair.
* Provide first aid treatment when needed.
* Conduct and document required emergency and episodic drills (e.g., including fire, hurricane, medical emergency, etc.).
* Serve as a Mandated Reporter; report suspected child abuse or neglect to the appropriate authorities, as required by law.
* Maintain confidentiality of resident personal and medical information.
* Maintain complete and accurate daily records; document all reportable incidents in the CHS Incident Reporting System.
* Complete all forms and other paperwork to comply with funding and licensing requirements.
* Maintain coverage during designated shift; continue to provide shift coverage until next assigned team member arrives.
Child/Adolescent Growth & Development
* Provide appropriate direction to residents in their daily routine, conducting oneself as a role model.
* Provide daily living and social skills training and development.
* Engage in and supervise age-appropriate play activities.
* Facilitate positive relationships between residents and their families.
* Teach life and independent living skills using individually designed service plans.
* Provide positive reinforcement; implement trauma-informed behavior modification techniques as instructed.
