General Surgeons of Western Colorado (GSWC) specializes in all aspects of General Surgery including colon, rectal, endocrine, surgical oncology, trauma and advanced laparoscopic surgeries. We are the largest center for minimally invasive surgery in Western Colorado, and we are now Western Colorado's first center to offer Mammosite treatment for early breast cancer.
Job Duties:
Employee will provide support to physicians in general/trauma surgery. As a General Surgeons of Western Colorado Advanced Practice Provider (APP), the employee will be assigned to a rotating schedule including day, weekend, and night shifts on the Trauma Acute Care Surgery (TACS) service with occasional elective general surgery and clinic coverage.
General Duties:
Perform complete evaluations on patients including consults and admission for acute care general surgery patients and multisystem trauma patients. Perform complete pre-operative H&P?s on surgical patients, identifying and alerting the surgeon to any disqualifying conditions that present. Daily rounding on inpatient trauma/general surgery patients. Order and interpret appropriate lab, x-ray, and special studies. Establish a working diagnosis based upon acquired data and develop a treatment/surgical plan. Provide appropriate follow-up care. Exercise judgment on conditions requiring consultation, referral, or evaluation by the supervising physician. Evaluate, screen and counsel patients on health maintenance and promote utilization of community resources where appropriate.
Hospital Duties:
Conduct daily hospital rounds on TACS patients both in ICU and on the floor. Perform surgical first-assist duties for all patients on TACS service and occasional elective coverage. See consults and admissions for acute care general surgery and multisystem trauma patients. Order appropriate labs, special studies, IVs, diet and therapeutic interventions, as appropriate.
Office Duties:
See new and follow-up patients and provide intervention that is problem-oriented for his/her condition. Refer patients, when appropriate, to the supervising surgeon and assist in care, treatment, and follow-up as needed. Perform and/or assist in office procedures. Order and interpret lab, x-ray, special studies, and clinical procedures and provide appropriate care and follow-up. Provide phone consultation for patient complaints and prescription refills.

The primary role of the NursingAssistant (CNA) is to provide assigned residents with routine daily nursing care in accordance with established facility nursing care procedures and, in a manner, compliant with all state and federal regulations as well as with any applicable facility policies and procedures.
QUALIFICATIONS:
High school graduate or equivalent.
Must be a CertifiedNursingAssistant in the state of CO.
Ability to safely perform the essential functions of the job without harm to self or others.
Experience in a long-term care setting preferred.
Ability to prioritize duties.
Ability to understand and follow work assignments.
Ability to work as part of a team.
Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation.
ESSENTIAL FUNCTIONS:
Assists residents with personal cares, including bathing, dressing, grooming, oral hygiene, bed mobility, toileting and eating. Documents ADL'S appropriately.
Follows all shift routines, policy and procedures regarding accurate performance of vital signs, weights, etc. Understands/supports Resident Bill of Rights.
Serves meals and assists residents with eating. Records accurate food and fluid intake as assigned. Reports any chewing or swallowing difficulty to the nurse.
Assists the resident with the restorative nursing program including but not limited to, passive and active range of motion, ambulation with appropriate assistive devices and use of transfer belt.
Uses safety standards at all times in transfer gait of residents: (a) Uses 2 trained staff members for all mechanical lifts, gait belt and/or proper mechanical equipment for all transfers/ambulation of residents. (b) Uses proper body mechanics. (c) Ambulates/ transfers residents per plan of care. (d) Positions residents to maintain body alignment per policy and procedure and plan of care.
Reports promptly to the nurse any changes in the resident condition.
Answers call lights promptly and determines residents' needs.
Neatly makes and changes bed linens.
Ensures that residents are clean, neatly dressed according to preference or age, and well-groomed.
Reports any indication of resident illness to the nurse. Completes Stop 'N' Watch forms on Point of Click or uses paper form.
Follows standard precautions and works within OSHA guidelines. Follows infection prevention and infection control policy/procedure: (a) Follows resident skin care protocol. (b) Properly cleans and stores resident equipment. (c) Maintains sanitary conditions in common areas (e.g., tub rooms, dining areas, and hallways). (d) Follows basic principles of linen handling. (e) Follows principles of universal precautions. (f) Follows isolation precautions. (g) Reports any concern related to infection control to nurse.
Observe and report any noted changes in mental status to the nurse.
Report to charge nurse all incidents/accidents that occur involving employee, residents, and visitors.
Treats and ensures that residents are treated with respect and dignity. Shows respect for resident's right to confidentiality, dignity and privacy.
Immediately reports any suspected abuse or neglect per facility policy.
Remains aware and takes special precautions to ensure a clean and safe environment for residents and staff.
Reports any equipment malfunction to designated employee.
Completes miscellaneous tasks related to care of residents (e.g., transporting patients).
Reports to nurse prior to shift and at end of shift.
Accurately documents vital signs, weights, intake and output as delegated per facility protocol.
Utilizes electronic systems of documentation as required per facility policy.
Attends designated nursing meetings as scheduled.
Develops and maintains a good working relationship with inter-departmental personnel.
Attends educational offerings and obtains at least twelve (12) hours of continuing education a year per state regulations.
Follows federal and state rules and regulations governing nursing facilities.
Reports emergency situations immediately to nurse.
Responds immediately to an emergency. This includes assisting with the evacuation of residents.
Establishes and maintains respectful communication with all employees, residents and families.
Attends required in-services and completes assigned on-line modules.
Uses proper chain of command to communicate concerns.
Complete all assigned in-services and on-line modules.
Serves on, participates in and attends various committees and meetings of the facility to enhance interdisciplinary process.
Participates/supports activities of facility committees.
Other duties as assigned or directed by nurse.

