    Substance abuse counselor job at Change Health Systems

    This individual is capable of performing independent work in the therapeutic care, treatment and rehabilitation of persons within the program. This individual plans and conducts therapeutic activities and counsels therapeutically with clients on an individual basis. He/she must be capable of understanding the full continuum of care. In addition, this person must have a broad understanding of how a therapeutic program operates and how each client moves along in the treatment system. Duties may include but are not limited to: carrying out tasks in the therapeutic care, treatment and rehabilitation of persons with chemical dependency and/or mental health problems; facilitating and supervising individual and group counseling; working with other social agencies, FAP, SAP and managed care organizations, hospitals, clinics, courts, prisons and gatekeepers in planning to meet the needs of clients and assist them in utilizing these resources; preparing treatment plans, histories, writing letters, making reports and keeping records; encouraging, reassuring and giving needed support; recommending educational and occupational training and/or possibilities for placement; providing assessment and evaluations based on appropriate criteria. Work is performed in accordance with established regulations, policies and procedures but employees are expected to exercise significant initiative and independent judgement in discharging duties. Work is reviewed by the Program Director and/or Clinical Supervisor through an evaluation of the completion of the assignments through independent and group supervision. Duties and Responsibilities: Continue special training in areas designated by the Supervisor. Keep records such as bio-psycho-history and assessments, evaluations and keep client case records current according to the established guidelines. Ensure collection of urine samples. Attend, participate and when necessary, help facilitate special events such as, retreats, trips, workshops etc. Be a role model for pro-social behaviors. Be available for weekend and/or evening duty. Be responsible for a full client caseload. Attend all staff meetings. Support and demonstrate adherence to CHS's Code of Ethics, Code of Conduct and all personnel policies and procedures. Maintain a valid driver's license and appropriate professional certificates and credentials. All other duties as described in the Unit Unique Job Description or as assigned by the Program Director and/or the Program/Clinical Supervisor. Equipment and Working Conditions All materials and equipment necessary to perform job functions are provided. Staff members can be transferred if it has been decided it is in the best interest of the agency and will enhance services provided. Required Knowledge, Skills and Abilities Working knowledge of the basic principles, practices and procedures of CHS Knowledge of the bio-psycho-social characteristics and manifestations of chemical dependency and/or mental illness. Knowledge of individual and group behavior and the effective ways of working with people to produce ongoing recovery. Ability to work effectively with people and aid them in adjusting to their specific problems. Ability to identify the needs of clients and to gain their confidence and cooperation in assisting them to obtain their goals. Ability to establish and maintain effective working relationships with clients, other members of staff, outside agencies, institutions and the general public.     The successful candidate that will be selected to join a dynamic and growing team of professionals will provide training in substance abuse prevention and sobriety maintenance, incorporating a 12 step focus. Must be able to work with individuals with co-occurring mental illness and/or substance abuse in the community as well as in groups. This position also will provide transportation to and from community activities and AA/NA meetings. Accepts direction and supervision from the psychiatrist as provided.Meets daily with the multidisciplinary team to facilitate review of status of all assigned patients to update treatment plans and complete discharge planning and other referrals as necessary.Conducts family education sessions as scheduled.Conducts family counseling sessions to explain the role of the family in recovery and makes recommendations for discharge planning with the family as required.Assists and participates in any special patient activities and recreational events including appropriate and required documentation of patient responses.Accepts on-call counselor responsibilities on a rotating schedule.Facilitates referral to outside agencies, continuing care agencies, and special counseling agencies based upon patient need to insure continuity of care post-discharge.Consults and interacts with probation officers, attorneys, protective service workers, etc. to act as a patient advocate and insure comprehensiveness of care.Attends and participates in all in-services, staff meetings, counselor meetings, and clinical meetings as required by Hope House.Demonstrates knowledge in and compliance with all emergency and variance reporting requirements.Demonstrates appropriate knowledge and skill in safety, fire safety, and security procedures at Hope House.Attends all mandatory, annual training-re-training to insure continued competency in safety and other issues.Maintains current CPR certification at all times.Attends a minimum of one counseling related course/workshop per year to maintain certification and competency standards.Participates in CQI related functions and monitoring as required by Department Supervisor.Other duties as assigned. IDEAL QUALIFICATIONS: Bachelor's Degree (Master's preferred) from an accredited college or university in counseling, human services, or related field with course work specific to counseling chemical dependency clients. Certification as an addiction counselor in the state of Maryland under the Board of Professional Counselors and Therapists or active pursuit thereof: two years of supervised experience providing chemical dependency counseling services in a health or social work setting. PHYSICAL DEMANDS: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