The CertifiedNursingAssistant is responsible for providing and assisting participants with activities of daily living, restorative and supportive care necessary to the preservation of assisting participant's in living independently in the community. Assists in monitoring and reporting the health status and providing a safe environment for the PACE participants of Senior CommUnity Care. Functions as a member of the Interdisciplinary Team. This position may require travel to participant homes.
QUALIFICATIONS:
High School diploma or equivalent required.

Must have medical clearance for communicable diseases and up-to-date immunizations after having direct participant contact.

ESSENTIAL FUNCTIONS:
Consults each day with the Personal Care Supervisor (or designee) to be updated on Senior CommUnity Care participant's status, changes in plan of care and to confirm scheduled visits.
Under the direction and supervision of the Home Care Manager and Personal Care Supervisor assists participants in the home and in the day center with the following duties:
Personal hygiene such as skin, mouth, hair care and bathing/showering.
Assists participants in and out of bed, assisting with ambulation.
Assisting with prescribed exercise which participants and aide have been taught by appropriate personnel.
Prepares meals, including therapeutic diets, assists participants with meals and feeding.
Assist participants to the bathroom or in using commodes, bedpans, or urinals.
Assisting participants with medications which are ordinarily self-administered.
Other duties taught by a health profession for a specific participant such as taking of vitals, changing colostomy bags, changing of non-sterile dressing and non-sterile bowel and bladder hygiene care.
Assists participants with Independent Activities of Daily Living (IADL's) including but not limited to grocery shopping, cleaning, changing bed linens, laundry, meal preparation and other routine household care as assigned. Reports changes in participant's condition and needs to the Personal Care Supervisor (or designee).
Assists with participant activities in the Day Center, including meal service and clean-up.
Responsible for daily cleaning of the Day Center, including maintenance of cleanliness throughout the day and following all infection control measures related to Day Center cleaning.
Completes appropriate documentation of services provided following Senior CommUnity Care policy.
Follows all Senior CommUnity Care policies and procedures and Occupational Safety and Health Administration (OSHA) safety guidelines.
Maintains the confidentiality of all company procedures, results, and information about participants, clients or families.
Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal Precautions.
Maintains safe working environment. Follows Senior CommUnity Care Safety policies and procedures.
Acts within the scope of his or her authority to practice.

Medical Office Assistant or CNA
Job Summary:
Provide basic patient care, patient care tasks, and education under the direction of the physician. To keep exam rooms stocked and maintain clean facilities for patient care.
Duties/Responsibilities:
Patient Care
Obtain and chart vital statistics
To reconcile medications
Follow workflow protocols
Provide patient instruction and education materials
Make and chart appointments for out-of-office appointments
Prepare, administer and chart medications and injections
Perform skills from checklist
Call patients with results and questions
Order various procedures and labs as directed by physician
Perform EKGs, spirometry, urinalysis, A1cs, Hemocult, INRs, ear lavage, throat swabs and phlebotomy
Clean and supply patient rooms
Autoclave and wrap used instruments
Provide wound care at providers instruction
Assisting Providers
Assist physicians with specialized minor procedures
Keep providers informed of the daily schedule
Keep up with providers pace
Perform functions according to providers orders
Dispense medications at providers direction
Preparing specimens for microscopy
Administrative
Complete encounter form
Add charges to office visits
Track orders

Prepare laundry for pick-up

Experience:
Must be a MedicalAssistant or CertifiedMedical Office Assistant who is capable of working back office position; caring personality with a positive attitude; calm under pressure and exhibit sound judgment; excellent time management and problem solving abilities, good communication skills, and the ability to get along with others and work as a team to assist with all the providers. Must have knowledge of the proper use of medical and lab equipment. Previous medical office experience preferred
Education Requirements:
Have appropriate certification or licensure. Attend Continuing Education programs as requested or as required by the position
Working Conditions:
Works in a well-lighted and ventilated area; primarily standing and walking; light to moderate lifting all the time; heavy lifting occasionally

Job Summary

Responsibilities
